    Factors associated with the presence of tricuspid valve regurgitation in patients with systemic right ventricles following atrial switch

    Background: The development of significant tricuspid regurgitation (TR) is associated with an unfavorable clinical outcome in patients with systemic right ventricles. Increased knowledge about the factors contributing to its presence would help prevent its progression. Methods: This was a retrospective analysis of the factors predictive of significant TR in 60 patients with systemic right ventricles following an atrial switch procedure for complete transposition of the great arteries. Data from echocardiographic examinations, exercise radionuclide angiography, and myocardial perfusion imaging were analyzed. Results: Significant TR was present in 20% of patients. Compared to patients without significant TR, patients with significant TR were older at the time of surgery (p ≤ 0.001), with a higher body mass index (p ≤ 0.005), lower right ventricular ejection fraction (RVEF; p ≤ 0.01), higher exercise perfusion abnormalities score on radionuclide angiography (p ≤ 0.03), and higher systolic blood pressure (p ≤ 0.02). At univariate logistic regression analysis systolic blood pressure (p = 0.03), increasing age at surgery (p = 0.01), and RVEF (p = 0.02), were predictors of significant tricuspid regurgitation. The latter two remained significant at multivariate analysis. Conclusions: Patients operated upon later in life, with decreased RVEF and higher blood pressure, are at risk of significant tricuspid regurgitation and therefore warrant special attention. Prospective studies are needed to ascertain whether appropriate pharmacological intervention would prevent the development and/or progression of TR in these patients. (Cardiol J 2010; 17, 1: 29-34

    Subclinical target organ damage in patients with essential hypertension

    W ostatnich dekadach dokonał się znaczący postęp w możliwościach oceny struktury i funkcji układu sercowo-naczyniowego, który u chorych z nadciśnieniem tętniczym pierwotnym umożliwił ocenę wczesnych zmian w obrębie serca i naczyń. Za najbardziej charakterystyczne zmiany w sercu w przebiegu nadciśnienia uważa się pogrubienie ścian, wzrost masy mięśnia lewej komory, przebudowę komory oraz dysfunkcję rozkurczową. Przyjmuje się, że u chorych z nadciśnieniem tętniczym pogrubienie błony wewnętrznej i środkowej oceniane najczęściej w obrębie tętnicy szyjnej poprzedza rozwój ogniskowych zmian miażdżycowych. Badania rozpoczęte na początku lat 90. ubiegłego wieku dostarczyły dowodów wskazujących na rozwój dysfunkcji śródbłonka w przebiegu nadciśnienia tętniczego. Mikroalbuminuria należy do najwcześniejszych objawów uszkodzenia nerek w przebiegu nadciśnienia i zaliczana jest do najważniejszych czynników ryzyka sercowo-naczyniowego. Wyniki dotychczasowych badań wskazują na związek mikroalbuminurii zarówno z klasycznymi, jak i nowo poznanymi czynnikami ryzyka.Assessment of subclinical target organ damage is a key element in the management of patients with hypertension. Impaired diastolic function and increased LV mass are common findings in hypertensive patients and may occur early in the natural history of hypertension. Ultrasonographic measurements of intima-media thickness (IMT) in carotid arteries are being applied extensively and have been regarded as a valid indicator of atherosclerosis. Several studies showed association between IMT and hypertensive target organ damage, cardiovascular events and risk factors. Microalbuminuria is a marker for generalized vascular dysfunction, is the earliest sign of nephropathy and predicts target organ damage, notably renal disease. Hypertensive patients with microalbuminuria had a significantly higher prevalence of CAD, LV hypertrophy, MI and stroke. The modern goal of the treatment of hypertension is to prevent progression, or even to reverse, subclinical target organ damage

    The impact of pulmonary regurgitation on right ventricular size and function in patients with repaired tetralogy of Fallot and additional haemodynamic abnormalities

    Purpose: Right ventricular (RV) outflow tract obstruction (RVOTO) was demonstrated to be protective against RV dilatation in patients with repaired tetralogy of Fallot and chronic pulmonary regurgitation (PR). We hypothesised that the presence of additional haemodynamic abnormalities (more than mild tricuspid regurgitation, residual ventricular septal defect) reduces this protective association. Accordingly, we aimed to assess the impact of PR on RV size and function in this population. Material and methods: Consecutive patients with additional haemodynamic abnormalities after tetralogy of Fallot (TOF) repair, who had undergone cardiovascular magnetic resonance, were included. Results: Out of 90 patients studied, 18 individuals (mean age 32.5 ± 10.7 years, 72.2% males) met the inclusion criteria. There were no differences in RV volumes and ejection fraction between patients with and without RVOTO. Neither PR fraction (PRF) nor PR volume (PRV) correlated with RV end-diastolic volume (r = 0.36; p = 0.15 and r = 0.37; p = 0.14, respectively, for PRF and PRV) or RV end-systolic volume (r = 0.2; p = 0.42 and r = 0.19; p = 0.45, respectively, for PRF and PRV). Similarly, no significant correlations were observed between PRF or PRV and RV ejection fraction (r = –0.04; p = 0.87 and r = –0.03; p = 0.9, respectively). Conclusions: Additional haemodynamic abnormalities are associated with the abolition of the protective effect of RVOTO on RV size. There was no significant relationship between measures of PR and RV volumes in patients after TOF repair with concomitant haemodynamic abnormalities. These abnormalities acted as confounding factors in the assessment of the impact of pulmonary regurgitation on RV size and function

Prowadzony tryb życia ze szczególnym uwzględnieniem aktywności ruchowej, ściśle wiąże się z doborem odpowiednio zbilansowanej diety. Celem badań była ocena odżywiania się sportowców danej dyscypliny sportowej. Badaniem objęto 30 piłkarzy nożnych z klubu sportowego Olimpia Grudziądz, którzy wyrazili chęć wypełnienia ankiety w okresie od marca do maja 2016 roku. Ogólna ocena żywienia piłkarzy uprawiających piłkę nożną jest zadowalająca. Badania wykazały, że piłkarze spożywają posiłek mniej niż 1 godzinę po treningu, czyli odpowiedni czas niezbędny w szybkiej regeneracji glikogenu mięśniowego. Ocena spożywania warzyw i owoców jest nie w pełni prawidłowa, ponieważ są one spożywane w różnej częstotliwości i nie są spożywane codziennie. Racjonalny sposób żywienia sportowców wymaga systematycznego dostarczania organizmowi wszystkich niezbędnych składników odżywczych w ilościach i proporcjach odpowiadających jego potrzebom, a całodzienna racja pokarmowa powinna uwzględniać pięć posiłków w ciągu dnia. Abstract             Dietary nutrients play a significant role in the normal function of the body. Life style focused on physical activity necessitates appropriately balanced diet. The goal of this study was to assess the dietary habits of a selected group of athletes. Participants of the study were 30 male football players from the Olimpia Grudziadz athletic club, who, between March and May of 2016, agreed to fill out a questionnaire pertaining to their diets. Based on the compiled data from the questionnaire, the overall diet can be considered well-balanced with minor deficiencies. Participating athletes consume a meal rich in key nutrients within an hour of their training, the optimal time necessary to replenish muscle glycogen. This study revealed, however, that the number of servings of fruit and vegetables is below the recommended daily norm, and the fruit and vegetables are not consumed daily. To maximize performance and ensure rapid recovery necessitates carefully balanced and appropriately timed diet. It is recommended that the daily dietary regimen consists of five evenly spaced meals rich in nutrients that prepare the athlete for the strenuous physical activity and replenish depleted energy stores during recovery period

    The running pattern and its importance in running long-distance gears

    The running pattern is individual for each runner, regardless of distance. We can characterize it as the sum of the data of the runner (age, height, training time, etc.) and the parameters of his run. Building the proper technique should focus first and foremost on the work of movement coordination and the power of the runner. In training the correct running steps we can use similar tools as working on deep feeling.The aim of this paper was to define what we can call a running pattern, what is its influence in long-distance running, and the relationship between the training technique and the running pattern.The importance of a running pattern in long-distance racing is immense, as the more distracted and departed from the norm, the greater the harm to the body will cause it to repetition in long run. Putting on training exercises that shape the technique is very important and affects the running pattern significantly

    Influence of percutaneous pulmonary valve implantation on exercise capacity: Which group of patients benefits most from the intervention?

    Background: The aim of the study was to evaluate the role of cardiopulmonary exercise testing (CPET) parameters in assessing exercise capacity improvement after percutaneous pulmonary valve implantation (PPVI). Additionally, it aimed to determine if there are any baseline characteristics influencing that change. Methods and results: The study comprised 32 patients (mean age 26 ± 9); 53% males; diagnosis: tetralogy of Fallot (n = 18), pulmonary atresia (n = 6), Ross procedure (n = 4), other (transposition of great arteries, pulmonary stenosis, double outlet right ventricle, common arterial trunk type II — n = 4) who underwent successful PPVI due to right ventricular out­flow tract dysfunction (predominant pulmonary regurgitation — n = 17, predominant pulmo­nary stenosis — n = 15). Treadmill CPET was performed before and a year after PPVI along with clinical evaluation, cardiac magnetic resonance and transthoracic echocardiography. Twelve months post successful PPVI (pulmonary valve competence restoration and pulmonary gradient reduction from 58.8 ± 47.1 to 26.6 ± 10.8 mm Hg) there was a significant decrease in the ventilatory equivalent for CO2 at peak exercise (EQCO2) (25.3 ± 3.3 to 24.3 ± 3.0, p = 0.04) and oxygen consumption at peak exercise (pVO2) (20.4 ± 5.0 to 22.6 ± 5.3 mL/kg/min, p = 0.04). Improved EQCO2 correlated with an increase in right and left ventricular ejection fraction (respectively R = –0.57, p = 0.002; R = –0.56, p = 0.002). In this study, no baseline factors that might affect improvement in exercise function were found. Conclusions: Successful PPVI leads to an improvement in exercise capacity and hemodynamic response to exercise. The correlation between the improvement in EQCO2 or peak VO2 and baseline characteristics was too weak to reliably identify the group of patients that will benefit from the procedur

    QTc prolongation in patients with hearing loss: Electrocardiographic and genetic study

    Background: The aim of the study was to determine, whether electrocardiogram (ECG) screening could reduce the risk of sudden cardiac death in patients with hearing loss through the early diagnosis of Jervell and Lange-Nielsen syndrome and the introduction of the therapy. Methods: One thousand and eighty patients with hearing loss (aged 21.8 ± 19.9 years) underwent ECG. Additionally, all subjects were asked to complete a 3-question survey. Those who met, at least, one of the high-risk criteria underwent further cardiac assessment and genetic testing. Results: QTc assessment was possible in 1,027 patients. Mean QTc measured 422.8 ± 23.7 ms in 313 women, 414.9 ± 27.7 ms in 273 men and 421.1 ± 21.5 ms in 441 children (individuals younger than 14 years). Abnormal QTc was found in 13 (4.1%) women, 20 (7.3%) men, and 72 (16.3%) children. In the studied group, no recessive mutation of KNCQ1 or KCNE1 was found. In 6 patients, other mutations were found: in KCNQ1 (n = 1), in KCNH2 (n = 3) and in SCN5A (n = 1), which were pathogenic for long-QT-syndromes (LQTS), and 2 mutations of unknown clinical significance in SCN5A. Overall, out of these 6 patients LQTS was diagnosed in 3 asymptomatic patients, but with abnormal QTc and in 2 patients with normal QTc, but who were previously treated for epilepsy. Conclusions: Jervell and Lange-Nielsen syndrome is a very rare condition even in a population with hearing loss. In this population, the prevalence of prolonged QT interval is increased over the general population. Further investigations are necessary.
