    Anhand eines Interviewbeispiels wird in diesem Artikel die subjektive Bedeutungszuschreibung an jugendkulturelle Orte (Techno-Szene) und den sich darüber eröffnenden Seins- und Handlungsformen (Partydrogen-Konsum, modische Selbstdarstellungen) nachgezeichnet. Diese unmittelbare Sichtweise der Interviewpartnerin auf ihre Alltags- und Freizeitwelt erfährt jedoch erst im Kontext gesellschaftstheoretischer Phänomene wie dem Geschlechterverhältnis ihre Verstehbarkeit - wodurch letztendlich ein komplexes und differenziertes Bild von weiblicher Adoleszenz und jugendtypischen Seins- und Handlungsweisen entsteht. Der Artikel basiert dabei auf Ergebnissen der qualitativen Untersuchung zum illegalen Drogenkonsum von jungen Frauen

    Listeriose-Ausbrüche in Deutschland – Hinweise auf geräucherte oder gebeizte Lachsprodukte als Ursache von Infektionen, Fortschreibung des Berichts aus 2021

    Listeriose, verursacht durch das Bakterium Listeria (L.) monocytogenes, tritt in verschiedenen Formen auf und wird vor allem durch Konsum kontaminierter Lebensmittel übertragen. Die Krankheit ist mit teilweise schweren Verläufen und einer hohen Sterblichkeit assoziiert. Im Epidemiologischen Bulletin 3/2021 wurde ein Artikel zu 22 Listeriose-Ausbrüchen in Deutschland mit Hinweisen auf geräucherte oder gebeizte Lachsprodukte als Ursache von Infektionen publiziert. Im vorliegenden Bericht wird nun ein Update zur aktuellen Situation gegeben: Es konnten insgesamt 24 bundeslandübergreifende Listeriose-Ausbrüche mit Hinweisen auf geräucherte oder gebeizte Lachsprodukte als Ursache seit 2010 identifiziert werden. Dies sind zwei Ausbrüche mehr als im vorherigen Bericht. In den Jahren 2021 und 2022 gab es 66 Listeriose-Fälle in 15 Ausbrüchen. Die Daten bestätigen, dass geräucherte oder gebeizte Lachsprodukte in Deutschland weiterhin ein relevantes Risiko für Listeriose darstellen. Zudem ist insgesamt bei allen Listeriose-Ausbrüchen mit einer starken Untererfassung zu rechnen. Es ist deshalb davon auszugehen, dass die Erkrankungszahlen in der Bevölkerung in Deutschland deutlich höher sind.Peer Reviewe

    Impact of newborn screening for SCID on the management of congenital athymia

    BACKGROUND: Newborn screening (NBS) programmes for severe combined immunodeficiency (SCID) facilitate early SCID diagnosis and promote early treatment with haematopoietic stem cell transplantation, resulting in improved clinical outcomes. Infants with congenital athymia are also identified through NBS due to severe T-cell lymphopaenia. With the expanding introduction of NBS programmes, referrals of athymic patients for treatment with thymus transplantation have recently increased at Great Ormond Street Hospital (GOSH), London, United Kingdom. OBJECTIVE: We studied the impact of NBS on timely diagnosis and treatment of athymic infants with thymus transplantation at GOSH. METHODS: We compared the age at referral and complications between athymic infants diagnosed after clinical presentation (N=25) and patients identified through NBS (N=19), referred for thymus transplantation at GOSH between 10/2019 and 02/2023. We assessed whether age at time of treatment influences thymic output at 6 and 12 months after transplantation. RESULTS: Infants referred after NBS identification were significantly younger and had less complications, in particular less infections. All deaths occurred in the non-NBS group, including six patients before and two after thymus transplantation because of pre-existing infections. In the absence of significant co-morbidities or diagnostic uncertainties, timely treatment was more frequently achieved after NBS. Treatment at <4 months of age was associated with higher thymic output at 6- and 12-months post-transplantation. CONCLUSION: NBS contributes to earlier recognition of congenital athymia, promoting referral of athymic patients for thymus transplantation prior to acquiring infections or other complications, and facilitating treatment at younger age, thus playing an important role in improving their outcomes

    A ventricular fibrillation cardiac arrest model with extracorporeal cardiopulmonary resuscitation in rats: 8 minutes arrest time leads to increased myocardial damage but does not increase neuronal damage compared to 6 minutes

    IntroductionExtracorporeal cardiopulmonary resuscitation (ECPR) is an emerging strategy in highly selected patients with refractory cardiac arrest (CA). Animal models can help to identify new therapeutic strategies to improve neurological outcome and cardiac function after global ischemia in CA. Aim of the study was to establish a reproducible ECPR rat model of ventricular fibrillation CA (VFCA) that leads to consistent neuronal damage with acceptable long-term survival rates, which can be used for future research.Materials and methodsMale Sprague Dawley rats were resuscitated with ECPR from 6 min (n = 15) and 8 min (n = 16) VFCA. Animals surviving for 14 days after return of spontaneous resuscitation (ROSC) were compared with sham operated animals (n = 10); neurological outcome was assessed daily until day 14. In the hippocampal cornu ammonis 1 region viable neurons were counted. Microglia and astrocyte reaction was assessed by Iba1 and GFAP immunohistochemistry, and collagen fibers in the myocardium were detected in Azan staining. QuPath was applied for quantification.ResultsOf the 15 rats included in the 6 min CA group, all achieved ROSC (100%) and 10 (67%) survived to 14 days; in the 8 min CA group, 15 (94%) achieved ROSC and 5 (31%) reached the endpoint. All sham animals (n = 10) survived 2 weeks. The quantity of viable neurons was significantly decreased, while the area displaying Iba1 and GFAP positive pixels was significantly increased in the hippocampus across both groups that experienced CA. Interestingly, there was no difference between the two CA groups regarding these changes. The myocardium in the 8 min CA group exhibited significantly more collagen fibers compared to the sham animals, without differences between 6- and 8-min CA groups. However, this significant increase was not observed in the 6 min CA group.ConclusionOur findings indicate a uniform occurrence of neuronal damage in the hippocampus across both CA groups. However, there was a decrease in survival following an 8-min CA. Consequently, a 6-min duration of CA resulted in predictable neurological damage without significant cardiac damage and ensured adequate survival rates up to 14 days. This appears to offer a reliable model for investigating neuroprotective therapies

    The German National Registry of Primary Immunodeficiencies (2012-2017)

    Introduction: The German PID-NET registry was founded in 2009, serving as the first national registry of patients with primary immunodeficiencies (PID) in Germany. It is part of the European Society for Immunodeficiencies (ESID) registry. The primary purpose of the registry is to gather data on the epidemiology, diagnostic delay, diagnosis, and treatment of PIDs. Methods: Clinical and laboratory data was collected from 2,453 patients from 36 German PID centres in an online registry. Data was analysed with the software Stata® and Excel. Results: The minimum prevalence of PID in Germany is 2.72 per 100,000 inhabitants. Among patients aged 1–25, there was a clear predominance of males. The median age of living patients ranged between 7 and 40 years, depending on the respective PID. Predominantly antibody disorders were the most prevalent group with 57% of all 2,453 PID patients (including 728 CVID patients). A gene defect was identified in 36% of patients. Familial cases were observed in 21% of patients. The age of onset for presenting symptoms ranged from birth to late adulthood (range 0–88 years). Presenting symptoms comprised infections (74%) and immune dysregulation (22%). Ninety-three patients were diagnosed without prior clinical symptoms. Regarding the general and clinical diagnostic delay, no PID had undergone a slight decrease within the last decade. However, both, SCID and hyper IgE- syndrome showed a substantial improvement in shortening the time between onset of symptoms and genetic diagnosis. Regarding treatment, 49% of all patients received immunoglobulin G (IgG) substitution (70%—subcutaneous; 29%—intravenous; 1%—unknown). Three-hundred patients underwent at least one hematopoietic stem cell transplantation (HSCT). Five patients had gene therapy. Conclusion: The German PID-NET registry is a precious tool for physicians, researchers, the pharmaceutical industry, politicians, and ultimately the patients, for whom the outcomes will eventually lead to a more timely diagnosis and better treatment

    Within the e-mobility sector, which represents a major driver of the development of the overall lithium-ion battery market, batteries with nickel-manganese-cobalt (NMC) cathode chemistries are currently gaining ground. This work is specifically dedicated to this NMC battery type and investigates achievable recovery rates of the valuable materials contained when applying an unconventional, pyrometallurgical reactor concept. For this purpose, the currently most prevalent NMC modifications (5-3-2, 6-2-2, and 8-1-1) with carbon addition were analyzed using thermogravimetric analysis and differential scanning calorimetry, and treated in a lab-scale application of the mentioned reactor principle. It was shown that the reactor concept achieves high recovery rates for nickel, cobalt, and manganese of well above 80%. For lithium, which is usually oxidized and slagged, the transfer coefficient into the slag phase was less than 10% in every experimental trial. Instead, it was possible to remove the vast amount of it via a gas phase, which could potentially open up new paths regarding metal recovery from spent lithium-ion batteries
