Why Does the SEC Hate Lawyers and Will the Bitterness Ever Go Away: A Review of the Reasons for the Current State of this Relationship and a Proposed Path Forward
The United States Securities and Exchange Commission (“SEC” or “Commission”) and its staff (“Staff”) have brought numerous actions against lawyers in a variety of contexts over the last several years. These enforcement actions have arguably prevented zealous advocacy as well as potentially leaving lawyers reluctant to make certain arguments on behalf of their clients so as to avoid potential disciplinary actions against them. While it is important for the Commission and its Staff to ensure that lawyers do not engage in violative conduct, this Article notes that the SEC and its Staff’s actions should be limited to only those occasions where the conduct is notorious and obvious. To avoid unwarranted interference in the right to counsel, this Article argues, at the very least, that the SEC should instead of bringing circumspect actions: (1) clarify the SEC’s approach to its use of Rule 102(e); (2) make mandatory referrals to state bar associations; and (3) create an independent board to review potential SEC actions against lawyers to ensure the good faith nature of the proposed action
Epidural Analgesia Decreases Narcotic Requirements in Low Level Spina Bifida Patients Undergoing Urologic Laparotomy for Neurogenic Bladder and Bowel
Purpose
Concern of anatomical anomalies and worsening neurologic symptoms has prevented widespread use of epidural catheters in patients with low level spina bifida (LLSB). We hypothesize that thoracic epidural placement in the T9-T10 interspace is safe and decreases narcotic requirements in LLSB patients following major open lower urinary tract reconstruction (LUTR).
Materials and Methods
We reviewed consecutive LLSB patients who had LUTR and epidurals for post-operative pain control. Controls were LLSB patients who received single shot transversus abdominis plane (TAP) blocks with similar procedures. Complications from epidural placement, including changes in motor and sensory status were recorded. Opioid consumption was calculated utilizing equivalent IV morphine doses. Mean and maximum pain scores on post-operative day (POD) 0-3 were calculated.
Results
10 LLSB patients who had lower urinary tract reconstruction and epidurals were matched to 10 LLSB patients who had lower urinary tract reconstruction and transverse abdominis plane blocks. Groups were demographically similar. All had full abdominal sensation and functional levels at or below L3. No epidural complications or changes in neurological status were noted. The epidural group had decreased opioid consumption on POD 0-3 (0.75 mg/kg vs. 1.29 mg/kg, p=0.04). Pain scores were similar or improved in the epidural group.
Conclusions
Thoracic epidural analgesia appears to be a safe and effective opioid sparing option to assist with post-operative pain management following lower urinary tract reconstruction in LLSB patients

Fast non-negative deconvolution for spike train inference from population calcium imaging
Calcium imaging for observing spiking activity from large populations of
neurons are quickly gaining popularity. While the raw data are fluorescence
movies, the underlying spike trains are of interest. This work presents a fast
non-negative deconvolution filter to infer the approximately most likely spike
train for each neuron, given the fluorescence observations. This algorithm
outperforms optimal linear deconvolution (Wiener filtering) on both simulated
and biological data. The performance gains come from restricting the inferred
spike trains to be positive (using an interior-point method), unlike the Wiener
filter. The algorithm is fast enough that even when imaging over 100 neurons,
inference can be performed on the set of all observed traces faster than
real-time. Performing optimal spatial filtering on the images further refines
the estimates. Importantly, all the parameters required to perform the
inference can be estimated using only the fluorescence data, obviating the need
to perform joint electrophysiological and imaging calibration experiments.Comment: 22 pages, 10 figure
Prehospital amputation : an experimental comparison of techniques
"Prehospital limb amputation is a rare, but potentially lifesaving intervention. When adequate resuscitation is not possible due to difficult patient access, hemodynamically unstable patients may benefit from an emergent prehospital amputation. There have been a limited number of case reports detailing prehospital amputation. Furthermore, there has only been one experimental trial. Leech et al. explored prehospital-friendly methods of amputation on human cadavers; however, due to a small sample size of four trials, the data has limited reliability."--Introduction
