    Working at Home and Being at Home: The Interaction of Microcomputers and the Social Life of Households

    Not until the recent upsurge of interest in microcomputers and home work has attention been devoted to the household as a setting for technical learning and invention, or organizational and independent work. Drawing upon theoretical implications of research on industrial technology and the household, this study contributes to the development of an empirical basis for understanding the first ten years of microcomputer use at home. The environmental approach to this psychological study includes two stages. In the first, a survey and content analysis of over 400 articles in mainstream periodicals and national newspapers revealed that since 1976 the representation of computer home-use shifted toward work-related purposes. In the second stage, six case studies of urban families with school-age children were conducted in order to understand the evolution that had occurred since they purchased microcomputers and attempted to use them for self-employment. Interviews revealed that while adults and children use microcomputers to learn independently of their schools, and to work independently of their jobs, individual uses of microcomputers are not an entirely individual matter. A second income was needed to support the first years of self-employment, and gender influenced whose computing was encouraged, and whose work was interrupted. In every household, there was an increase in time spent working, though not necessarily, time spent earning. Media illustrations included

    Addressing the environmental, community and health impacts of resource development: Challenges across scales, sectors and sites

    Work that addresses the cumulative impacts of resource extraction on environment, community, and health is necessarily large in scope. This paper presents experiences from initiating research at this intersection and explores implications for the ambitious, integrative agenda of planetary health. The purpose is to outline origins, design features, and preliminary insights from our intersectoral and international project, based in Canada and titled the “Environment, Community, Health Observatory” (ECHO) Network. With a clear emphasis on rural, remote, and Indigenous communities, environments, and health, the ECHO Network is designed to answer the question: How can an Environment, Community, Health Observatory Network support the integrative tools and processes required to improve understanding and response to the cumulative health impacts of resource development? The Network is informed by four regional cases across Canada where we employ a framework and an approach grounded in observation, “taking notice for action”, and collective learning. Sharing insights from the foundational phase of this five-year project, we reflect on the hidden and obvious challenges of working across scales, sectors, and sites, and the overlap of generative and uncomfortable entanglements associated with health and resource development. Yet, although intersectoral work addressing the cumulative impacts of resource extraction presents uncertainty and unresolved tensions, ultimately we argue that it is worth staying with the trouble

    Effectiveness of nail bed repair in children with or without replacing the fingernail : NINJA multicentre randomized clinical trial

    Background Surgery for nail bed injuries in children is common. One of the key surgical decisions is whether to replace the nail plate following nail bed repair. The aim of this RCT was to assess the clinical effectiveness and cost-effectiveness of nail bed repair with fingernail replacement/substitution compared with repair without fingernail replacement. Methods A two-arm 1 : 1 parallel-group open multicentre superiority RCT was performed across 20 secondary-care hospitals in the UK. The co-primary outcomes were surgical-site infection at around 7 days after surgery and cosmetic appearance summary score at a minimum of 4 months. Results Some 451 children presenting with a suspected nail bed injury were recruited between July 2018 and July 2019; 224 were allocated to the nail-discarded arm, and 227 to the nail-replaced arm. There was no difference in the number of surgical-site infections at around 7 days between the two interventions or in cosmetic appearance. The mean total healthcare cost over the 4 months after surgery was €84 (95 per cent c.i. 34 to 140) lower for the nail-discarded arm than the nail-replaced arm (P < 0.001). Conclusion After nail bed repair, discarding the fingernail was associated with similar rates of infection and cosmesis ratings as replacement of the finger nail, but was cost saving
