    The effects of passing speed distribution on rail corrugation growth rate

    The transportation phenomenon known as wear-type rail corrugation is a significant problem in railway engineering, which manifests as a periodic wear pattern developing on the surface of the wheel and rail with use. Some field studies and recent theoretical results by the current authors have suggested that uniformity in pass speed causes an increase in corrugation growth rate. This paper presents the predicted change in corrugation growth rate and dominant wavelengths with change in passing speed distribution, based on state of the art cornering growth modelling techniques

    Future asymptotic expansions of Bianchi VIII vacuum metrics

    Bianchi VIII vacuum solutions to Einstein's equations are causally geodesically complete to the future, given an appropriate time orientation, and the objective of this article is to analyze the asymptotic behaviour of solutions in this time direction. For the Bianchi class A spacetimes, there is a formulation of the field equations that was presented in an article by Wainwright and Hsu, and in a previous article we analyzed the asymptotic behaviour of solutions in these variables. One objective of this paper is to give an asymptotic expansion for the metric. Furthermore, we relate this expansion to the topology of the compactified spatial hypersurfaces of homogeneity. The compactified spatial hypersurfaces have the topology of Seifert fibred spaces and we prove that in the case of NUT Bianchi VIII spacetimes, the length of a circle fibre converges to a positive constant but that in the case of general Bianchi VIII solutions, the length tends to infinity at a rate we determine.Comment: 50 pages, no figures.     Enduring mental health: Prevalence and prediction.

    We review epidemiological evidence indicating that most people will develop a diagnosable mental disorder, suggesting that only a minority experience enduring mental health. This minority has received little empirical study, leaving the prevalence and predictors of enduring mental health unknown. We turn to the population-representative Dunedin cohort, followed from birth to midlife, to compare people never-diagnosed with mental disorder (N = 171; 17% prevalence) to those diagnosed at 1–2 study waves, the cohort mode (N = 409). Surprisingly, compared to this modal group, never-diagnosed Study members were not born into unusually well-to-do families, nor did their enduring mental health follow markedly sound physical health, or unusually high intelligence. Instead, they tended to have an advantageous temperament/personality style, and negligible family history of mental disorder. As adults, they report superior educational and occupational attainment, greater life satisfaction, and higher-quality relationships. Our findings draw attention to “enduring mental health” as a revealing psychological phenotype and suggest it deserves further study

    We use the dynamical systems approach to investigate the Bianchi type VIII models with a tilted Îł\gamma-law perfect fluid. We introduce expansion-normalised variables and investigate the late-time asymptotic behaviour of the models and determine the late-time asymptotic states. For the Bianchi type VIII models the state space is unbounded and consequently, for all non-inflationary perfect fluids, one of the curvature variables grows without bound. Moreover, we show that for fluids stiffer than dust (1<Îł<21<\gamma<2), the fluid will in general tend towards a state of extreme tilt. For dust (Îł=1\gamma=1), or for fluids less stiff than dust (0<Îł<10<\gamma< 1), we show that the fluid will in the future be asymptotically non-tilted.     Effect of local anaesthetic infiltration on chronic postsurgical pain after total hip and knee replacement:The APEX randomised controlled trials

    Total hip replacement (THR) and total knee replacement (TKR) are usually effective at relieving pain; however, 7% to 23% of patients experience chronic postsurgical pain. These trials aimed to investigate the effect of local anaesthetic wound infiltration on pain severity at 12 months after primary THR or TKR for osteoarthritis. Between November 2009 and February 2012, 322 patients listed for THR and 316 listed for TKR were recruited into a single-centre double-blind randomised controlled trial. Participants were randomly assigned (1:1) to receive local anaesthetic infiltration and standard care or standard care alone. Participants and outcomes assessors were masked to group allocation. The primary outcome was pain severity on the WOMAC Pain Scale at 12 months after surgery. Analyses were conducted using intention-to-treat and per-protocol approaches. In the hip trial, patients in the intervention group had significantly less pain at 12 months postoperative than patients in the standard care group (differences in means: 4.74; 95% confidence interval [CI]: 0.95-8.54; P = 0.015), although the difference was not clinically significant. Post hoc analysis found that patients in the intervention group were more likely to have none to moderate pain than severe pain at 12 months than those in the standard care group (odds ratio: 10.19; 95% CI: 2.10-49.55; P = 0.004). In the knee trial, there was no strong evidence that the intervention influenced pain severity at 12 months postoperative (difference in means: 3.83; 95% CI: −0.83 to 8.49; P = 0.107). In conclusion, routine use of infiltration could be beneficial in improving long-term pain relief for some patients after THR

    Correlates of Complete Childhood Vaccination in East African Countries.

    Despite the benefits of childhood vaccinations, vaccination rates in low-income countries (LICs) vary widely. Increasing coverage of vaccines to 90% in the poorest countries over the next 10 years has been estimated to prevent 426 million cases of illness and avert nearly 6.4 million childhood deaths worldwide. Consequently, we sought to provide a comprehensive examination of contemporary vaccination patterns in East Africa and to identify common and country-specific barriers to complete childhood vaccination. Using data from the Demographic and Health Surveys (DHS) for Burundi, Ethiopia, Kenya, Rwanda, Tanzania, and Uganda, we looked at the prevalence of complete vaccination for polio, measles, Bacillus Calmette-Guérin (BCG) and DTwPHibHep (DTP) as recommended by the WHO among children ages 12 to 23 months. We conducted multivariable logistic regression within each country to estimate associations between complete vaccination status and health care access and sociodemographic variables using backwards stepwise regression. Vaccination varied significantly by country. In all countries, the majority of children received at least one dose of a WHO recommended vaccine; however, in Ethiopia, Tanzania, and Uganda less than 50% of children received a complete schedule of recommended vaccines. Being delivered in a public or private institution compared with being delivered at home was associated with increased odds of complete vaccination status. Sociodemographic covariates were not consistently associated with complete vaccination status across countries. Although no consistent set of predictors accounted for complete vaccination status, we observed differences based on region and the location of delivery. These differences point to the need to examine the historical, political, and economic context of each country in order to maximize vaccination coverage. Vaccination against these childhood diseases is a critical step towards reaching the Millennium Development Goal of reducing under-five mortality by two-thirds by 2015 and thus should be a global priority

    Detection of enteric viral and bacterial pathogens associated with paediatric diarrhoea in Goroka, Papua New Guinea

    Objectives: The aim of this study was to investigate the viral and bacterial causes of acute watery diarrhoea in hospitalized children in Papua New Guinea. Methods: A retrospective analysis was conducted on stool samples collected from 199 children (age < 5 years) admitted to the paediatric ward of Goroka General Hospital from August 2009 through November 2010. A large range of viral and bacterial enteric pathogens were targeted using real-time PCR/RT-PCR assays. Results: Young children were much more likely to be admitted with acute gastroenteritis, with 62.8% of patients aged <1 year and 88.4% aged <2 years. An enteric pathogen was detected in 69.8% (n = 138) of patients. The most commonly detected pathogens were Shigella spp (26.6%), rotavirus (25.6%), adenovirus types 40/41 (11.6%), enterotoxigenic Escherichia coli (11.1%), enteropathogenic E. coli (8.5%), norovirus G2 (6.0%), and Campylobacter spp (4.0%). Norovirus G1, sapovirus, and Salmonella spp were also detected, but below our statistical limit of detection. Vibrio cholerae and astrovirus were not detected in any patients. Mixed infections were detected in 22.1% of patients, with Shigella and rotavirus most commonly detected in co-infections with other pathogens. Conclusions: This study demonstrates that Shigella and rotavirus are the major pathogens associated with acute paediatric gastroenteritis in this setting

    Self-similar Bianchi models: I. Class A models

    We present a study of Bianchi class A tilted cosmological models admitting a proper homothetic vector field together with the restrictions, both at the geometrical and dynamical level, imposed by the existence of the simply transitive similarity group. The general solution of the symmetry equations and the form of the homothetic vector field are given in terms of a set of arbitrary integration constants. We apply the geometrical results for tilted perfect fluids sources and give the general Bianchi II self-similar solution and the form of the similarity vector field. In addition we show that self-similar perfect fluid Bianchi VII0_0 models and irrotational Bianchi VI0_0 models do not exist.Comment: 14 pages, Latex; to appear in Classical and Quantum Gravit
