    The value of Tc-99m (V) dimercaptosuccinic acid in detecting intra-abdominal infection: compared with gallium scan

    Objective Gallium-67 (Ga-67) and labeled leukocytes are useful in the detection of an unknown infectious source. However, the delay in the diagnosis of a Ga-67 citrate scan (gallium scan) and the complicated labeling technique of a leukocyte scan are major drawbacks to their clinical use. Recently, Tc-99m (V) dimercaptosuccinic acid (DMSA) has been found to be very useful in the detection of infection. Tc-99m (V) DMSA is inexpensive, easy to prepare. and provides a result within hours. In this study. we evaluated the potential of Tc-99m (V) DMSA scan (DMSA scan) in the detection of intra-abdominal infection. Methods A total of 33 patients who suffered from an unknown cause of fever after colorectal surgery were enrolled in this study. All patients received both a gallium scan and a DMSA scan. DMSA scintigraphy was performed 3-4 h after an injection of 740 MBq (20 m Ci) of Tc-99m DMSA. After completion of the DMSA image, 111 M Bq (3 mCi) of Ga-67 citrate was injected intravenously. Gallium scintigraphy was performed after 24 h and later as needed. Results Of the 33 patients, 17 (51.5%) were diagnosed with intra-abdominal abscesses. For DMSA scans, the sensitivity, specificity, and overall accuracy were 88.2%, 93.7%, and 90.9%, respectively. For gallium scans, the diagnostic sensitivity, specificity, and accuracy were 100%, 87.5%, and 93.9%, respectively. No statistical difference was found in the diagnostic accuracy between these two diagnostic modalities using Fisher's exact test. Conclusions DMSA scan is a useful alternative to gallium scan in the detection of intra-abdominal infection in patients with colorectal surgery because Tc-99m DMSA is inexpensive, easy to prepare, and most importantly the result can be obtained within hours

    Trajectory of functional outcome and health status after moderate-to-major trauma in Hong Kong: A prospective 5 year cohort study

    Background Trauma care systems in Asia have been developing in recent years, but there has been little long-term outcome data from injured survivors. This study aims to evaluate the trajectory of functional outcome and health status up to five years after moderate to major trauma in Hong Kong. Methods We report the five year follow up results of a multicentre, prospective cohort from the trauma registries of three regional trauma centres in Hong Kong. The original cohort recruited 400 adult trauma patients with ISS ≥ 9. Telephone follow up was conducted longitudinally at seven time points, and the extended Glasgow Outcome Scale (GOSE) and Short-Form 36 (SF36) were tracked. Results 119 out of 309 surviving patients (39%) completed follow up after 5 years. The trajectory of GOSE, PCS and MCS showed gradual improvements over the seven time points. 56/119 (47.1%) patients reported a GOSE = 8 (upper good recovery), and the mean PCS and MCS was 47.8 (95% CI 45.8, 49.9) and 55.8 (95% CI 54.1, 57.5) respectively at five years. Univariate logistic regression showed change in PCS - baseline to 1 year and 1 year to 2 years, and change in MCS - baseline to 1 year were associated with GOSE = 8 at 5 years. Linear mixed effects model showed differences in PCS and MCS were greatest between 1-month and 6-month follow up. Conclusions After injury, the most rapid improvement in PCS and MCS occurred in the first six to 12 months, but further recovery was still evident for MCS in patients aged under 65 years for up to five years

    Columbus’ footprint in Hispaniola:A paleoenvironmental record of indigenous and colonial impacts on the landscape of the central Cibao Valley, northern Dominican Republic

    The 1100-year sedimentary record of Laguna Biajaca reveals human-driven landscape changes in the central Cibao Valley, Dominican Republic, Hispaniola. This sediment-filled cutoff meander is located in close proximity to pre-Colonial archaeological sites and a Colonial urban hub. It provided a nutrient-rich floodable locus for agricultural activities for indigenous communities and for the first introduction of Old World crops and cattle in the Americas. Integration of paleoecological proxies revealed the formation of a clear-water body surrounded by a palm-rich forested landscape around 1100 cal yr BP. Changes in the drainage system were linked to human-driven deforestation, which also changed the composition of the vegetation and fungal communities around the site between AD 1150 and 1500 (800 and 700 cal yr BP). Pre-Colonial modifications of the landscape were primarily the result of fire-use and small-scale clearings. Crop cultivation developed between AD 1250 and 1450 (700–500 cal yr BP). Within decades after Columbus’ arrival in Hispaniola in AD 1492, the first impacts of European colonization included the abandonment of indigenous sites and the introduction of Old World domesticated animals. During the 15th and 16th centuries the area underwent intensive land-clearing that allowed for larger scale crop cultivation. An increase of aquatic vegetation points to sediment-filling around AD 1700 (250 cal yr BP). At that time, cattle breeding expanded and rapidly provoked eutrophication while, concurrently, monocultures became regionally established. This paper provides a framework of past environmental dynamics and offers an opportunity to place archaeological findings in a context of natural and anthropogenic change

    Impact loading during distracted running before and after auditory gait retraining.

    Visual feedback gait retraining has been reported to successfully reduce impact loading in runners, even when the runners were distracted. However, auditory feedback is more feasible in real life application. Hence, this study compared the peak positive acceleration (PPA), vertical average (VALR) and instantaneous (VILR) loading rate during distracted running before and after a course of auditory feedback gait retraining in 16 runners. The runners were asked to land with softer footfalls with and without auditory feedback. Low or high sound pitch was generated according to the impact of particular footfall, when compared with the preset target. Runners then received a course of auditory gait retraining, and after the gait retraining, runners completed a reassessment. Runners before gait retraining exhibited lower PPA, VALR and VILR with augmented auditory feedback (p0.104). A small effect of auditory feedback on VILR in runners after gait retraining was observed (p=0.032). Real time auditory feedback gait retraining is effective in impact loading reduction, even when the runners were distracted

    Schwinger boson theory of anisotropic ferromagnetic ultrathin films

    Ferromagnetic thin films with magnetic single-ion anisotropies are studied within the framework of Schwinger bosonization of a quantum Heisenberg model. Two alternative bosonizations are discussed. We show that qualitatively correct results are obtained even at the mean-field level of the theory, similar to Schwinger boson results for other magnetic systems. In particular, the Mermin-Wagner theorem is satisfied: a spontaneous magnetization at finite temperatures is not found if the ground state of the anisotropic system exhibits a continuous degeneracy. We calculate the magnetization and effective anisotropies as functions of exchange interaction, magnetic anisotropies, external magnetic field, and temperature for arbitrary values of the spin quantum number. Magnetic reorientation transitions and effective anisotropies are discussed. In particular, we will overview template assisted placement that utilizes physical, molecular, or electrostatic templates, DNA-programmed assembly, placement using dielectrophoresis, approaches for non-close-packed assembly of spherical particles, and recent development of focused placement schemes including electrostatic funneling, focused placement via molecular gradient patterns, electrodynamic focusing of charged aerosols, and others
