    Wearable device to assist independent living.

    Older people increasingly want to remain living independently in their own homes. The aim of the ENABLE project is to develop a wearable device that can be used both within and outside of the home to support older people in their daily lives and which can monitor their health status, detect potential problems, provide activity reminders and offer communication and alarm services. In order to determine the specifications and functionality required for development of the device user surveys and focus groups were undertaken and use case analysis and scenario modeling carried out. The project has resulted in the development of a wrist worn device and mobile phone combination that can support and assist older and vulnerable wearers with a range of activities and services both inside and outside of their homes. The device is currently undergoing pilot trials in five European countries. The aim of this paper is to describe the ENABLE device, its features and services, and the infrastructure within which it operates

    Evidence for ACTN3 as a genetic modifier of Duchenne muscular dystrophy

    Duchenne muscular dystrophy (DMD) is characterized by muscle degeneration and progressive weakness. There is considerable inter-patient variability in disease onset and progression, which can confound the results of clinical trials. Here we show that a common null polymorphism (R577X) in ACTN3 results in significantly reduced muscle strength and a longer 10\u2009m walk test time in young, ambulant patients with DMD; both of which are primary outcome measures in clinical trials. We have developed a double knockout mouse model, which also shows reduced muscle strength, but is protected from stretch-induced eccentric damage with age. This suggests that \u3b1-actinin-3 deficiency reduces muscle performance at baseline, but ameliorates the progression of dystrophic pathology. Mechanistically, we show that \u3b1-actinin-3 deficiency triggers an increase in oxidative muscle metabolism through activation of calcineurin, which likely confers the protective effect. Our studies suggest that ACTN3 R577X genotype is a modifier of clinical phenotype in DMD patients

    Twin Paradox and the logical foundation of relativity theory

    We study the foundation of space-time theory in the framework of first-order logic (FOL). Since the foundation of mathematics has been successfully carried through (via set theory) in FOL, it is not entirely impossible to do the same for space-time theory (or relativity). First we recall a simple and streamlined FOL-axiomatization SpecRel of special relativity from the literature. SpecRel is complete with respect to questions about inertial motion. Then we ask ourselves whether we can prove usual relativistic properties of accelerated motion (e.g., clocks in acceleration) in SpecRel. As it turns out, this is practically equivalent to asking whether SpecRel is strong enough to "handle" (or treat) accelerated observers. We show that there is a mathematical principle called induction (IND) coming from real analysis which needs to be added to SpecRel in order to handle situations involving relativistic acceleration. We present an extended version AccRel of SpecRel which is strong enough to handle accelerated motion, in particular, accelerated observers. Among others, we show that the Twin Paradox becomes provable in AccRel, but it is not provable without IND.Comment: 24 pages, 6 figure

    Evaluation of elicitation methods to quantify Bayes linear models

    The Bayes linear methodology allows decision makers to express their subjective beliefs and adjust these beliefs as observations are made. It is similar in spirit to probabilistic Bayesian approaches, but differs as it uses expectation as its primitive. While substantial work has been carried out in Bayes linear analysis, both in terms of theory development and application, there is little published material on the elicitation of structured expert judgement to quantify models. This paper investigates different methods that could be used by analysts when creating an elicitation process. The theoretical underpinnings of the elicitation methods developed are explored and an evaluation of their use is presented. This work was motivated by, and is a precursor to, an industrial application of Bayes linear modelling of the reliability of defence systems. An illustrative example demonstrates how the methods can be used in practice

    A framework for complex climate change risk assessment

    The approach encourages divergent thinking, which traverses sectoral and regional boundaries and recognizes links between physical and socio-economic drivers of risk. In this paper recent work describing complex climate change risk is synthesized. It includes concepts of compound, connected, and cascading interactions. It reflects on the consequences of risk assessment and response. It then establishes a framework for risk assessment that encompasses increasing levels of complexity. The framework is demonstrated through diverse case studies to illustrate how risk assessments can better consider and categorize complexity.     Airway delivery of both a BCG prime and adenoviral boost drives CD4 and CD8 T cells into the lung tissue parenchyma

    Abstract: Heterologous BCG prime-boost regimens represent a promising strategy for an urgently required improved tuberculosis vaccine. Identifying the mechanisms which underpin the enhanced protection induced by such strategies is one key aim which would significantly accelerate rational vaccine development. Experimentally, airway vaccination induces greater efficacy than parenteral delivery; in both conventional vaccination and heterologous boosting of parenteral BCG immunisation. However, the effect of delivering both the component prime and boost immunisations via the airway is not well known. Here we investigate delivery of both the BCG prime and adenovirus boost vaccination via the airway in a murine model, and demonstrate this approach may be able to improve the protective outcome over parenteral prime/airway boost. Intravascular staining of T cells in the lung revealed that the airway prime regimen induced more antigen-specific multifunctional CD4 and CD8 T cells to the lung parenchyma prior to challenge and indicated the route of both prime and boost to be critical to the location of induced resident T cells in the lung. Further, in the absence of a defined phenotype of vaccine-induced protection to tuberculosis; the magnitude and phenotype of vaccine-specific T cells in the parenchyma of the lung may provide insights into potential correlates of immunity
