    Blind Benchmark Exercise for Spent Nuclear Fuel Decay Heat

    The decay heat rate of five spent nuclear fuel assemblies of the pressurized water reactor type were measured by calorimetry at the interim storage for spent nuclear fuel in Sweden. Calculations of the decay heat rate of the five assemblies were performed by 20 organizations using different codes and nuclear data libraries resulting in 31 results for each assembly, spanning most of the current state-of-the-art practice. The calculations were based on a selected subset of information, such as reactor operating history and fuel assembly properties. The relative difference between the measured and average calculated decay heat rate ranged from 0.6% to 3.3% for the five assemblies. The standard deviation of these relative differences ranged from 1.9% to 2.4%

    Factors impacting employment for people with autism spectrum disorder: A scoping review

    The aim of this study is to holistically synthesise the extent and range of literature relating to the employment of individuals with autism spectrum disorder. Database searches of Medline, CINAHL, PsychINFO, Scopus, ERIC, Web of Science and EMBASE were conducted. Studies describing adults with autism spectrum disorder employed in competitive, supported or sheltered employment were included. Content analysis was used to identify the strengths and abilities in the workplace of employees with autism spectrum disorder. Finally, meaningful concepts relating to employment interventions were extracted and linked to the International Classification of Functioning, Disability and Health Core Sets for autism spectrum disorder. The search identified 134 studies for inclusion with methodological quality ranging from limited to strong. Of these studies, only 36 evaluated employment interventions that were coded and linked to the International Classification of Functioning, Disability and Health, primarily focusing on modifying autism spectrum disorder characteristics for improved job performance, with little consideration of the impact of contextual factors on work participation. The International Classification of Functioning, Disability and Health Core Sets for autism spectrum disorder are a useful tool in holistically examining the employment literature for individuals with autism spectrum disorder. This review highlighted the key role that environmental factors play as barriers and facilitators in the employment of people with autism spectrum disorder and the critical need for interventions which target contextual factors if employment outcomes are to be improved

    Human Milk Secretory Antibodies against Attaching and Effacing Escherichia coli Antigens

    Secretory immunoglobulin A (sIgA) is a primary factor responsible for preventing attachment of enteropathogens to gut epithelium in breastfeeding infants. We compared the frequency of sIgA to major surface antigens of enterohemorrhagic Escherichia coli (EHEC) in milk of 123 women from the United States and Mexico to determine whether regional differences existed in the frequency of antibodies to these surface antigens. In both groups of women, milk commonly has sIgA against various EHEC lipopolysaccharides, EspA, EspB, intimin, and less frequently against Shiga toxin. The study suggests that persons living in the U.S. are exposed to attaching/effacing enteropathogens more frequently than is generally assumed. The low frequency of antibodies to Stx1 (in 12% of Mexican and in 22% of U.S. samples) suggests that the rare appearance of hemolytic uremic syndrome in adults is not due to neutralization of toxin at the gut level. Only anti-EspA is found in most milk samples from both populations of women. EspA may represent a useful target for an immunization strategy to prevent EHEC disease in humans

    Evaluating the Effectiveness of an Autism-Specific Workplace Tool for Employers: A Randomised Controlled Trial

    A randomised controlled trial evaluated the effectiveness of the Integrated Employment Success Tool (IEST™) in improving employers’ self-efficacy in modifying the workplace for individuals on the autism spectrum. Employers (N = 84) were randomised to the IEST™ or support as usual groups. Measurements of self-efficacy, knowledge and attitudes towards disability in the workplace were obtained at baseline and post-test. Results revealed a significant improvement in self-efficacy within the IEST™ group between baseline and post-test (p = 0.016). At post-test, there were no significant differences between groups in relation to self-efficacy in implementing autism-specific workplace modifications and employer attitudes towards disability in the workplace. Given the lack of significant outcomes, further research is needed to determine the effectiveness of the IEST™ for employers

    Global, regional, and national disability-adjusted life-years (DALYs) for 333 diseases and injuries and healthy life expectancy (HALE) for 195 countries and territories, 1990–2016: a systematic analysis for the Global Burden of Disease Study 2016

    BACKGROUND: Measurement of changes in health across locations is useful to compare and contrast changing epidemiological patterns against health system performance and identify specific needs for resource allocation in research, policy development, and programme decision making. Using the Global Burden of Diseases, Injuries, and Risk Factors Study 2016, we drew from two widely used summary measures to monitor such changes in population health: disability-adjusted life-years (DALYs) and healthy life expectancy (HALE). We used these measures to track trends and benchmark progress compared with expected trends on the basis of the Socio-demographic Index (SDI). METHODS: We used results from the Global Burden of Diseases, Injuries, and Risk Factors Study 2016 for all-cause mortality, cause-specific mortality, and non-fatal disease burden to derive HALE and DALYs by sex for 195 countries and territories from 1990 to 2016. We calculated DALYs by summing years of life lost and years of life lived with disability for each location, age group, sex, and year. We estimated HALE using age-specific death rates and years of life lived with disability per capita. We explored how DALYs and HALE differed from expected trends when compared with the SDI: the geometric mean of income per person, educational attainment in the population older than age 15 years, and total fertility rate. FINDINGS: The highest globally observed HALE at birth for both women and men was in Singapore, at 75·2 years (95% uncertainty interval 71·9-78·6) for females and 72·0 years (68·8-75·1) for males. The lowest for females was in the Central African Republic (45·6 years [42·0-49·5]) and for males was in Lesotho (41·5 years [39·0-44·0]). From 1990 to 2016, global HALE increased by an average of 6·24 years (5·97-6·48) for both sexes combined. Global HALE increased by 6·04 years (5·74-6·27) for males and 6·49 years (6·08-6·77) for females, whereas HALE at age 65 years increased by 1·78 years (1·61-1·93) for males and 1·96 years (1·69-2·13) for females. Total global DALYs remained largely unchanged from 1990 to 2016 (-2·3% [-5·9 to 0·9]), with decreases in communicable, maternal, neonatal, and nutritional (CMNN) disease DALYs offset by increased DALYs due to non-communicable diseases (NCDs). The exemplars, calculated as the five lowest ratios of observed to expected age-standardised DALY rates in 2016, were Nicaragua, Costa Rica, the Maldives, Peru, and Israel. The leading three causes of DALYs globally were ischaemic heart disease, cerebrovascular disease, and lower respiratory infections, comprising 16·1% of all DALYs. Total DALYs and age-standardised DALY rates due to most CMNN causes decreased from 1990 to 2016. Conversely, the total DALY burden rose for most NCDs; however, age-standardised DALY rates due to NCDs declined globally. INTERPRETATION: At a global level, DALYs and HALE continue to show improvements. At the same time, we observe that many populations are facing growing functional health loss. Rising SDI was associated with increases in cumulative years of life lived with disability and decreases in CMNN DALYs offset by increased NCD DALYs. Relative compression of morbidity highlights the importance of continued health interventions, which has changed in most locations in pace with the gross domestic product per person, education, and family planning. The analysis of DALYs and HALE and their relationship to SDI represents a robust framework with which to benchmark location-specific health performance. Country-specific drivers of disease burden, particularly for causes with higher-than-expected DALYs, should inform health policies, health system improvement initiatives, targeted prevention efforts, and development assistance for health, including financial and research investments for all countries, regardless of their level of sociodemographic development. The presence of countries that substantially outperform others suggests the need for increased scrutiny for proven examples of best practices, which can help to extend gains, whereas the presence of underperforming countries suggests the need for devotion of extra attention to health systems that need more robust support.     Nivel de satisfacción de usuarios hospitalizados en servicio de cirugía del hospital regional Lambayeque – 2018

    OBJECTIVE: To determine the level of satisfaction of recently discharged users from the surgery service of the Hospital Regional Lambayeque during 2018. METHODS: A cross-sectional descriptive study was designed; the population were 751 people and a non probabilistic sample of 248 users were selected. The modified SERVQUAL questionnaire was used as the instrument; it has 5 dimen-sions: reliability, responsiveness, empathy, security and tangible aspects. RESULTS: 64.71% were women. The mean of age was 39; 90.59% had SIS insurance affiliation and the average hospitalization time in days was 6; 76.61% were satisfied with the service; the dimension “Tangible aspects” showed the highest percentage of satisfaction (83.02%). The dimension whith the lowest percentage of satisfaction was “response capacity” (66.09%), especially in the aspect that refers to “the procedures for discharge are fast” (53.88%). CONCLUSIONS: in recently discharged users from the surgery service of the Hospital Regional Lambayeque, a high level of satisfac-tion was found during 2018OBJETIVO: determinar el nivel de satisfacción de los usuarios hospitalizados en el servicio de cirugía en el Hospital Regional Lambayeque en el año 2018. MÉTODOS: Se diseñó un estudio descriptivo transversal, se encuestó a una muestra de 255 usuarios de una población de 751. El instrumento utilizado fue el cuestionario SERVQUAL modificado, que cuenta con 5 dimensiones: fiabilidad, capacidad de respuesta, empatía, seguridad y aspectos tangibles. RESULTADOS: La mayoría de los usuarios entrevistados (64.71%) fueron mujeres. La edad promedio de los usuarios fue 39, el 90.59% contaban con afiliación al seguro SIS y el tiempo promedio de hospitalización en días fue 6. Los usuarios hospitalizados muestran un alto porcentaje (76.61%) de satisfacción con el servicio recibido, siendo la dimensión “aspectos tangibles” la que presenta el más alto porcentaje de satisfacción (83.02%). La dimensión en la que existe menor porcentaje de satisfacción (66.09%) corresponde a la “capacidad de respuesta”, especialmente en el aspecto que se refiere a “los trámites para el alta son rápidos” (53.88%).  CONCLUSIONES: Se concluye que existe un alto nivel de satisfacción (76.61) en los usuarios hospitalizados en el servicio de cirugía
