    Which medicine? Whose standard? Critical reflections on medical integration in China

    There is a prevailing conviction that if traditional medicine (TRM) or complementary and alternative medicine (CAM) are integrated into healthcare systems, modern scientific medicine (MSM) should retain its principal status. This paper contends that this position is misguided in medical contexts where TRM is established and remains vibrant. By reflecting on the Chinese policy on three entrenched forms of TRM (Tibetan, Mongolian and Uighur medicines) in western regions of China, the paper challenges the ideology of science that lies behind the demand that all traditional forms of medicine be evaluated and reformed according to MSM standards. Tibetan medicine is used as a case study to indicate the falsity of a major premise of the scientific ideology. The conclusion is that the proper integrative system for TRM and MSM is a dual standard based system in which both TRM and MSM are free to operate according to their own medical standards.published_or_final_versio

    Analysis and design of a modular multilevel converter with trapezoidal modulation for medium and high voltage DC-DC transformers

    Conventional dual active bridge topologies provide galvanic isolation and soft-switching over a reasonable operating range without dedicated resonant circuits. However, scaling the two-level dual active bridge to higher dc voltage levels is impeded by several challenges among which the high dv/dt stress on the coupling transformer insulation. Gating and thermal characteristics of series switch arrays add to the limitations. To avoid the use of standard bulky modular multilevel bridges, this paper analyzes an alternative modulation technique where staircase approximated trapezoidal voltage waveforms are produced; thus alleviating developed dv/dt stresses. Modular design is realized by the utilization of half-bridge chopper cells. Therefore, the analyzed converter is a modular multi-level converter operated in a new mode with no common-mode dc arm currents as well as reduced capacitor size, hence reduced cell footprint. Suitable switching patterns are developed and various design and operation aspects are studied. Soft switching characteristics will be shown to be comparable to those of the two-level dual active bridge. Experimental results from a scaled test rig validate the presented concept

    Earthquake occurrence in nature is thought to result from correlated elastic stresses, leading to clustering in space and time. We show that occurrence of major earthquakes in California correlates with time intervals when fluctuations in small earthquakes are suppressed relative to the long term average.     Characterising professional drivers’ exposure to traffic-related air pollution: Evidence for reduction strategies from in-vehicle personal exposure monitoring

    Professional drivers working in congested urban areas are required to work near harmful traffic related pollutants for extended periods, representing a significant, but understudied occupational risk. This study collected personal black carbon (BC) exposures for 141 drivers across seven sectors in London. The aim of the study was to assess the magnitude and the primary determinants of their exposure, leading to the formulation of targeted exposure reduction strategies for the occupation. Each participant’s personal BC exposures were continuously measured using real-time monitors for 96 h, incorporating four shifts per participant. ‘At work’ BC exposures (3.1 ± 3.5 µg/m3) were 2.6 times higher compared to when ‘not at work’ (1.2 ± 0.7 µg/m3). Workers spent 19% of their time ‘at work driving’, however this activity contributed 36% of total BC exposure, highlighting the disproportionate effect driving had on their daily exposure. Taxi drivers experienced the highest BC exposures due to the time they spent working in congested central London, while emergency services had the lowest. Spikes in exposure were observed while driving and were at times greater than 100 µg/m3. The most significant determinants of drivers’ exposures were driving in tunnels, congestion, location, day of week and time of shift. Driving with closed windows significantly reduced exposures and is a simple behaviour change drivers could implement. Our results highlight strategies by which employers and local policy makers can reduce professional drivers’ exposure to traffic-related air pollution

    ‘I already have a culture.’ Negotiating competing grand and personal narratives in interview conversations with new study abroad arrivals

    In an interview with a postgraduate student about her intercultural experience of recently arriving for study abroad, it was found that the two researchers and the student were engaged in a mutual exploration of cultural identity. The in- terview events became conversational and took the form of small culture formation on the go in which each participant employed diverse narratives to project, make sense of and negotiate expression of cultural identity. The stu- dent shifted between personal narratives drawn from her particular cultural trajectories and splintered from grand narratives of nation and global position- ing, between non- essentialist threads and essentialist blocks. The researchers learned from her and intervened to facilitate shifts to non-essentialist threads, drawing on narratives from their own personal cultural trajectories, but some- times also falling into essentialist blocks splintered from grand narratives. The roles of ideology and competing essentialist and non-essentialist discourses of culture were implicit in these negotiations, as were the personal agency of the student as she responded to the constraining conflicts, structures and hierarchies encountered through the events she spoke about. Rather than providing a picture of intercultural assimilation and integration, interculturality is revealed as a hesitant and searching negotiation, sometimes of vulnerability, wrong-footedness and occasional assault on identity

    Rapid export of waters formed by convection near the Irminger Sea's western boundary

    The standard view of the overturning circulation emphasizes the role of convection, yet for waters to contribute to overturning, they must not only be transformed to higher densities but also exported equatorward. From novel mooring observations in the Irminger Sea (2014–2016), we describe two water masses that are formed by convection and show that they have different rates of export in the western boundary current. Upper Irminger Sea Intermediate Water appears to form near the boundary current and is exported rapidly within 3 months of its formation. Deep Irminger Sea Intermediate Water forms in the basin interior and is exported on longer time scales. The subduction of these waters into the boundary current is consistent with an eddy transport mechanism. Our results suggest that light intermediate waters can contribute to overturning as much as waters formed by deeper convection and that the export time scales of both project onto overturning variability. Plain Language Summary The deep ocean can regulate the Earth's climate by storing carbon and heat. At high latitudes, waters are cooled by the atmosphere and sink, but they can only be successfully stored in the deep ocean if they are exported toward the equator. In this study, we analyze new mooring observations in the Irminger Sea to investigate the cooling and export of high‐latitude waters. In addition to the well‐documented waters that are cooled in the center of the Irminger Sea, we find that saltier waters are cooled near the western boundary current. Both of these water types make it into boundary current and are exported. Our observations are consistent with the dynamics of swirling eddy motions. The eddy transport process is more effective for the waters cooled near the boundary current, implying that cooling near boundary currents may be more important for the climate than has been appreciated to date
