    Mitochondrial Dysregulation in the Pathogenesis of Diabetes: Potential for Mitochondrial Biogenesis-Mediated Interventions

    Muscle mitochondrial metabolism is a tightly controlled process that involves the coordination of signaling pathways and factors from both the nuclear and mitochondrial genomes. Perhaps the most important pathway regulating metabolism in muscle is mitochondrial biogenesis. In response to physiological stimuli such as exercise, retrograde signaling pathways are activated that allow crosstalk between the nucleus and mitochondria, upregulating hundreds of genes and leading to higher mitochondrial content and increased oxidation of substrates. With type 2 diabetes, these processes can become dysregulated and the ability of the cell to respond to nutrient and energy fluctuations is diminished. This, coupled with reduced mitochondrial content and altered mitochondrial morphology, has been directly linked to the pathogenesis of this disease. In this paper, we will discuss our current understanding of mitochondrial dysregulation in skeletal muscle as it relates to type 2 diabetes, placing particular emphasis on the pathways of mitochondrial biogenesis and mitochondrial dynamics, and the therapeutic value of exercise and other interventions

    Sox21b underlies the rapid diversification of a novel male genital structure between Drosophila species

    The emergence and diversification of morphological novelties is a major feature of animal evolution. However, relatively little is known about the genetic basis of the evolution of novel structures and the mechanisms underlying their diversification. The epandrial posterior lobes of male genitalia are a novelty of particular Drosophila species. The lobes grasp the female ovipositor and insert between her abdominal tergitesand, therefore, are important for copulation and species recognition. The posterior lobes likely evolved from co-option of a Hox-regulated gene network from the posterior spiracles and have sincediversified in morphology in the D.simulans clade, in particular, over the last 240,000 years, drivenby sexual selection. The genetic basis of this diversification is polygenic but, to the best ofour knowledge, none of the causative genes have been identified. Identifying the genes underlyingthe diversification of these secondary sexual structures is essential to understanding theevolutionary impact on copulation and species recognition. Here, we show that Sox21b negatively regulates posterior lobe size. This is consistent with expanded Sox21b expression in D.mauritiana, which develops smaller posterior lobes than D.simulans. We tested this by generating reciprocal hemizygotes and confirmed that changes in Sox21b underlie posterior lobe evolution between these species. Furthermore, we found that posterior lobe size differences caused by the species-specific allele of Sox21b significantly affect copulation duration. Taken together, our study reveals the genetic basis for the sexual-selection-driven diversification of a novel morphological structure and its functional impact on copulatory behavior.     The dog meat trade (DMT) in Bali is a black-market trade that threatens international tourism on both animal welfare and human health grounds. Outlets selling dog meat often advertise this by displaying ‘RW’ signs as this abbreviation stands for ‘rintek wuuk’ (soft fur) in Manadonese. The DMT is notoriously cruel and uses inhumane methods to catch, transport, and slaughter dogs, many of which are believed to be stolen.Dog meat is not a food under Indonesian law and as such is not subject to food safety or zoonotic disease regulation. In particular, the DMT is believed to contribute to the transmission of rabies, which is endemic in Bali.Despite this, there was little factual information about the DMT in Bali, and this was hindering Government efforts to close the trade. We believe our study is the first to characterize the DMT in Bali and provides insights as to how and why the trade exists, and what actions are needed to end it

    A study of practitioner–service user relationships in social work

    This paper reports on findings from a qualitative study of practitioner–service user relationships in social work. The research aimed to identify social workers’ personal constructs of their relationships with service users and explore how these constructs differed across roles and settings. A qualitative methodology employing a variation on role repertory grid techniques was used to carry out semi-structured interviews with social workers. Twenty-five social workers from seven different practice settings were interviewed and altogether identified over 200 personal constructs. The research team undertook a thematic analysis of these constructs along with their explanation and discussion in interview transcripts. The results identified twenty-five superordinate constructs within ten core themes, which reflected practitioners’ experience of relationships, their systemic context, along with dynamics of power and collaboration. The constructs were often found to contrast a positive or preferred attribute of relationships with a more negative or challenging attribute, although the reality of relationships was often found to be complex and ambiguous. Some implications are explored for current theories of relationship-based practice in social work

    A classification model of homelessness using integrated administrative data: Implications for targeting interventions to improve the housing status, health and well-being of a highly vulnerable population

    Homelessness is poorly captured in most administrative data sets making it difficult to understand how, when, and where this population can be better served. This study sought to develop and validate a classification model of homelessness. Our sample included 5,050,639 individuals aged 11 years and older who were included in a linked dataset of administrative records from multiple state-maintained databases in Massachusetts for the period from 2011-2015. We used logistic regression to develop a classification model with 94 predictors and subsequently tested its performance. The model had high specificity (95.4%), moderate sensitivity (77.8%) for predicting known cases of homelessness, and excellent classification properties (area under the receiver operating curve 0.94; balanced accuracy 86.4%). To demonstrate the potential opportunity that exists for using such a modeling approach to target interventions to mitigate the risk of an adverse health outcome, we also estimated the association between model predicted homeless status and fatal opioid overdoses, finding that model predicted homeless status was associated with a nearly 23-fold increase in the risk of fatal opioid overdose. This study provides a novel approach for identifying homelessness using integrated administrative data. The strong performance of our model underscores the potential value of linking data from multiple service systems to improve the identification of housing instability and to assist government in developing programs that seek to improve health and other outcomes for homeless individuals

    Closed-Loop Insulin Delivery for Glycemic Control in Noncritical Care.

    BACKGROUND: In patients with diabetes, hospitalization can complicate the achievement of recommended glycemic targets. There is increasing evidence that a closed-loop delivery system (artificial pancreas) can improve glucose control in patients with type 1 diabetes. We wanted to investigate whether a closed-loop system could also improve glycemic control in patients with type 2 diabetes who were receiving noncritical care. METHODS: In this randomized, open-label trial conducted on general wards in two tertiary hospitals located in the United Kingdom and Switzerland, we assigned 136 adults with type 2 diabetes who required subcutaneous insulin therapy to receive either closed-loop insulin delivery (70 patients) or conventional subcutaneous insulin therapy, according to local clinical practice (66 patients). The primary end point was the percentage of time that the sensor glucose measurement was within the target range of 100 to 180 mg per deciliter (5.6 to 10.0 mmol per liter) for up to 15 days or until hospital discharge. RESULTS: The mean (±SD) percentage of time that the sensor glucose measurement was in the target range was 65.8±16.8% in the closed-loop group and 41.5±16.9% in the control group, a difference of 24.3±2.9 percentage points (95% confidence interval [CI], 18.6 to 30.0; P<0.001); values above the target range were found in 23.6±16.6% and 49.5±22.8% of the patients, respectively, a difference of 25.9±3.4 percentage points (95% CI, 19.2 to 32.7; P<0.001). The mean glucose level was 154 mg per deciliter (8.5 mmol per liter) in the closed-loop group and 188 mg per deciliter (10.4 mmol per liter) in the control group (P<0.001). There was no significant between-group difference in the duration of hypoglycemia (as defined by a sensor glucose measurement of <54 mg per deciliter; P=0.80) or in the amount of insulin that was delivered (median dose, 44.4 U and 40.2 U, respectively; P=0.50). No episode of severe hypoglycemia or clinically significant hyperglycemia with ketonemia occurred in either trial group. CONCLUSIONS: Among inpatients with type 2 diabetes receiving noncritical care, the use of an automated, closed-loop insulin-delivery system resulted in significantly better glycemic control than conventional subcutaneous insulin therapy, without a higher risk of hypoglycemia.
