    Progesterone-Related Issues and Coping Strategies for Women Undergoing Assisted Reproductive Treatment

    Objective:This study aims to identify the problems experienced by women undergoing assisted reproductive treatment due to progesterone use, as well as their coping strategies.Methods:The study included 145 women who were admitted to an infertility outpatient clinic of a university hospital between July 25 and August 8, 2022. The researchers collected data using a questionnaire designed to assess the problems that might arise due to the use of various types of progesterone-containing drugs and coping methods. The data were evaluated using descriptive statistics in the SPSS program.Results:The mean age of the women was 30.76±5.53 years, and the mean duration of their marriages was 6.6±4.51 years. Of women 64.8% had primary infertility and 68.3% of women underwent assisted reproductive treatment (ART) at least once before. Women who received intramuscular progesterone treatment during ART frequently experienced pain, swelling, abscess, redness at the injection site. Women who received progesterone treatment vaginally experienced vaginal wetness, drying of the drug in the vagina, lumps, the drug leaving the body, the drug not being completely absorbed, vaginal itching. Women who received progesterone treatment orally experienced sleepiness, nausea, dizziness, headache, fatigue. Women coped with these problems using various methods.Conclusion:The study revealed that women undergoing ART experienced many problems related to the use of progesterone and they attempted to address these issues through their own means. It is necessary to inform patients and to plan nursing interventions to minimize/eliminate these problems

    Extremely Preterm Infant Admissions Within the SafeBoosC-III Consortium During the COVID-19 Lockdown

    Objective: To evaluate if the number of admitted extremely preterm (EP) infants (born before 28 weeks of gestational age) differed in the neonatal intensive care units (NICUs) of the SafeBoosC-III consortium during the global lockdown when compared to the corresponding time period in 2019. Design: This is a retrospective, observational study. Forty-six out of 79 NICUs (58%) from 17 countries participated. Principal investigators were asked to report the following information: (1) Total number of EP infant admissions to their NICU in the 3 months where the lockdown restrictions were most rigorous during the first phase of the COVID-19 pandemic, (2) Similar EP infant admissions in the corresponding 3 months of 2019, (3) the level of local restrictions during the lockdown period, and (4) the local impact of the COVID-19 lockdown on the everyday life of a pregnant woman. Results: The number of EP infant admissions during the first wave of the COVID-19 pandemic was 428 compared to 457 in the corresponding 3 months in 2019 (−6.6%, 95% CI −18.2 to +7.1%, p = 0.33). There were no statistically significant differences within individual geographic regions and no significant association between the level of lockdown restrictions and difference in the number of EP infant admissions. A post-hoc analysis based on data from the 46 NICUs found a decrease of 10.3%in the total number of NICU admissions (n = 7,499 in 2020 vs. n = 8,362 in 2019). Conclusion: This ad hoc study did not confirm previous reports of a major reduction in the number of extremely pretermbirths during the first phase of the COVID-19 pandemic.     Katıldığı seferler sayesinde yarım yüzyılı aşkın süre boyunca araştırma ve gözlem yapma imkânı bulan Sinan, kendinden önceki çeşitli kültürlere ilişkin eserleri izlemiş ancak hiçbir kopyacılığa ve taklitçiliğe başvurmadan gözlemlerini sentezlemeyi, kendi üslubunu yaratmayı başarmıştır. Ömrünün yarısını gözlem, araştırma ve deneyime adayan Sinan’ın, analizci döneminde kubbeyi iyi inceleyip kubbe sorunlarını çözebilecek düzeyde olgunluğa ulaşmıştır. Aynı yüzyılda Batı dünyasında var olan Mimar Andrea Palladio tasarımlarında Erken Rönesans’ın kübik hacim, yalın cephe hatlarını kullanmıştır. Simetrik cepheli yarım sütunların üstünde üçgen alınlıklı, yüz binlerce ev, kilise ve kamusal yapının kökleri yine Andrea Palladio’nun tasarımlarına dayanmaktadır. Kendisi tarihte en çok taklit edilen mimarlardan biridir. Bu iki mimarın mekan algısındaki sınırlama ve farkındalıkların başarısı yapıları incelendiğinde net bir şekilde ortaya çıkmaktadır

    Gönüllülerin, ilgili mevzuat hükümleri gereğince bilgilendirilmiş gönüllü olur formunu imzalayarak uygun bilgilendirme şartları altında klinik araştırmaya katılmayı kabul etmesi gerekmektedir. İyi Klinik Uygulamaları Kılavuzu’nda ‘Gönüllü veya kanuni temsilcisi, araştırmanın ilgili bütün yönleri hakkında bilgilendirmelidir.’ hükmü bulunmakta olup bilgilendirilmiş gönüllü olur formunun, araştırmanın amacı ve tasarımı hakkındaki bilginin yanı sıra, gönüllünün kendine uygulanacak tanı ve tedavi yöntemlerinin kapsamını da içermesi gerekmektedir. Bu nedenle uygulanacak tetkik ve tedavi yöntemleri kapsamında gönüllülerden alınan biyolojik materyallerin ve bu örneklerin değerlendirilmesi ile elde edilen verilerin kullanımı konusunda gönüllüler bilgilendirilmelidir. Örnekler ve veriler ancak doğru ve net bilgilendirme şartları altında ve gönüllünün açık rızası ile kullanılabilir. Ancak bilgilendirilmiş gönüllü olur formlarında örnek ve verilerin alınma amacı dışında kullanılmasına ve paylaşılmasına olanak verildiği durumlarla da karşılaşılabilmektedir. Bu uygunsuz kullanım ve olur işlemleri gönüllü haklarının ihlaline ve hukuka aykırı durumların ortaya çıkmasına neden olmaktadır. Dolayısıyla yürürlükte olan yasal düzenlemelerin bilinmesi ve uygulanması suretiyle gönüllülerin haklarının etik ilkeler ve hukuki çerçevede korunması gerekmektedir.Volunteers should participate in clinical research by signing the informed consent form in accordance with the relevant legislation which must be accepted under the appropriate disclosure requirements. Since the “Good Clinical Practice Guidelines” states that “Voluntary or legal representative must be informed about all aspects of the research”, informed consent form should include not only the purpose and design of the research, but also, the scope of diagnostic and therapeutic methods as well. Therefore, volunteers should be informed about the use of the data that would be obtained by the examination of biological materials which were taken from them and from the evaluation of these samples. The samples and the data shall only be used under volunteer’s clear consent which is subject to accurate and clear disclosure requirements. However, sometimes in the informed consent forms, one can face with the circumstances that allow the use and share of the samples as well as the data, in contrary to the purpose of taking them. This improper use and consent proceedings cause the violation of the volunteer’s right and the law. Therefore, the volunteer’s rights should be respected in line with the ethical principles and the legal framework by knowing and applying the due regulations

    Variational Measurement Update for Extended Object Tracking Using Gaussian Processes

    We present an alternative inference framework for the Gaussian process-based extended object tracking (GPEOT) models. The method provides an approximate solution to the Bayesian filtering problem in GPEOT by relying on a new measurement update, which we derive using variational Bayes techniques. The resulting algorithm effectively computes approximate posterior densities of the kinematic and the extent states. We conduct various experiments on simulated and real data and examine the performance compared with a reference method, which employs an extended Kalman filter for inference. The proposed algorithm significantly improves the accuracy of both the kinematic and the extent estimates and proves robust against model uncertainties
