    Get PDF
A diffuse interstitial infiltrative pattern of lung metastasis in a patient with malignant melanoma is rare and can be confused with benign conditions such as pulmonary edema or drug-induced pneumonitis. We experienced a case of diffuse interstitial infiltrative lung metastasis in malignant melanoma in a 37-year-old man. This case was confirmed by a transbronchial lung biopsy. We herein describe the findings on CT and positron emission tomography scan

<p>Abstract</p> <p>Background</p> <p>The aim of this study was to determine the prevalence of human papillomavirus (HPV) and 15 species that cause sexually transmitted infections (STIs) in negative cytology. In addition, we compared the diagnostic performance of multiplex polymerase chain reaction (PCR) with widely available techniques used to detect HPV.</p> <p>Methods</p> <p>We recruited 235 women of reproductive age who had negative cytology findings in a liquid-based cervical smear. STIs were identified by multiplex PCR, and HPV genotypes by multiplex PCR, hybrid capture 2, and DNA microaray; discordant results were analyzed by direct sequencing.</p> <p>Results</p> <p>Approximately 96.6% of patients with negative cytology results were positive for pathogens that cause STIs. The pathogens most frequently detected were <it>Gardnerella vaginalis, Ureaplasma urealyticum</it>. The incidence of HPV in negative cytology was 23.3%. Low-risk HPV infection was significantly correlated with <it>Chalmaydia trachomatis</it>, and high-risk HPV infection was significantly correlated with <it>Group β streptococcus</it>. The analytical sensitivities of the multiplex PCR and DNA microarray were higher than 80%, and the analytical specificity was nearly 100% for all tests.</p> <p>Conclusions</p> <p>Multiplex PCR yielded results that most of patients with negative cytology were positive for pathogens that cause STIs, and were more similar to that of DNA microarray, than that of hybrid capture 2 in terms of analytical sensitivity and prediction value of HPV infection.</p

Improved reactor regulating system logical architecture using genetic algorithm

    No full text
An improved Reactor Regulating System (RRS) logic architecture, which is combined with genetic algorithm (GA), is implemented in this work. It is devised to provide an optimal solution to the current RRS. The current system works desirably and has contributed to safe and stable nuclear power plant operation. However, during the ascent and descent section of the reactor power, the RRS output reveals a relatively high steady-state error, and the output also carries a considerable level of overshoot. In an attempt to consolidate conservatism and minimize the error, this work proposes to apply GA to RRS and suggests reconfiguring the system. Prior to the use of GA, reverse engineering is implemented to build a Simulink-based RRS model. Reengineering is followed to produce a newly configured RRS to generate an output that has a reduced steady-state error and diminished overshoot level. A full-scope APR1400 simulator is used to examine the dynamic behaviors of RRS and to build the RRS Simulink model

Expression of androgen receptors and inhibin/activin alpha and betaA subunits in breast apocrine lesions

The importance of androgens and their receptors inhibin and activin remains unknown for mammary epithelial cells. We investigated the role of these hormones in breast apocrine lesions (BAL) using immunohistochemistry to study androgen receptors (AR) and the inhibin/activin alpha and betaA subunits. Forty-two cases of BAL were evaluated, including 22 cases of fibrocystic disease (FCD) showing prominent apocrine changes, 10 intraductal papillomas with extensive apocrine metaplasia, 5 cases of apocrine carcinoma in situ (CIS), and 5 cases of apocrine carcinoma. Fifty non-apocrine lesions were included as controls: 20 cases of FCD, 5 cases of DCIS, and 25 cases of invasive ductal carcinoma. AR was more frequently expressed in BAL than in non-apocrine lesions (p=0.001). AR expression was not related to tumor progression. AR showed a significant positive correlation with betaA subunits (r=0.832, p<0.001), and an inverse correlation with alpha subunits (r=-0.233). The alpha and betaA subunits demonstrated a significant inverse correlation with each other (r=-0.271, p=0.0048). As the expression of the alpha and betaA subunits reflects inhibin and activin A, respectively, AR and activin A may be implicated in apocrine morphogenesis, but not in tumor progression

Multilayer Epitaxial Growth of Lead Phthalocyanine and C70 Using CuBr as a Templating Layer for Enhancing the Efficiency of Organic Photovoltaic Cells

    No full text
The molecular orientation and crystallinity of donor and acceptor molecules are important for high-efficiency organic photovoltaic cells (OPVs) because they significantly influence both the absorption of light and charge-transport characteristics. We report that the templating effect extends to multilayers to increase the crystallinity and to modify the orientation of the crystals of lead phthalocyanine (PbPc) and C70 layers at the same time by adopting CuBr as a new templating layer on indium tin oxide (ITO). The formation of a monoclinic phase with a preferred orientation of (320) for PbPc and a fcc phase with a preferred orientation of (220) for C70 on the PbPc layer is revealed by X-ray diffraction (XRD) patterns. The multilayer epitaxy results in an increase of the exciton diffusion lengths from 5.6 to 8.8 nm for PbPc and from 6.9 to 13.8 nm for C70 to enhance the power conversion efficiency (PCE) of the planar heterojunction OPVs composed of PbPc and C70 from 1.4 to 2.3%. The quasi-epitaxy model is proposed to explain the multilayer epitaxy

Clinical practice guidelines for optimizing bone health in Korean children and adolescents

    No full text
The Committee on Pediatric Bone Health of the Korean Society of Pediatric Endocrinology has newly developed evidence-based clinical practice guidelines for optimizing bone health in Korean children and adolescents. These guidelines present recommendations based on the Grading of Recommendations, which includes the quality of evidence. In the absence of sufficient evidence, conclusions were based on expert opinion. These guidelines include processes of bone acquisition, definition, and evaluation of low bone mineral density (BMD), causes of osteoporosis, methods for optimizing bone health, and pharmacological treatments for enhancing BMD in children and adolescents. While these guidelines provide current evidence-based recommendations, further research is required to strengthen these guidelines.N

Enhancement of the Fill Factor through an Increase of the Crystallinity in Fullerene-Based Small-Molecule Organic Photovoltaic Cells

We report that the crystallinity of C70 is improved significantly if CuI is used as a templating layer, leading to remarkable enhancement of hole mobilities from 8.32 × 10–6 to 3.26 × 10–5 cm2/(V s). As a result, the use of the templating layer in C70-based solar cells with low donor concentration resulted in significant improvement of the fill factor from 0.51 to 0.57 and the power conversion efficiency from 5.56% to 6.23% under simulated AM 1.5G, 1 sun irradiation. This result demonstrates that the CuI templating layer is effective at improving the crystallinity of the fullerene derivatives as well as the donor materials

Early inner retinal thinning and cardiovascular autonomic dysfunction in type 2 diabetes.

BACKGROUND:To investigate changes in the neural retina according to the presence of retinal nerve fiber layer (RNFL) defects in type 2 diabetes, and to determine the association between inner retina thickness and the severity of diabetic complications. METHODS:We studied non-glaucomatous patients with type 2 diabetes and control subjects Circumpapillary RNFL and macula ganglion cell-inner plexiform layer (GCIPL) thicknesses were measured by spectral-domain optical coherence tomography. In patients with type 2 diabetes, a cardiovascular autonomic function test (AFT) was performed, which included the heart rate parameter of beat-beat variation-with deep breathing, in response to the Valsalva maneuver, and on postural change from lying to standing. The results of each test were scored as 0 for normal and 1 for abnormal. A total AFT score of 1 was defined as early cardiovascular autonomic neuropathy (CAN), and an AFT score≥ 2 as definite CAN. RESULTS:We compared control eyes (n = 70), diabetic eyes with RNFL defects (n = 47), and eyes without RNFL defects (n = 30). The average RNFL and GCIPL thicknesses were significantly different among groups (all, P<0.05). On post-hoc testing, diabetic eyes with RNFL defects had a significantly thinner average GCIPL thickness than those without RNFL defects. On multivariate analyses, significantly thinner average GCIPL was seen in early CAN staging (B = -4.32, P = 0.016) and in definite CAN staging (B = -10.33, P<0.001), compared with no CAN involvement, after adjusting for confounding parameters. CONCLUSIONS:Cardiovascular autonomic dysfunction was associated with early neurodegenerative changes in type 2 diabetes
