    An Evening Spent with Bill van Zwet

    Willem Rutger van Zwet was born in Leiden, the Netherlands, on March 31, 1934. He received his high school education at the Gymnasium Haganum in The Hague and obtained his Masters degree in Mathematics at the University of Leiden in 1959. After serving in the army for almost two years, he obtained his Ph.D. at the University of Amsterdam in 1964, with Jan Hemelrijk as advisor. In 1965, he was appointed Associate Professor of Statistics at the University of Leiden and promoted to Full Professor in 1968. He remained in Leiden until his retirement in 1999, while also serving as Associate Professor at the University of Oregon (1965), William Newman Professor at the University of North Carolina at Chapel Hill (1990--1996), frequent visitor and Miller Professor (1997) at the University of California at Berkeley, director of the Thomas Stieltjes Institute of Mathematics in the Netherlands (1992--1999), and founding director of the European research institute EURANDOM (1997--2000). At Leiden, he was Dean of the School of Mathematics and Natural Sciences (1982--1984).     Effective Dielectric Tensor for Electromagnetic Wave Propagation in Random Media

    We derive exact strong-contrast expansions for the effective dielectric tensor \epeff of electromagnetic waves propagating in a two-phase composite random medium with isotropic components explicitly in terms of certain integrals over the nn-point correlation functions of the medium. Our focus is the long-wavelength regime, i.e., when the wavelength is much larger than the scale of inhomogeneities in the medium. Lower-order truncations of these expansions lead to approximations for the effective dielectric constant that depend upon whether the medium is below or above the percolation threshold. In particular, we apply two- and three-point approximations for \epeff to a variety of different three-dimensional model microstructures, including dispersions of hard spheres, hard oriented spheroids and fully penetrable spheres as well as Debye random media, the random checkerboard, and power-law-correlated materials. We demonstrate the importance of employing nn-point correlation functions of order higher than two for high dielectric-phase-contrast ratio. We show that disorder in the microstructure results in an imaginary component of the effective dielectric tensor that is directly related to the {\it coarseness} of the composite, i.e., local volume-fraction fluctuations for infinitely large windows. The source of this imaginary component is the attenuation of the coherent homogenized wave due to scattering. We also remark on whether there is such attenuation in the case of a two-phase medium with a quasiperiodic structure.Comment: 40 pages, 13 figure

    Theory of the Half-Polarized Quantum Hall States

    We report a theoretical analysis of the half-polarized quantum Hall states observed in a recent experiment. Our numerical results indicate that the ground state energy of the quantum Hall ν=2/3\nu= 2/3 and ν=2/5\nu= 2/5 states versus spin polarization has a downward cusp at half the maximal spin polarization. We map the two-component fermion system onto a system of excitons and describe the ground state as a liquid state of excitons with non-zero values of exciton angular momentum.Comment: 4 pages (RevTeX), 3 figures (PostScript), added reference

    Ability of verbal autopsy data to detect deaths due to uncontrolled hyperglycaemia:testing existing methods and development and validation of a novel weighted score

    Objectives: Verbal autopsy (VA) is a useful tool to ascertain cause of death where no other mechanisms exist. We aimed to assess the utility of VA data to ascertain deaths due to uncontrolled hyperglycaemia and to develop a weighted score (WS) to specifically identify cases. Cases were identified by a study or site physician with training in diabetes. These diagnoses were also compared with diagnoses produced by a standard computer algorithm (InterVA-4). Setting: This study was done using VA data from the Health and Demographic Survey sites in Agincourt in rural South Africa. Validation of the WS was done using VA data from Karonga in Malawi. Participants: All deaths from ages 1 to 49 years between 1992 and 2015 and between 2002 and 2016 from Agincourt and Karonga, respectively. There were 8699 relevant deaths in Agincourt and 1663 in Karonga. Results: Of the Agincourt deaths, there were 77 study physician classified cases and 58 computer algorithm classified cases. Agreement between study physician classified cases and computer algorithm classified cases was poor (Cohen’s kappa 0.14). Our WS produced a receiver operator curve with area under the curve of 0.952 (95% CI 0.920 to 0.985). However, positive predictive value (PPV) was below 50% when the WS was applied to the development set and the score was dominated by the necessity for a premortem diagnosis of diabetes. Independent validation showed the WS performed reasonably against site physician classified cases with sensitivity of 86%, specificity of 99%, PPV of 60% and negative predictive value of 99%. Conclusion: Our results suggest that widely used VA methodologies may be missing deaths due to uncontrolled hyperglycaemia. Our WS may offer improved ability to detect deaths due to uncontrolled hyperglycaemia in large populations studies where no other means exist

    Type-Decomposition of a Pseudo-Effect Algebra

    The theory of direct decomposition of a centrally orthocomplete effect algebra into direct summands of various types utilizes the notion of a type-determining (TD) set. A pseudo-effect algebra (PEA) is a (possibly) noncommutative version of an effect algebra. In this article we develop the basic theory of centrally orthocomplete PEAs, generalize the notion of a TD set to PEAs, and show that TD sets induce decompositions of centrally orthocomplete PEAs into direct summands.Comment: 18 page

    Transport properties of heterogeneous materials derived from Gaussian random fields: Bounds and Simulation

    We investigate the effective conductivity (σe\sigma_e) of a class of amorphous media defined by the level-cut of a Gaussian random field. The three point solid-solid correlation function is derived and utilised in the evaluation of the Beran-Milton bounds. Simulations are used to calculate σe\sigma_e for a variety of fields and volume fractions at several different conductivity contrasts. Relatively large differences in σe\sigma_e are observed between the Gaussian media and the identical overlapping sphere model used previously as a `model' amorphous medium. In contrast σe\sigma_e shows little variability between different Gaussian media.Comment: 15 pages, 14 figure

    Diabetes in sub-Saharan Africa: from clinical care to health policy.

    Rapid demographic, sociocultural, and economic transitions are driving increases in the risk and prevalence of diabetes and other non-communicable diseases (NCDs) in sub-Saharan Africa. The impacts of these transitions and their health and economic consequences are evident. Whereas, in 1990, the leading causes of death in sub-Saharan Africa were HIV/AIDS, lower respiratory infections, diarrhoeal diseases, malaria, and vaccine-preventable diseases in children, in more recent years, cardiovascular diseases and their risk factors are replacing infectious diseases as the leading causes of death in this region, and rates of increase of cardiovascular risk factors are predicted to be greater in sub-Saharan Africa than in other parts of the world. Thus, sub-Saharan Africa—which contains a high proportion of the world\u27s least developed countries—will face the multifaceted challenge of dealing with a high burden of infectious diseases and diseases of poverty, while also addressing the increasing burden of cardiovascular disease and its risk factors. At present, many of the health systems in sub-Saharan Africa struggle to cope with infectious diseases. Meeting the goals of the UN high-level meeting on NCDs (to reduce premature mortality from NCDs by 25% by 2025) and Sustainable Development Goals (SDGs; to reduce premature mortality from NCDs by a third by 2030) requires a coordinated approach within countries, which starts with a firm consideration of disease burden, needs, and priorities

    The Strongly Coupled 't Hooft Model on the Lattice

    We study the strong coupling limit of the one-flavor and two-flavor massless 't Hooft models, largeNclarge-{\cal N}_c-color QCD2QCD_2, on a lattice. We use staggered fermions and the Hamiltonian approach to lattice gauge theories. We show that the one-flavor model is effectively described by the antiferromagnetic Ising model, whose ground state is the vacuum of the gauge model in the infinite coupling limit; expanding around this ground state we derive a strong coupling expansion and compute the lowest lying hadron masses as well as the chiral condensate of the gauge theory. Our lattice computation well reproduces the results of the continuum theory. Baryons are massless in the infinite coupling limit; they acquire a mass already at the second order in the strong coupling expansion in agreement with the Witten argument that baryons are the QCDQCD solitons. The spectrum and chiral condensate of the two-flavor model are effectively described in terms of observables of the quantum antiferromagnetic Heisenberg model. We explicitly write the lowest lying hadron masses and chiral condensate in terms of spin-spin correlators on the ground state of the spin model. We show that the planar limit (Nc{\cal N}_c\longrightarrow \infty) of the gauge model corresponds to the large spin limit (SS\longrightarrow \infty) of the antiferromagnet and compute the hadron mass spectrum in this limit finding that, also in this model, the pattern of chiral symmetry breaking of the continuum theory is well reproduced on the lattice.Comment: LaTex, 25 pages, no figure
