Comprehensive characterization of a mesoporous cerium oxide nanomaterial with high surface area and high thermal stability
In the present study, the pore space of a mesoporous cerium oxide material is investigated, which forms by the self-assembly of primary particles into a spherical secondary structure possessing a disordered mesopore space. The material under study exhibits quite stable mesoporosity upon aging at high temperatures (800 °C) and is, thus, of potential interest in high-temperature catalysis. Here, different characterization techniques were applied to elucidate the structural evolution taking place between heat treatment at 400 °C and aging at 800 °C, i.e., in a water-containing atmosphere, which is usually detrimental to nanoscaled porosity. The changes in the mesoporosity were monitored by advanced physisorption experiments, including hysteresis scanning, and electron tomography analysis coupled with a 3D reconstruction of the mesopore space. These methods indicate that the 3D spatial arrangement of the primary particles during the synthesis under hydrothermal conditions via thermal hydrolysis is related to the thermal stability of the hierarchical mesopore structure. The assembly of the primary CeO particles (âŒ4 nm in size) results in an interparticulate space constituting an open 3D mesopore network, as revealed by skeleton analysis of tomography data, being in conformity with hysteresis scanning. At elevated temperatures (800 °C), sinter processes occur resulting in the growth of the primary particles, but the 3D mesopore network and the spherical secondary structure are preserved
EMT-Related Genes Have No Prognostic Relevance in Metastatic Colorectal Cancer as Opposed to Stage II/III: Analysis of the Randomised, Phase III Trial FIRE-3 (AIO KRK 0306; FIRE-3)
Introduction: There is no standard treatment after resection of colorectal liver metastases and the role of systemic therapy remains controversial. To avoid over- or undertreatment, proper risk stratification with regard to postoperative treatment strategy is highly needed. We recently demonstrated the prognostic relevance of EMT-related (epithelial-mesenchymal transition) genes in stage II/III CRC. As EMT is a major step in CRC progression, we now aimed to analyse the prognostic relevance of EMT-related genes in stage IV CRC using the study cohort of the FIRE-3 trial, an open-label multi-centre randomised controlled phase III trial of patients with metastatic CRC.
Methods: Overall and progression free survival were considered as endpoints (n = 350). To investigate the prognostic relevance of EMT-related genes on either endpoint, we compared predictive performance of different models using clinical data only to models using gene data in addition to clinical data, expecting better predictive performance if EMT-related genes have prognostic value. In addition to baseline models (Kaplan Meier (KM), (regularised) Cox), Random Survival Forest (RSF), and gradient boosted trees (GBT) were fit to the data. Repeated, nested five-fold cross-validation was used for hyperparameter optimisation and performance evaluation. Predictive performance was measured by the integrated Brier score (IBS).
Results: The baseline KM model showed the best performance (OS: 0.250, PFS: 0.251). None of the other models were able to outperform the KM when using clinical data only according to the IBS scores (OS: 0.253 (Cox), 0.256 (RSF), 0.284 (GBT); PFS: 0.254 (Cox), 0.256 (RSF), 0.276 (GBT)). When adding gene data, performance of GBT improved slightly (OS: 0.262 vs. 0.284; PFS: 0.268 vs. 0.276), however, none of the models performed better than the KM baseline.
Conclusion: Overall, the results suggest that the prognostic relevance of EMT-related genes may be stage-dependent and that EMT-related genes have no prognostic relevance in stage IV CRC
ChatGPT's Gastrointestinal Tumor Board Tango: A limping dance partner?
Objectives: This study aimed to assess the consistency and replicability of treatment recommendations provided by ChatGPT 3.5 compared to gastrointestinal tumor cases presented at multidisciplinary tumor boards (MTBs). It also aimed to distinguish between general and case-specific responses and investigated the precision of ChatGPTâs recommendations in replicating exact treatment plans, particularly regarding chemotherapy regimens and follow-up protocols. Material and methods: A retrospective study was carried out on 115 cases of gastrointestinal malignancies, selected from 448 patients reviewed in MTB meetings. A senior resident fed patient data into ChatGPT 3.5 to produce treatment recommendations, which were then evaluated against the tumor boardâs decisions by senior oncology fellows.
Results: Among the examined cases, ChatGPT 3.5 provided general information about the malignancy without considering individual patient characteristics in 19% of cases. However, only in 81% of cases, ChatGPT generated responses that were specific to the individual clinical scenarios. In the subset of case-specific responses, 83% of recommendations exhibited overall treatment strategy concordance between ChatGPT and MTB. However, the exact treatment concordance dropped to 65%, notably lower in recommending specific chemotherapy regimens. Cases recommended for surgery showed the highest concordance rates, while those involving chemotherapy recommendations faced challenges in precision.
Conclusions: ChatGPT 3.5 demonstrates potential in aligning conceptual approaches to treatment strategies with MTB guidelines. However, it falls short in accurately duplicating specific treatment plans, especially concerning chemotherapy regimens and ollow-up procedures. Ethical concerns and challenges in achieving exact replication necessitate prudence when considering ChatGPT 3.5 for direct clinical decision-making in MTBs

Das betrifft insbesondere die Energieeffizienz auf der Nachfrageseite, aber auch die Kraft-WĂ€rme-Kopplung und Erneuerbare Energien-WĂ€rme. FĂŒr die MaĂnahmenfelder "Fluorierte Treibhausgase", "Industrieprozesse" und "Landwirtschaft" mĂŒssen ĂŒberhaupt erst verbindliche Reduktionsziele festgelegt und Politikinstrumente eingefĂŒhrt werden
Outcomes and risks in palliative pancreatic surgery: an analysis of the German StuDoQ|Pancreas registry
BACKGROUND: Non-resectability is common in patients with pancreatic ductal adenocarcinoma (PDAC) due to local invasion or distant metastases. Then, biliary or gastroenteric bypasses or both are often established despite associated morbidity and mortality. The current study explores outcomes after palliative bypass surgery in patients with non-resectable PDAC. METHODS: From the prospectively maintained German StuDoQ|Pancreas registry, all patients with histopathologically confirmed PDAC who underwent non-resective pancreatic surgery between 2013 and 2018 were retrospectively identified, and the influence of the surgical procedure on morbidity and mortality was analyzed. RESULTS: Of 389 included patients, 127 (32.6%) underwent explorative surgery only, and a biliary, gastroenteric or double bypass was established in 92 (23.7%), 65 (16.7%) and 105 (27.0%). After exploration only, patients had a significantly shorter stay in the intensive care unit (mean 0.5Â days [SD 1.7] vs. 1.9 [3.6], 2.0 [2.8] or 2.1 [2.8]; Pâ<â0.0001) and in the hospital (median 7Â days [IQR 4â11] vs. 12 [10â18], 12 [8â19] or 12 [9â17]; Pâ<â0.0001), and complications occurred less frequently (22/127 [17.3%] vs. 37/92 [40.2%], 29/65 [44.6%] or 48/105 [45.7%]; Pâ<â0.0001). In multivariable logistic regression, biliary stents were associated with less major (ClavienâDindo gradeââ„âIIIa) complications (OR 0.49 [95% CI 0.25â0.96], Pâ=â0.037), whereasâcompared to exploration onlyâbiliary, gastroenteric, and double bypass were associated with more major complications (OR 3.58 [1.48â8.64], Pâ=â0.005; 3.50 [1.39â8.81], Pâ=â0.008; 4.96 [2.15â11.43], Pâ<â0.001). CONCLUSIONS: In patients with non-resectable PDAC, biliary, gastroenteric or double bypass surgery is associated with relevant morbidity and mortality. Although surgical palliation is indicated if interventional alternatives are inapplicable, or life expectancy is high, less invasive options should be considered. Management of Suspected Cases of Feline Immunodeficiency Virus Infection in Eurasian Lynx (Lynx lynx) During an International Translocation Program.
The Eurasian lynx (Lynx lynx) population in Switzerland serves as a source for reintroductions in neighboring countries. In 2016-2017, three lynx from the same geographical area were found seropositive for feline immunodeficiency virus (FIV) in the framework of an international translocation program. This novel finding raised questions about the virus origin and pathogenicity to lynx, the emerging character of the infection, and the interpretation of serological results in other lynx caught for translocation. Archived serum samples from 84 lynx captured in 2001-2016 were retrospectively tested for FIV antibodies by Western blot. All archived samples were FIV-negative. The three seropositive lynx were monitored in quarantine enclosures prior to euthanasia and necropsy. They showed disease signs, pathological findings, and occurrence of co-infections reminding of those described in FIV-infected domestic cats. All attempts to isolate and characterize the virus failed but serological data and spatiotemporal proximity of the cases suggested emergence of a lentivirus with antigenic and pathogenic similarities to FIV in the Swiss lynx population. A decision scheme was developed to minimize potential health risks posed by FIV infection, both in the recipient and source lynx populations, considering conservation goals, animal welfare, and the limited action range resulting from local human conflicts. Development and implementation of a cautious decision scheme was particularly challenging because FIV pathogenic potential in lynx was unclear, negative FIV serological results obtained within the first weeks after infection are unpredictable, and neither euthanasia nor repatriation of multiple lynx was acceptable options. The proposed scheme distinguished between three scenarios: release at the capture site, translocation, or euthanasia. Until April 2021, none of the 40 lynx newly captured in Switzerland tested FIV-seropositive. Altogether, seropositivity to FIV was documented in none of 124 lynx tested at their first capture, but three of them seroconverted in 2016-2017. Diagnosis of FIV infection in the three seropositive lynx remains uncertain, but clinical observations and pathological findings confirmed that euthanasia was appropriate. Our experiences underline the necessity to include FIV in pathogen screenings of free-ranging European wild felids, the importance of lynx health monitoring, and the usefulness of health protocols in wildlife translocation

During the last 40 years a focus in research was put on sutureless tissue fusion, to handle the limits of conventional suturing such as vascular wall damage due to the enetrating needle, intraluminal foreign body reactions caused by non-absorbable suture material and thrombocyte aggregation, impaired endothelial function, intimal hyperplasia and hence stenosis. Sutured wounds have greater and longer duration inïŹammatory response than laser soldered wounds. Furthermore suturing does not create a watertight connection, which can, for example, in visceral surgery lead to an entry for pathogens resulting in severe complications such as infections or death. The trend in modern surgery towards endoscopic and minimally invasive approaches requires adaptation of the surgical instruments and techniques. The method of tissue adaptation is therefore limited by free moving space thus revealing conventional suturing unsuitable. Fibrin based adhesives or glues achieve insufïŹcient tensile strength for suture replacement and are further limited by early resorption. These products can be used as temporary sealants or hemostatic agents. Cyanoacrylate based adhesives offer sufïŹcient tensile strength, but are limited by toxicity, allergic reaction, anaphylaxis and immediate polymerization properties, precluding a precise surgical application. Disseminated cancer cells detected by immunocytology in lymph nodes of NSCLC patients are highly prognostic and undergo parallel molecular evolution
In melanoma, immunocytology (IC) after sentinel lymph node disaggregation not only enables better quantification of disseminated cancer cells (DCCs) than routine histopathology (HP) but also provides a unique opportunity to detect, isolate, and analyse these earliest harbingers of metachronous metastasis. Here, we explored lymph node IC in non-small cell lung cancer (NSCLC). For 122 NSCLC patients, 220 lymph nodes (LNs) were split in half and prepared for IC and HP. When both methods were compared, IC identified 22% positive patients as opposed to 4.5% by HP, revealing a much higher sensitivity of IC (pâ<â0.001). Assessment of all available 2,952 LNs of the same patients by HP uncovered additional patients escaping detection of lymphatic tumour spread by IC alone, consistent with the concept of skip metastasis. A combined lymph node status of IC and complete HP on a larger cohort of patients outperformed all risk factors in multivariable analysis for prognosis (pâ<â0.001; RRâ=â2.290; CI 1.407â3.728). Moreover, isolation of DCCs and single-cell molecular characterization revealed that (1) LN-DCCs differ from primary tumours in terms of copy number alterations and selected mutations and (2) critical alterations are acquired during colony formation within LNs. We conclude that LN-IC in NSCLC patients when combined with HP improves diagnostic precision, has the potential to reduce total workload, and facilitates molecular characterization of lymphatically spread cancer cells, which may become key for the selection and development of novel systemic therapies. © 2022 The Authors.
