Seksuaaliterveyden edistämisen osaaminen hoitotyössä : osaamisen määrittely ja mittaaminen
Seksuaaliterveys on tärkeä osa potilaan hyvinvointia, ja se tulisi ottaa huomioon potilaan kokonaisvaltaisessa hoidossa. Seksuaaliterveyteen on alettu kiinnittää huomiota myös kansainvälisesti seksuaalisuuteen liittyvien ongelmien lisäännyttyä. Kuitenkin tutkimuksissa on todettu, että potilaat eivät saa tarvitsemaansa seksuaalineuvontaa.
Tutkimuksen tarkoituksena oli kerätä systemaattisesti tutkittua tietoa seksuaaliterveyden edistämisen osaamisen määrittelystä ja mittaamisesta hoitotyössä. Tutkimus on osa Helsingin ammattikorkeakoulu Metropolian hanketta, jonka tarkoitus on kehittää seksuaaliterveyden edistämisen osaamista metropolialueella ja hoitotyön koulutuksessa. Tutkimusmenetelmänä oli sovellettu systemaattinen kirjallisuuskatsaus. Kirjallisuuskatsauksen avulla on hankkeessa tarkoituksena jäsentää ja määrittää seksuaaliterveyden edistämisen osaamisen tietoperusta ja osaamisen kriteerit, joiden avulla seksuaaliterveyden edistämisen osaamista voidaan arvioida ja mitata hoitotyössä.
Tutkimuksen tulokset osoittavat, että seksuaaliterveyden edistämisen osaamista on määritelty useimmiten hoitajien arvojen ja asenteiden sekä tietojen ja taitojen kautta. Mittaamisessa on käytetty useita mittareita, jotka ovat useimmiten mitanneet vain yhtä osaamisen aluetta, kuten asenteita tai tietoja seksuaalisuudesta. Vain muutamassa tutkimuksessa on selvitetty seksuaaliterveyden edistämisen osaamista kokonaisuudessaan.
Analysoimiemme tutkimusartikkeleiden mukaan hoitajat kokevat seksuaaliterveyden edistämisen tärkeäksi, mutta eivät kuitenkaan toteuta seksuaalineuvontaa käytännön hoitotyössä. Hoitajien tiedot ja taidot seksuaaliterveyden edistämisestä ovat puutteelliset ja henkilökohtaiset arvot ja asenteet esimerkiksi seksuaalivähemmistöjä kohtaan saattavat vaikuttaa hoitotyöhön. Osaamisen arvioimiseksi ja mittaamiseksi tarvitaan tarkoitusta varten kehitetty mittari, jonka avulla pystytään arvioimaan hoitajien seksuaaliterveyden edistämisen osaamista ja tämän myötä myös kehittämään osaamista. Myös seksuaaliterveyden edistämisen koulutusta on kehitettävä.Sexual health is an integral part of the well-being of a patient and should be taken into account in patient care. Recently, sexual health has been the subject of international attention with the rising amount of sexuality-related health issues. Despite this, studies have shown that patients feel like they are not receiving competent sexual counselling.
The purpose of this study was to gather systematically researched information about the defining and assessing of sexual health promotion competence in nursing. Our study is part of the Helsinki Metropolia University of Applied Sciences project designed to develop the skill of sexual health promotion and the education in sexual health promotion in the Helsinki Metropolitan area. An applied systematic literature review was used as the research method. The literature review will be used later in the project for analysing and defining the knowledge base and skill criteria in the promotion of sexual health.
The results showed that knowledge and skills in the matter were defined through the values, attitudes, knowledge and skills of the registered nurses. Several indicators were used for studying the sexual health promotion competence, although usually only one singular subject was under research, e.g. attitudes or general knowledge of sexuality. Very few studies were carried out to research the overall skills in sexual health promotion as a whole.
Registered nurses felt that the sexual health promotion was important, yet they did not apply sexual health promotion in their day-to-day work. The knowledge and skills of the nursing community in the subject were incomplete, and the attitudes are often prejudiced. In order to inclusively evaluate the overall competence in sexual health promotion, there is a need for a new evaluation method. Vocational education in sexual health promotion needs to be developed as well

Multi-ancestry genome-wide gene–sleep interactions identify novel loci for blood pressure
Long and short sleep duration are associated with elevated blood pressure (BP), possibly through effects on molecular pathways that influence neuroendocrine and vascular systems. To gain new insights into the genetic basis of sleep-related BP variation, we performed genome-wide gene by short or long sleep duration interaction analyses on four BP traits (systolic BP, diastolic BP, mean arterial pressure, and pulse pressure) across five ancestry groups in two stages using 2 degree of freedom (df) joint test followed by 1df test of interaction effects. Primary multi-ancestry analysis in 62,969 individuals in stage 1 identified three novel gene by sleep interactions that were replicated in an additional 59,296 individuals in stage 2 (stage 1 + 2 Pjoint < 5 × 10-8), including rs7955964 (FIGNL2/ANKRD33) that increases BP among long sleepers, and rs73493041 (SNORA26/C9orf170) and rs10406644 (KCTD15/LSM14A) that increase BP among short sleepers (Pint < 5 × 10-8). Secondary ancestry-specific analysis identified another novel gene by long sleep interaction at rs111887471 (TRPC3/KIAA1109) in individuals of African ancestry (Pint = 2 × 10-6). Combined stage 1 and 2 analyses additionally identified significant gene by long sleep interactions at 10 loci including MKLN1 and RGL3/ELAVL3 previously associated with BP, and significant gene by short sleep interactions at 10 loci including C2orf43 previously associated with BP (Pint < 10-3). 2df test also identified novel loci for BP after modeling sleep that has known functions in sleep-wake regulation, nervous and cardiometabolic systems. This study indicates that sleep and primary mechanisms regulating BP may interact to elevate BP level, suggesting novel insights into sleep-related BP regulation
