    Oligodendroglioma in a patient with AIDS: case report and review of the literature

    In the last years, new techniques of neuroimages and histopathological methods have been added to the management of cerebral mass lesions in patients with AIDS. Stereotactic biopsy is necessary when after 14 days of empirical treatment for Toxoplasma gondii encephalitis there is no clinical or neuroradiologic improvement. We report a woman with AIDS who developed a single focal brain lesion on the right frontal lobe. She presented a long history of headache and seizures. After two weeks of empirical treatment for toxoplasma encephalitis without response, a magnetic resonance image with spectroscopy was performed and showed a tumoral pattern with a choline peak, diminished of N-acetyl-aspartate and presence of lactate. A stereotactic biopsy was performed. Histopathological diagnosis was a diffuse oligodendroglioma type A. A microsurgical resection of the tumor was carried out and antiretroviral treatment was started. To date she is in good clinical condition, with undetectable plasma viral load and CD4 T cell count >; 200 cell/uL.En los últimos años, las nuevas técnicas de neuroimágenes y diversos métodos de diagnóstico histopatológico se han agregado al manejo clínico de las lesiones de masa cerebral ocupante en los pacientes con sida. La biopsia estereotáxica es necesaria cuando, luego de dos semanas de tratamiento empírico para toxoplasmosis cerebral, no se comprueba mejoría clínica ni neurorradiológica. Presentamos una paciente con sida que desarrolló una lesión cerebral a nivel del lóbulo frontal derecho. Como antecedente refirió una larga historia de cefalea y convulsiones. La resonancia nuclear magnética con espectroscopia de voxel único ubicado a nivel de la lesión mostró un patrón de lesión tumoral con pico de colina, déficit de N-acetil-aspartato y presencia de ácido láctico. La biopsia estereotáxica y el estudio histopatológico permitieron arribar al diagnóstico de oligodendroglioma difuso de tipo A. Se le efectuó resección por microcirugía y tratamiento antirretroviral de alta eficacia. Actualmente la paciente se encuentra en buen estado clínico, con carga viral indetectable y recuento de linfocitos T CD4 + >; de 200 cél/uL

    Cryptococcus neoformans in feces of an AIDS patient

    Se comunica el hallazgo de levaduras capsuladas en materias fecales recolectadas en forma seriada y remitidas al Laboratorio de Parasitología del Hospital Muñiz, para su examen especíLco. Paciente masculino de 49 años de edad, VIH positivo en el que se realizó el diagnóstico de meningoencefalitis por Cryptococcus neoformans mediante el examen del líquido cefalorraquídeo (LCR). El examen parasitológico de materia fecal realizado por haber presentado diarrea durante la internación, resultó negativo para enteroparásitos. La microscopia en fresco del concentrado reveló levaduras redondas, de pared gruesa y rodeadas de un halo claro producido por el desplazamiento de la materia fecal circundante, compatibles con Cryptococcus neoformans. En la muestra de esputo se observaron levaduras capsuladas, y en el cultivo del mismo se obtuvo el desarrollo de C. neoformans. El hallazgo de las levaduras capsuladas en la materia fecal se interpreta como producto de la deglución de secreciones respiratorias en un paciente portador de una localización pulmonar de esta micosis.The finding of capsulated yeasts in feces collected in a serial way for a parasitological study was reported. A 49- year-old VIH positive man was admitted at ward on account of fever, dysuria and back pain. Criptococal meningitis diagnosis was made and speci1c treatment was started. Pneumocystis jiroveci pneumonia could not be assessed. Bacteriological culture of urine samples was negative. A parasitological study of feces, performed on account of mild diarrhoea, was negative for enteroparasites. Microscopy of concentrated fresh sample revealed rounded thick wall yeasts surrounded by a clear halo, resulting from a sliding movement of the feces around it, consistent with Cryptococcus neoformans. Direct examination with optic microscope of a sputum sample showed capsulated yeasts which developed C. neoformans in culture. These findings are in accordance with respiratory secretions swallowed by the patient, who carried a pulmonary localization of this disseminated mycosis.Facultad de Ciencias Exacta

    Mycobacteria other than tuberculosis (MOTT) have a low incidence as pathogens in human pathology. The most frequent clinical expression is the disseminated disease in subjects with compromised cellular immunity. Bacteriological characteristics in culture can generate confusion with other pathogens, which delays the appropriate diagnosis and treatment. We present a case of a disseminated infection due to Mycobacterium chelonae with scleritis, spondylodiscitis and spinal epidural abscess in a man with a medical background of cellular immunity deficit induced by therapeutic drugs. The antibiotic scheme of twenty-one weeks, during the follow-up period, controlled the infection, however, the optimum duration of treatment has not been established

    Neoplasmas da musculatura lisa são mais freqüentes em crianças infectadas pelo vírus da imunodeficiência humana do que em adultos HIV-soropositivos. Leiomioma endobronquial é um tumor benigno em pacientes adultos infectados por HIV. Vírus Epstein-Barr (EBV) tem sido implicado na patogenia destes tumores. Descrevemos paciente adulto infectado pelo HIV com atelectasia do lobo pulmonar superior esquerdo como primeira manifestação clínica de leiomioma intrabronquial. Neste caso não pudemos demonstrar a associação com EBV. Nosso relato sugere que tumores de musculatura lisa como leiomioma deveriam ser incluídos no diagnóstico diferencial de massas endobronquiais em pacientes com AIDS.Smooth muscle neoplasms are more frequent in human immunodeficiency infected children than in HIV seropositive adults. Endobronchial leiomyoma is a rare benign tumor in HIV infected adult patients. Epstein-Barr virus (EBV) has been implicated in the pathogenesis of these tumors. Here we describe an adult patient with HIV infection with atelectasis of the left upper pulmonary lobe as the first clinical expression of an intrabronchial leiomyoma. In this case, we can not show the association with EBV. Our report suggests that smooth muscle tumors as leiomyoma should be included in the differential diagnosis of endobronchial masses in AIDS patients

    Spinal epidural abscess (SEA) is a rare infectious disorder that often has delayed diagnosis and is associated with significant morbidity and mortality rates. We present a case of an AIDS patient with a SEA due to Mycobacterium tuberculosis. This type of SEA in AIDS patients is characterized by localized spinal pain and prolonged fever. Magnetic resonance imaging is the method of choice in the diagnostic process. Early diagnosis, followed by specific therapy (surgical decompression combined with antituberculous drugs), is necessary to improve the prognosis of these kinds of patients
