    Towards a formative assessment of classroom competencies (FACCs) for postgraduate medical trainees

    Background An assumption of clinical competency is no longer acceptable or feasible in routine clinical practice. We sought to determine the feasibility, practicability and efficacy of undertaking a formal assessment of clinical competency for all postgraduate medical trainees in a large NHS foundation trust. Methods FY1 doctors were asked to complete a questionnaire to determine prior experience and self reported confidence in performing the GMC core competencies. From this a consensus panel of key partners considered and developed an 8 station Objective Structured Clinical Examination (OSCE) circuit to assess clinical competencies in all training grade medical staff... The OSCE was then administered to all training grade doctors as part of their NHS trust induction process. Results 106 (87.6% of all trainees) participated in the assessment during the first 14 days of appointment. Candidates achieved high median raw percentage scores for the majority of stations however analysis of pre defined critical errors and omissions identified important areas for concern. Performance of newly qualified FY1 doctor was significantly better than other grades for the arterial blood gas estimation and nasogastric tube insertion stations. Discussion Delivering a formal classroom assessment of clinical competencies to all trainees as part of the induction process was both feasible and useful. The assessment identified areas of concern for future training and also served to reassure as to the proficiency of trainees in undertaking the majority of core competencies

    Accomplishments of the NASA Johnson Space Center portion of the soil moisture project in fiscal year 1981

    The NASA/JSC ground scatterometer system was used in a row structure and row direction effects experiment to understand these effects on radar remote sensing of soil moisture. Also, a modification of the scatterometer system was begun and is continuing, to allow cross-polarization experiments to be conducted in fiscal years 1982 and 1983. Preprocessing of the 1978 agricultural soil moisture experiment (ASME) data was completed. Preparations for analysis of the ASME data is fiscal year 1982 were completed. A radar image simulation procedure developed by the University of Kansas is being improved. Profile soil moisture model outputs were compared quantitatively for the same soil and climate conditions. A new model was developed and tested to predict the soil moisture characteristic (water tension versus volumetric soil moisture content) from particle-size distribution and bulk density data. Relationships between surface-zone soil moisture, surface flux, and subsurface moisture conditions are being studied as well as the ways in which measured soil moisture (as obtained from remote sensing) can be used for agricultural applications

    Mosquito Populations from Eastern South Dakota During 2001 and 2002

    In 2001, the South Dakota Department of Health initiated a program to monitor mosquitoes in South Dakota for the presence of the West Nile Virus. During the first year (2001), a pilot survey was conducted near Brookings, SD. To collect mosquitoes, CDC miniture light traps were used without carbondioxide baiting, beginning on July 3 and ending on August 2, 2001. Results from this small study (total of 2,042 mosquitoes during 10 collection days) showed that the most common mosquotes were Aedes vexans (88.2%), Culex tarsalis, (5.2%), and Aedes dorsalis (4.9%). An additional survey was conducted during the summer of 2002 focusing on 8 different sites (Brandon, Brookings, Huron, North Sioux City, Oak Lake Field Station, Watertown, Waubay and Yankton) in eastern South Dakota. Mosquitoes were collected with the same traps used for 2001, however, the traps were baited with carbon diaoxide (dry ice). Trapping began on June 1 and ended on September 1, 2002. A total of 18,971 mosquitoes were collected from the 8 sites during the 127 trapping days of this survey. From this population, 21 different mosquito species from 8 different genera were identified. The vast majority of mosquitoes were Aedes vexans (86.3%), but Culex tarsalis was also present in significant numbers (7.2%). Aedes vexans populations varied to a greater degree during the summer than did Culex tarsalis. The public’s perceptions of the danger of West Nile Virus transmission is probably more determined by Aedes population than by Culex populations even though Aedes likely plays little or no role in the transmission of this disease

    Innate lymphoid cells (ILCs) are tissue-resident sentinels that are essential for early host protection from pathogens at initial sites of infection. However, whether pathogen-derived antigens directly modulate the responses of tissue-resident ILCs has remained unclear. In the present study, it was found that liver-resident type 1 ILCs (ILC1s) expanded locally and persisted after the resolution of infection with mouse cytomegalovirus (MCMV). ILC1s acquired stable transcriptional, epigenetic and phenotypic changes a month after the resolution of MCMV infection, and showed an enhanced protective effector response to secondary challenge with MCMV consistent with a memory lymphocyte response. Memory ILC1 responses were dependent on the MCMV-encoded glycoprotein m12, and were independent of bystander activation by proinflammatory cytokines after heterologous infection. Thus, liver ILC1s acquire adaptive features in an MCMV-specific manner

    Visual and refractive outcome of one-site phacotrabeculectomy compared with temporal approach phacoemulsification

    Background We aimed to compare visual and refractive outcome following phacoemulsification and intraocular lens implant (IOL) and combined one-site phacotrabeculectomy. Method We performed a retrospective study of case records of patients who had temporal incision phacoemulsification with IOL or one-site phacotrabeculectomy, between June 1997 and June 2001. The patients were matched for age group, operating list and IOL type. All patients were operated on under local anesthesia by the same surgeon. Each arm of the study had 90 patients, age range 60 to 75 years. We collected pre- and postoperative visual acuity, pre- and postoperative refraction within six months after surgery, and intended refraction. The intraocular pressure control was not recorded, as it was not the aim of our study. Results In the phacotrabeculectomy group, 76.6% of patients achieved aimed spherical equivalent, 15.5% of patients had against-the-rule (ATR) astigmatism induced by the surgery, and 90% of the patients had best corrected visual acuity (BCVA) more than 6/12. In the temporal incision phacoemulsification group, 81.1% of patients achieved aimed spherical equivalent, 10% of the patients had induced ATR by the surgery and 95.55% of patients achieved BCVA more than 6/12. Conclusion In this study the visual outcome of the phacotrabeculectomy group did not differ significantly from the visual outcome of temporal approach phacoemulsification

    On Synergies Between Information Retrieval and Digital Libraries

    In this paper we present the results of a longitudinal analysis of ACM SIGIR papers from 2003 to 2017. ACM SIGIR is the main venue where Information Retrieval (IR) research and innovative results are presented yearly; it is a highly competitive venue and only the best and most relevant works are accepted for publication. The analysis of ACM SIGIR papers gives us a unique opportunity to understand where the field is going and what are the most trending topics in information access and search. In particular, we conduct this analysis with a focus on Digital Library (DL) topics to understand what is the relation between these two fields that we know to be closely linked. We see that DL provide document collections and challenging tasks to be addressed by the IR community and in turn exploit the latest advancements in IR to improve the offered services. We also point to the role of public investments in the DL field as one of the core drivers of DL research which in turn may also have a positive effect on information accessing and searching in general
