Federated Recommendation via Hybrid Retrieval Augmented Generation

Federated Recommendation (FR) emerges as a novel paradigm that enables privacy-preserving recommendations. However, traditional FR systems usually represent users/items with discrete identities (IDs), suffering from performance degradation due to the data sparsity and heterogeneity in FR. On the other hand, Large Language Models (LLMs) as recommenders have proven effective across various recommendation scenarios. Yet, LLM-based recommenders encounter challenges such as low inference efficiency and potential hallucination, compromising their performance in real-world scenarios. To this end, we propose GPT-FedRec, a federated recommendation framework leveraging ChatGPT and a novel hybrid Retrieval Augmented Generation (RAG) mechanism. GPT-FedRec is a two-stage solution. The first stage is a hybrid retrieval process, mining ID-based user patterns and text-based item features. Next, the retrieved results are converted into text prompts and fed into GPT for re-ranking. Our proposed hybrid retrieval mechanism and LLM-based re-rank aims to extract generalized features from data and exploit pretrained knowledge within LLM, overcoming data sparsity and heterogeneity in FR. In addition, the RAG approach also prevents LLM hallucination, improving the recommendation performance for real-world users. Experimental results on diverse benchmark datasets demonstrate the superior performance of GPT-FedRec against state-of-the-art baseline methods

Clinical implications of four different nutritional indexes in patients with IgA nephropathy

    Get PDF
BackgroundImmunoglobulin A nephropathy (IgAN) is the most prevalent form of chronic kidney disease (CKD), marked by diverse pathological patterns and variable prognostic outcomes. Nutritional indexes are crucial for disease assessment and prognosis prediction. This study investigates associations between nutritional indexes and renal function in patients with IgAN.MethodsA cohort of 736 adults diagnosed with IgAN, who underwent renal biopsy at the First Hospital of Jilin University between January 2010 and October 2022, was examined. Clinical and laboratory data were reviewed, and four nutritional indexes were calculated: controlling nutritional status (CONUT) score, geriatric nutritional risk index (GNRI), body mass index (BMI), and prognostic nutritional index (PNI). Cox-proportional hazard analysis evaluated factors associated with end-stage renal disease (ESRD).ResultsPatients with ESRD showed significantly lower GNRI (91.84 vs. 98.94, p < 0.001) and median PNI (41.90 vs. 46.30, p < 0.001), with higher median CONUT score (2.00 vs. 1.00, p = 0.001) compared to those without ESRD. PNI, GNRI, and CONUT scores correlated significantly with C2 in MEST-C classification. Kaplan–Meier analysis indicated increased ESRD probability in individuals with specific thresholds of PNI, GNRI, or CONUT scores. Additionally, GNRI emerged as an independent predictor of ESRD (hazard ratio: 0.963, 95% CI: 0.940–0.979, p < 0.001), along with platelet count, serum creatinine, eGFR (CKD-EPI), and triglyceride levels.ConclusionGNRI, PNI, and CONUT scores hold potential in reflecting IgAN severity and predicting ESRD risk. GNRI especially may serve as a valuable tool for identifying high-risk individuals for ESRD in IgAN

Oxidative stress: The nexus of obesity and cognitive dysfunction in diabetes

    Get PDF
Obesity has been associated with oxidative stress. Obese patients are at increased risk for diabetic cognitive dysfunction, indicating a pathological link between obesity, oxidative stress, and diabetic cognitive dysfunction. Obesity can induce the biological process of oxidative stress by disrupting the adipose microenvironment (adipocytes, macrophages), mediating low-grade chronic inflammation, and mitochondrial dysfunction (mitochondrial division, fusion). Furthermore, oxidative stress can be implicated in insulin resistance, inflammation in neural tissues, and lipid metabolism disorders, affecting cognitive dysfunction in diabetics

Enzymatic Degradation of Alginate and in Vitro Immunological Activity of Its Degraded Products

In this study, changes in the molecular mass of alginate were investigated during its enzymatic degradation and the processing parameters for the enzymatic preparation of alginate oligosaccharides were explored. Furthermore, the in vitro immunological activity of degraded products with different molecular mass was evaluated. The results showed that the molecular mass of alginate decreased significantly after degradation by alginate lyase, and three degradation products with different molecular mass were obtained through gradient ethanol fractionation. Their weight-average molecular masses were 13.4, 5.73 and 3.85 kDa, respectively. Using single factor experiments, the optimal processing parameters were determined as pH 7.0, alginate lyase dosage 15 U/g substrate, and hydrolysis time 24 h, giving a yield of 28.05%. All alginate and its degraded products had immunoenhancing activity in mouse macrophages, and among them, the effect of the product with a weight-average molecular mass of 5.73 kDa was most pronounced and more pronounced than that of alginate oligosaccharides. By adding TAK-242, a blocker of macrophage Toll-like receptor 4 (TLR4), it was verified that the degraded products of alginate regulated macrophage immune activity by inducing the secretion of TLR4 and consequently causing cascade reactions to increase the secretion of NO, TNF-α and IL-6. These results can provide a theoretical basis for the high-value utilization of alginate

High Prevalence of blaNDM Variants Among Carbapenem-Resistant Escherichia coli in Northern Jiangsu Province, China

The continuous emergence of carbapenem-resistant Escherichia coli (CRECO) presents a great challenge to public health. New Delhi metallo-lactamase (NDM) variants are widely disseminated in China, so the research on the prevalence and transmission of diverse blaNDM variants is urgently needed. In the present study, 54 CRECO isolates were collected from 1,185 Escherichia coli isolates in five hospitals in Northern Jiangsu Province, China from September 2015 to August 2016. Antimicrobial susceptibility tests, PCR detection of resistance determinants, multi-locus sequence typing (MLST) and pulsed-field gel electrophoresis (PFGE) were performed to characterize these strains. Plasmid conjugation experiments were carried out to determine the transferability of resistant genes from selected isolates. PCR-based replicon typing (PBRT), S1 nuclease-PFGE, and Southern blotting were conducted for plasmid profiling. Carbapenemase genes were detectable in all CRECO isolates, among which thirty-one CRECO isolates were found to carry blaNDM−5 (54.7%), while, blaNDM−1, blaNDM−7, blaNDM−4, blaNDM−9, and blaKPC−2 were identified in 14, five, two, one, and one isolates, respectively. MLST results revealed 15 different STs and four new STs were first reported to be linked with NDM-producing isolates. PFGE typing showed that no more than two isolates with the same ST appeared to the same band pattern except three ST410 isolates. Twenty-six selected NDM-producing isolates were successfully transferred to E. coli J53 by conjugation experiments. Notably, 50.0% (13/26) of blaNDM variants were found to be carried by ~55 kb IncX3 plasmid. Our study reported a high prevalence of blaNDM variants, especially blaNDM−5, in Northern Jiangsu province, China. Diverse blaNDM variants were mainly carried by ~55 kb IncX3 plasmids, suggesting that the fast evolution and high transferability of this kind of plasmid promote the high prevalence of blaNDM variants. Therefore, large-scale surveillance and effective infection control measures are also urgently needed to prevent diverse blaNDM variants from becoming epidemic in the future
