    Observing bullying at school: The mental health implications of witness status

    This study explores the impact of bullying on the mental health of students who witness it. A representative sample of 2,002 students aged 12 to 16 years attending 14 schools in the United Kingdom were surveyed using a questionnaire that included measures of bullying at school, substance abuse, and mental health risk. The results suggest that observing bullying at school predicted risks to mental health over and above that predicted for those students who were directly involved in bullying behavior as either a perpetrator or a victim. Observing others was also found to predict higher risk irrespective of whether students were or were not victims themselves. The results are discussed with reference to past research on bystander and witness behavior

    Numerical Study of Vortex Reconnection

    With a Biot-Savart model of vortex filaments to provide initial conditions, a finite difference scheme for the incompressible Navier-Stokes equation is used in the region of closest approach of two vortex rings. In the Navier-Stokes solution, we see that the low pressure which develops between the interacting vorticity regions causes the distortion of the initially circular vortex cross section and forces the rearrangement of vorticity on a convective time scale which is much faster than that estimated from viscous transport

    Vortex Reconnection as the Dissipative Scattering of Dipoles

    We propose a phenomenological model of vortex tube reconnection at high Reynolds numbers. The basic picture is that squeezed vortex lines, formed by stretching in the region of closest approach between filaments, interact like dipoles (monopole-antimonopole pairs) of a confining electrostatic theory. The probability of dipole creation is found from a canonical ensemble spanned by foldings of the vortex tubes, with temperature parameter estimated from the typical energy variation taking place in the reconnection process. Vortex line reshuffling by viscous diffusion is described in terms of directional transitions of the dipoles. The model is used to fit with reasonable accuracy experimental data established long ago on the symmetric collision of vortex rings. We also study along similar lines the asymmetric case, related to the reconnection of non-parallel vortex tubes.Comment: 8 pages, 3 postscript figure

    Patients’ preferences for nutrition-related health outcomes in liver disease : a preliminary study using an electronic questionnaire

    Background: Patients with liver disease frequently have nutritional problems but intervening to improve these is challenging. Healthcare interventions that respond to patients’ needs are associated with better health outcomes but no studies investigating patients’ preferences for nutrition-related outcomes in liver disease have been published. The aim of this study was to identify nutrition-related health outcomes that are important to patients with liver disease. Methodology: An electronic questionnaire was devised and reviewed by patients and dietitians with relevant experience. It comprised Likert scale and open questions focussing on six domains considered pertinent to patients with liver disease. An invitation to participate was posted on the website of a national liver charity and sent to liver patient support groups. Results: Fifty-one patients participated (22 men / 29 women). Responses indicated a wide range of preferred nutrition-related outcomes with those identified as very important most frequently focussing on gaining knowledge about which foods to eat more or less of, and on understanding why nutrition is important in liver disease. Women tended to score outcomes as more important than men. Participants who considered themselves overweight scored outcomes on body size and shape as more important than those with other nutritional problems. Additional outcomes were identified and included increased knowledge of healthy eating, interactions between medication and food, and supplementation. Conclusions: The study identified a wide range of nutrition-related outcomes that were important to this small sample of patients with liver disease and these may be useful to guide the direction of future nutrition-related management.Peer reviewedFinal Accepted Versio

    Non-linear model equation for three-dimensional Bunsen flames

    The non linear description of laminar premixed flames has been very successful, because of the existence of model equations describing the dynamics of these flames. The Michelson Sivashinsky equation is the most well known of these equations, and has been used in different geometries, including three-dimensional quasi-planar and spherical flames. Another interesting model, usually known as the Frankel equation,which could in principle take into account large deviations of the flame front, has been used for the moment only for two-dimensional expanding and Bunsen flames. We report here for the first time numerical solutions of this equation for three-dimensional flames

    Oncology-led early identification of nutritional risk: a pragmatic, evidence-based protocol (PRONTO)

    Simple Summary Early identification of patients on antineoplastic therapy who are at risk for or already malnourished is critical for optimizing treatment success. Malnourished patients are at increased risk for being unable to tolerate the most effective 'level' and 'duration' of treatment, with grave implications for both the short- (during treatment) and long-term outcomes. Herein, we provide a practical PROtocol for NuTritional risk in Oncology (PRONTO) to enable oncologists to identify patients with or at risk of malnutrition for further evaluation and follow-up with members of the multidisciplinary care team (MDT). Additional guidance is included on the oncologist-led provision of nutritional support if referral to a dietary service is not available. Nutritional issues, including malnutrition, low muscle mass, sarcopenia (i.e., low muscle mass and strength), and cachexia (i.e., weight loss characterized by a continuous decline in skeletal muscle mass, with or without fat loss), are commonly experienced by patients with cancer at all stages of disease. Cancer cachexia may be associated with poor nutritional status and can compromise a patient's ability to tolerate antineoplastic therapy, increase the likelihood of post-surgical complications, and impact long-term outcomes including survival, quality of life, and function. One of the primary nutritional problems these patients experience is malnutrition, of which muscle depletion represents a clinically relevant feature. There have been recent calls for nutritional screening, assessment, treatment, and monitoring as a consistent component of care for all patients diagnosed with cancer. To achieve this, there is a need for a standardized approach to enable oncologists to identify patients commencing and undergoing antineoplastic therapy who are or who may be at risk of malnutrition and/or muscle depletion. This approach should not replace existing tools used in the dietitian's role, but rather give the oncologist a simple nutritional protocol for optimization of the patient care pathway where this is needed. Given the considerable time constraints in day-to-day oncology practice, any such approach must be simple and quick to implement so that oncologists can flag individual patients for further evaluation and follow-up with appropriate members of the multidisciplinary care team. To enable the rapid and routine identification of patients with or at risk of malnutrition and/or muscle depletion, an expert panel of nutrition specialists and practicing oncologists developed the PROtocol for NuTritional risk in Oncology (PRONTO). The protocol enables the rapid identification of patients with or at risk of malnutrition and/or muscle depletion and provides guidance on next steps. The protocol is adaptable to multiple settings and countries, which makes implementation feasible by oncologists and may optimize patient outcomes. We advise the use of this protocol in countries/clinical scenarios where a specialized approach to nutrition assessment and care is not available

    Predicting the safety and efficacy of butter therapy to raise tumour pHe: an integrative modelling study

    Background: Clinical positron emission tomography imaging has demonstrated the vast majority of human cancers exhibit significantly increased glucose metabolism when compared with adjacent normal tissue, resulting in an acidic tumour microenvironment. Recent studies demonstrated reducing this acidity through systemic buffers significantly inhibits development and growth of metastases in mouse xenografts.\ud \ud Methods: We apply and extend a previously developed mathematical model of blood and tumour buffering to examine the impact of oral administration of bicarbonate buffer in mice, and the potential impact in humans. We recapitulate the experimentally observed tumour pHe effect of buffer therapy, testing a model prediction in vivo in mice. We parameterise the model to humans to determine the translational safety and efficacy, and predict patient subgroups who could have enhanced treatment response, and the most promising combination or alternative buffer therapies.\ud \ud Results: The model predicts a previously unseen potentially dangerous elevation in blood pHe resulting from bicarbonate therapy in mice, which is confirmed by our in vivo experiments. Simulations predict limited efficacy of bicarbonate, especially in humans with more aggressive cancers. We predict buffer therapy would be most effectual: in elderly patients or individuals with renal impairments; in combination with proton production inhibitors (such as dichloroacetate), renal glomular filtration rate inhibitors (such as non-steroidal anti-inflammatory drugs and angiotensin-converting enzyme inhibitors), or with an alternative buffer reagent possessing an optimal pK of 7.1–7.2.\ud \ud Conclusion: Our mathematical model confirms bicarbonate acts as an effective agent to raise tumour pHe, but potentially induces metabolic alkalosis at the high doses necessary for tumour pHe normalisation. We predict use in elderly patients or in combination with proton production inhibitors or buffers with a pK of 7.1–7.2 is most promising

    We analyze the diffusion of a Brownian particle in a fluid under stationary flow. By using the scheme of non-equilibrium thermodynamics in phase space, we obtain the Fokker-Planck equation which is compared with others derived from kinetic theory and projector operator techniques. That equation exhibits violation of the fluctuation dissipation-theorem. By implementing the hydrodynamic regime described by the first moments of the non-equilibrium distribution, we find relaxation equations for the diffusion current and pressure tensor, allowing us to arrive at a complete description of the system in the inertial and diffusion regimes. The simplicity and generality of the method we propose, makes it applicable to more complex situations, often encountered in problems of soft condensed matter, in which not only one but more degrees of freedom are coupled to a non-equilibrium bath.Comment: 10 pages, accepted in Phys.     Flame front propagation IV: Random Noise and Pole-Dynamics in Unstable Front Propagation II

    The current paper is a corrected version of our previous paper arXiv:adap-org/9608001. Similarly to previous version we investigate the problem of flame propagation. This problem is studied as an example of unstable fronts that wrinkle on many scales. The analytic tool of pole expansion in the complex plane is employed to address the interaction of the unstable growth process with random initial conditions and perturbations. We argue that the effect of random noise is immense and that it can never be neglected in sufficiently large systems. We present simulations that lead to scaling laws for the velocity and acceleration of the front as a function of the system size and the level of noise, and analytic arguments that explain these results in terms of the noisy pole dynamics.This version corrects some very critical errors made in arXiv:adap-org/9608001 and makes more detailed description of excess number of poles in system, number of poles that appear in the system in unit of time, life time of pole.
