    A Systematic Literature Review of the Current State of Knowledge Related to Interventions for Bereaved Parents

    Aims: The purpose of this systematic literature review is to describe the interventions for bereaved parents, evaluate intervention effectiveness through study methodology rigor, replicability, and theoretical foundations. Methods: We searched MEDLINE via PubMed (1966-2018), CINAHL (1937-present), PsycINFO (1887-present), and Embase (1947-present) using various search words and MeSH terms related to the study purpose. A blinded screening of title/abstract was performed, with conflicting inclusion decisions resolved through group discussions. Matrices for remaining articles were created and discussed among the team. The levels of evidence of the 9 records were rated from very low to high based on the Grading of Recommendations Assessment, Development, and Evaluation guidelines. Results: Our initial pool included 1025 articles. After the screening of titles/abstracts, 63 articles were retained for full-text reviews. Evaluated based on the inclusion/exclusion criteria, 9 records met the review criteria. Of the 9 records, 1 was graded as very low, 3 low, and 5 low to moderate. The interventions for bereaved parents varied from using single-model interventions such as expressive arts therapy and telephone support to multimodal interventions that combined resources (ie, peer support, resource packets, and health-care support). Only 1 study explicitly illustrated how its bereavement intervention was designed based on the proposed theoretical model. Conclusions: This review highlights the need for individualized, well-tested, and effective bereavement care interventions to support bereaved parents. In summary, the state of the science on interventions for bereaved parents is poor and much work needs to be done to effectively address the needs of bereaved parents, including both their physical and emotional health needs

    Nursery Rhymes and Songs for Teaching English as a Second Language to Children

    Fifty-one rhymes and songs are presented here for use in an ESL classroom. Because the rhymes were originally written for English-speaking children, their most appropriate usage in the ESL environment is also with children (under 13 years in age). The rhymes are grouped in categories according to the material each one presents. The categories are: Vocabulary: Parts of the Body People and Occupations Animals Clothes Colors Daily Activities Flowers Food Grammatical Structures: Simple Present Present Continuous Simple Past Past Continuous Present Perfect Conversational Exchanges: Commands and Requests Conversational Exchanges Demonstration General Statements Narratives Personal Statements Also included are several suggestions for class activities based on the rhymes and songs. These are drawn from my teaching experience in England, Summer 1978, at the American School in Switzerland, Surrey, England. This project is a guide for the ESL teacher who would like to work with rhymes in the classroom. This collection is intended to add to the material a teacher needs not only to introduce new material to children but to reinforce, drill, and practice specific structures. The Table of Contents lists the rhymes according to the categories into which they fall. Therefore, a teacher can use the Table of Contents to determine if any of the rhymes will be suitable for his/her lesson plan

    LGM permafrost distribution: how well can the latest PMIP multi-model ensembles perform reconstruction?

    Here, global-scale frozen ground distribution from the Last Glacial Maximum (LGM) has been reconstructed using multi-model ensembles of global climate models, and then compared with evidence-based knowledge and earlier numerical results. Modeled soil temperatures, taken from Paleoclimate Modelling Intercomparison Project phase III (PMIP3) simulations, were used to diagnose the subsurface thermal regime and determine underlying frozen ground types for the present day (pre-industrial; 0 kya) and the LGM (21 kya). This direct method was then compared to an earlier indirect method, which categorizes underlying frozen ground type from surface air temperature, applying to both the PMIP2 (phase II) and PMIP3 products. Both direct and indirect diagnoses for 0 kya showed strong agreement with the present-day observation-based map. The soil temperature ensemble showed a higher diversity around the border between permafrost and seasonally frozen ground among the models, partly due to varying subsurface processes, implementation, and settings. The area of continuous permafrost estimated by the PMIP3 multi-model analysis through the direct (indirect) method was 26.0 (17.7) million km2 for LGM, in contrast to 15.1 (11.2) million km2 for the pre-industrial control, whereas seasonally frozen ground decreased from 34.5 (26.6) million km2 to 18.1 (16.0) million km2. These changes in area resulted mainly from a cooler climate at LGM, but from other factors as well, such as the presence of huge land ice sheets and the consequent expansion of total land area due to sea-level change. LGM permafrost boundaries modeled by the PMIP3 ensemble-improved over those of the PMIP2 due to higher spatial resolutions and improved climatology-also compared better to previous knowledge derived from geomorphological and geocryological evidence. Combinatorial applications of coupled climate models and detailed stand-alone physical-ecological models for the cold-region terrestrial, paleo-, and modern climates will advance our understanding of the functionality and variability of the frozen ground subsystem in the global eco-climate system

    Patient and family education in pediatric oncology: A systematic review

    Session presented on Saturday, July 23, 2016 and Sunday, July 24, 2016: A diagnosis of cancer is an extremely stressful experience for the pediatric patient and family; yet, essential education must be delivered to the patient and family during this emotional time. Nurses have a responsibility to provide education to patients and parents, but best methods for education have not been identified (Landier, Leonard, & Ruccione, 2013). The aim of this evidence-based review is to determine the current state of knowledge and develop recommendation statements regarding effective delivery of patient/family education with newly diagnosed pediatric oncology patients. Six areas of interest were explored for this review, and included educational methods, timing, location, content, influencing factors, and interventions. A literature search was conducted in MEDLINE, CINAHL, and The Cochrane Library databases, using key words and MeSH terms pertinent to the topic. Searches included no publication date limits but were limited to the English language. Due to the limited results within pediatric oncology, the search was expanded to other diseases or conditions that required the parent or patient to learn new information and/or skills. These diseases or conditions included diabetes, sickle cell disease, human immunodeficiency virus, epilepsy, hemophilia, newly placed tracheostomy or central line, chronic diseases requiring hospitalization, traumatic brain injury, traumatic injury, and premature or newborn infants. Removal of duplicates resulted in 2779 unique articles. Using the inclusion criteria of empirical evidence evaluating the outcome of education, knowledge level, communication technique among newly diagnosed pediatric patients, their parents, their siblings, and/or their healthcare providers, a total of 83 articles were selected for inclusion in the review. Preferences for educational delivery methods among patients newly diagnosed with cancer and their family included written materials, verbal discussions, audio recordings, and use of the Internet. No evidence provided a specific time period for the delivery of education; however, parents expect to receive cancer diagnosis information during the initial meeting with the oncologist but often become overwhelmed and need time to process the information (Auburn & Gott, 2014). No evidence was identified regarding the most effective or preferred location to receive education. Educational content considered important among patients with cancer and their family members ranged from cancer-specific information to psychosocial topics. Age and educational level influenced patient/family preferences regarding the amount and type of information provided (Zebrack et al., 2013). Several other factors influenced receipt of education among parents including information delivery, emotions, language barriers, relationship with healthcare providers, the child\u27s condition, and social issues. Only web-based programs and structured teaching tools have been evaluated with pediatric oncology patients and their families for education delivery. From this body of evidence, ten recommendation statements were developed for children newly diagnosed with cancer and their family members. Recommendations include: (a) methods such as written, short verbal discussion, and audio recordings, should be used to provide education; (b) parents need time to process the diagnosis before teaching can begin; (c) medical information should include prognosis, etiology, procedures, treatment and side effects, and for adolescents and young adults, sexuality and fertility information; (d) psychosocial information should include adjustment, interacting and communicating with friends, impact on family members, getting back to school, and making job or career plans; (e) healthcare providers should utilize anticipatory educative content; (f) education and developmental level should be considered when delivering educational information to the pediatric oncology patient; (g) educational information should be provided by consistent healthcare providers, using vocabulary that the recipient understands, providing consistent information, and allowing time to answer questions; (h) parents\u27 emotional state, language barriers, cultural issues, and social issues should be considered when providing education to parents; (i) structured teaching tools should be used to provide education and discharge instructions; and (j) siblings of children newly diagnosed with cancer should receive age-appropriate, interactive education. Incorporating these recommendations into practice may enhance the quality of education delivered by healthcare providers and received by patients and families. Optimal educational practices can improve the quality of life among patients and parents (Slone, Self, Friedman, & Heiman, 2014)

    Iron in Micronutrient Powder Promotes an Unfavorable Gut Microbiota in Kenyan Infants

    Iron supplementation may have adverse health effects in infants, probably through manipulation of the gut microbiome. Previous research in low-resource settings have focused primarily on anemic infants. This was a double blind, randomized, controlled trial of home fortification comparing multiple micronutrient powder (MNP) with and without iron. Six-month-old, non- or mildly anemic, predominantly-breastfed Kenyan infants in a rural malaria-endemic area were randomized to consume: (1) MNP containing 12.5 mg iron (MNP+Fe, n = 13); (2) MNP containing no iron (MNP−Fe, n = 13); or (3) Placebo (CONTROL, n = 7), from 6–9 months of age. Fecal microbiota were profiled by high-throughput bacterial 16S rRNA gene sequencing. Markers of inflammation in serum and stool samples were also measured. At baseline, the most abundant phylum was Proteobacteria (37.6% of rRNA sequences). The proteobacterial genus Escherichia was the most abundant genus across all phyla (30.1% of sequences). At the end of the intervention, the relative abundance of Escherichia significantly decreased in MNP−Fe (−16.05 ± 6.9%, p = 0.05) and CONTROL (−19.75 ± 4.5%, p = 0.01), but not in the MNP+Fe group (−6.23 ± 9%, p = 0.41). The second most abundant genus at baseline was Bifidobacterium (17.3%), the relative abundance of which significantly decreased in MNP+Fe (−6.38 ± 2.5%, p = 0.02) and CONTROL (−8.05 ± 1.46%, p = 0.01), but not in MNP-Fe (−4.27 ± 5%, p = 0.4445). Clostridium increased in MNP-Fe only (1.9 ± 0.5%, p = 0.02). No significant differences were observed in inflammation markers, except for IL-8, which decreased in CONTROL. MNP fortification over three months in non- or mildly anemic Kenyan infants can potentially alter the gut microbiome. Consistent with previous research, addition of iron to the MNP may adversely affect the colonization of potential beneficial microbes and attenuate the decrease of potential pathogens
