    Cooperation Among Technical Specialists in a Distributed Computing Environment

So, as organizations move to distribute their computing, they must also move technical specialists closer to these users. This leads to a situation where the distributed technical specialists, while close to their user, are isolated from each other. This isolation from others’ expertise may lead some specialists to “reinvent the wheel” unless they establish cooperative relationships with other distributed specialists

    The Moonraker Study: An Experimental Evaluation of Host-Based Deception

    Cyber deception has been discussed as providing enhanced cyber defense. This human subjects research, one of the first rigorously controlled studies on this topic, found that host-based deception was effective at preventing completion of a specific exfiltration task against a virtual network. In addition to impeding progress and preventing success, the deception resulted in increased confusion and surprise in the participants. This study provided the necessary rigor to scientifically attest to the effectiveness of cyber deception for cyber defense with computer specialists

    The Role of Preoperative Bilateral Breast Magnetic Resonance Imaging in Patient Selection for Partial Breast Irradiation in Ductal Carcinoma In Situ

    Purpose. Women with ductal carcinoma in situ (DCIS) are often candidates for breast-conserving therapy, and one option for radiation treatment is partial breast irradiation (PBI). This study evaluates the use of preoperative breast magnetic resonance imaging (MRI) for PBI selection in DCIS patients. Methods. Between 2002 and 2009, 136 women with newly diagnosed DCIS underwent a preoperative bilateral breast MRI at Mayo Clinic in Florida. One hundred seventeen women were deemed eligible for PBI by the NSABP B-39 (National Surgical Adjuvant Breast and Bowel Project, Protocol B-39) inclusion criteria using physical examination, mammogram, and/or ultrasound. MRIs were reviewed for their impact on patient eligibility, and findings were pathologically confirmed. Results. Of the 117 patients, 23 (20%) were found ineligible because of pathologically proven MRI findings. MRI detected additional ipsilateral breast cancer in 21 (18%) patients. Of these women, 15 (13%) had more extensive disease than originally noted before MRI, and 6 (5%) had multicentric disease in the ipsilateral breast. In addition, contralateral breast cancer was detected in 4 (4%). Conclusions. Preoperative breast MRI altered the PBI recommendations for 20% of women. Bilateral breast MRI should be an integral part of the preoperative evaluation of all patients with DCIS being considered for PBI

    Maternal and infant renal safety following tenofovir disoproxil fumarate exposure during pregnancy in a randomized control trial

    Background Tenofovir disoproxil fumarate (TDF) in combination with other antiretroviral (ARV) drugs has been in clinical use for HIV treatment since its approval in 2001. Although the effectiveness of TDF in preventing perinatal HIV infection is well established, information about renal safety during pregnancy is still limited. Trial design The IMPAACT PROMISE study was an open-label, strategy trial that randomized pregnant women to one of three arms: TDF based antiretroviral therapy (ART), zidovudine (ZDV) based ART, and ZDV alone (standard of care at start of enrollment). The P1084s substudy was a nested, comparative study of renal outcomes in women and their infants. Methods PROMISE participants (n = 3543) were assessed for renal dysfunction using calculated creatinine clearance (CrCl) at study entry (> 14 weeks gestation), delivery, and postpartum weeks 6, 26, and 74. Of these women, 479 were enrolled in the P1084s substudy that also assessed maternal calcium and phosphate as well as infant calculated CrCl, calcium, and phosphate at birth. Results Among the 1338 women who could be randomized to TDF, less than 1% had a baseline calculated CrCl below 80 mL/min. The mean (standard deviation) maternal calculated CrCl at delivery in the TDF-ART arm [147.0 mL/min (51.4)] was lower than the ZDV-ART [155.0 mL/min (43.3); primary comparison] and the ZDV Alone [158.5 mL/min (45.0)] arms; the mean differences (95% confidence interval) were − 8.0 mL/min (− 14.5, − 1.5) and − 11.5 mL/min (− 18.0, − 4.9), respectively. The TDF-ART arm had lower mean maternal phosphate at delivery compared with the ZDV-ART [− 0.14 mg/dL (− 0.28, − 0.01)] and the ZDV Alone [− 0.17 mg/dL (− 0.31, − 0.02)] arms, and a greater percentage of maternal hypophosphatemia at delivery (4.23%) compared with the ZDV-ART (1.38%) and the ZDV Alone (1.46%) arms. Maternal calcium was similar between arms. In infants, mean calculated CrCl, calcium, and phosphate at birth were similar between arms (all CIs included 0). Conclusions Although mean maternal calculated CrCl at Delivery was lower in the TDF-ART arm, the difference between arms is unlikely to be clinically significant. During pregnancy, the TDF-ART regimen had no observed safety concerns for maternal or infant renal function. Trial Registration: NCT01061151 on 10/02/2010 for PROMISE (1077BF).     A Candidate Dual AGN at z=1.175

    The X-ray source CXOXBJ142607.6+353351 (CXOJ1426+35), which was identified in a 172 ks Chandra image in the Bootes field, shows double-peaked rest-frame optical/UV emission lines, separated by 0.69" (5.5 kpc) in the spatial dimension and by 690 km s^-1 in the velocity dimension. The high excitation lines and emission line ratios indicate both systems are ionized by an AGN continuum, and the double-peaked profile resembles that of candidate dual AGN. At a redshift of z=1.175, this source is the highest redshift candidate dual AGN yet identified. However, many sources have similar emission line profiles for which other interpretations are favored. We have analyzed the substantial archival data available in this field, as well as acquired near-infrared (NIR) adaptive optics (AO) imaging and NIR slit spectroscopy. The X-ray spectrum is hard, implying a column density of several 10^23 cm^-2. Though heavily obscured, the source is also one of the brightest in the field, with an absorption-corrected 2-10 keV luminosity of ~10^45 erg s^-1. Outflows driven by an accretion disk may produce the double-peaked lines if the central engine accretes near the Eddington limit. However, we may be seeing the narrow line regions of two AGN following a galactic merger. While the AO image reveals only a single source, a second AGN would easily be obscured by the significant extinction inferred from the X-ray data. Understanding the physical processes producing the complex emission line profiles seen in CXOJ1426+35 and related sources is important for interpreting the growing population of dual AGN candidates.Comment: 18 pages and 9 figures.
