    Firefighter’s Workers’ Compensation Claims in Montana, 2007-2017: A Cost Analysis

    The objective of this study was to examine costs associated with workers’ compensation claims from firefighters in Montana. Workers’ compensation claims data were obtained from the Montana Department of Labor and Industry. The highest proportion of total benefit amount dollars was paid to male firefighters (93.7%), public employees (84.8%), and those who were 45-54 years of age at time of injury (23.9%). Part-time employees represented the employment category receiving the highest total benefit amount (766,354).Julyrepresentedthemonthwiththehighestaveragetotalbenefitamountcost(766,354). July represented the month with the highest average total benefit amount cost (52,084). Strains and sprains, back injuries, and lifting activities all represented the highest total benefit amount cost in terms of nature of injury, body part injured, and cause of injury, respectively. No statistically significant difference was found between the median total benefit costs in terms of gender, employment type, employment sector, or firefighting activity. Understanding the costs associated with workers’ compensation claims can provide insight into injury severity and these results may aid in the development of more informed decisions for resource allocation and effective prevention strategies

    Translocation of Humpback Chub into Tributary Streams of the Colorado River: Implications for Conservation of Large- River Fishes

    The Humpback Chub Gila cypha, a large-bodied, endangered cyprinid endemic to the Colorado River basin, is in decline throughout most of its range due largely to anthropogenic factors. Translocation of Humpback Chub into tributaries of the Colorado River is one conservation activity that may contribute to the expansion of the species’ current range and eventually provide population redundancy. We evaluated growth, survival, and dispersal following translocation of approximately 900 Humpback Chub over a period of 3 years (2009, 2010, and 2011) into Shinumo Creek, a tributary stream of the Colorado River within Grand Canyon National Park. Growth and condition of Humpback Chub in Shinumo Creek were consistent among year-classes and equaled or surpassed growth estimates from both the main-stem Colorado River and the Little Colorado River, where the largest (and most stable) Humpback Chub aggregation remains. Based on passive integrated tag recoveries, 53% ( D 483/902) of translocated Humpback Chub dispersed from Shinumo Creek into the main-stem Colorado River as of January 2013, 35% leaving within 25 d following translocation. Annual apparent survival estimates within Shinumo Creek ranged from 0.22 to 0.41, but were strongly influenced by emigration. Results indicate that Shinumo Creek provides favorable conditions for growth and survival of translocated Humpback Chub and could support a new population if reproduction and recruitment occur in the future. Adaptation of translocation strategies of Humpback Chub into tributary streams ultimately may refine the role translocation plays in recovery of the species

    Remarkable Response of Native Fishes to Invasive Trout Suppression Varies With Trout Density, Temperature, and Annual Hydrology

    Recovery of imperiled fishes can be achieved through suppression of invasives, but outcomes may vary with environmental conditions. We studied the response of imperiled desert fishes to an invasive brown (Salmo trutta) and rainbow trout (Oncorhynchus mykiss) suppression program in a Colorado River tributary, with natural flow and longitudinal variation in thermal characteristics. We investigated trends in fish populations related to suppression and tested hypotheses about the impacts of salmonid densities, hydrologic variation, and spatial–thermal gradients on the distribution and abundance of native fish species using zero-inflated generalized linear mixed effects models. Between 2012 and 2018, salmonids declined 89%, and native fishes increased dramatically (∌480%) once trout suppression surpassed ∌60%. Temperature and trout density were consistently retained in the top models predicting the abundance and distribution of native fishes. The greatest increases occurred in warmer reaches and in years with spring flooding. Surprisingly, given the evolution of native fishes in disturbance-prone systems, intense, monsoon-driven flooding limited native fish recruitment. Applied concertedly, invasive species suppression and efforts to mimic natural flow and thermal regimes may allow rapid and widespread native fish recovery

    A method for estimating pore water drainage from marsh soils using rainfall and well records

If one has long time series of both rain events and water levels in wells along transects from creek bank to marsh interior, one can correlate well response with rain amount. In cases examined so far the well response is found to be a linear function of rain amount. As it is reasonable to assume that the amount of tidal infiltration required to restore the water table to the elevation of the marsh surface is equal to the amount of rain that would be required to do so, one can estimate the annual drainage of pore water from a well site by dividing the mean drawdown of the water table at low tide by the slope of the response-versus-rain regression and then multiplying the result by the number of tidal drawdowns in a year. Integration of such results along the transect then gives an estimate of the total annual drainage. An example of the use of this method is given for two well transects in a Typha and a Spartina marsh at the Plum Island Estuary Long Term Ecological Research (PIE-LTER) site in Massachusetts, USA. Both transects yielded pore water drainage rates of about 160 m3 yr-1 per meter of channel length. Although the annual volume of pore water drainage is small compared to the annual volume of the tidal prism its impact on nutrient budgets in the estuary could be large because of the high concentrations of nutrients in marsh pore waters.     Positive psychology interventions in patients with medical illness: What predicts improvement in psychological state?

    Positive psychology interventions (PPIs) have been shown to promote wellbeing in individuals with medical illness, although it is still unknown whether certain patient characteristics make participants more likely to benefit from such interventions. The present study tested whether, using individual patient data across five published PPI studies (three single-arm proof-of-concept trials, one non-randomized controlled trial, and one randomized controlled trial) in medically-ill persons, sociodemographic or psychological factors predicted subsequent change in wellbeing. In 208 participants, lower baseline psychological wellbeing and optimism, and higher symptoms of depression and anxiety were associated with greater improvement in psychological symptoms during the PPI. Other factors were unrelated to symptom changes. In a sub-analysis of controlled studies, there were no group differences in the relationship between baseline factors and changes in wellbeing from pre- to post-intervention. Findings suggest that patients with more severe psychiatric and/or medical comorbidity are no less likely to benefit from a PPI compared to those with higher levels of health, even though these programs do not directly target psychological distress. PPIs may be widely applicable to medical patients, with lower psychological wellbeing a potential predictor of increased benefit

    Patient views on asthma diagnosis and how a clinical decision support system could help:a qualitative study

    Abstract Introduction Making a diagnosis of asthma can be challenging for clinicians and patients. A clinical decision support system (CDSS) for use in primary care including a patient‐facing mode, could change how information is shared between patients and healthcare professionals and improve the diagnostic process. Methods Participants diagnosed with asthma within the last 5 years were recruited from general practices across four UK regions. In‐depth interviews were used to explore patient experiences relating to their asthma diagnosis and to understand how a CDSS could be used to improve the diagnostic process for patients. Interviews were audio recorded, transcribed verbatim and analysed using a thematic approach. Results Seventeen participants (12 female) undertook interviews, including 14 individuals and 3 parents of children with asthma. Being diagnosed with asthma was generally considered an uncertain process. Participants felt a lack of consultation time and poor communication affected their understanding of asthma and what to expect. Had the nature of asthma and the steps required to make a diagnosis been explained more clearly, patients felt their understanding and engagement in asthma self‐management could have been improved. Participants considered that a CDSS could provide resources to support the diagnostic process, prompt dialogue, aid understanding and support shared decision‐making. Conclusion Undergoing an asthma diagnosis was uncertain for patients if their ideas and concerns were not addressed by clinicians and were influenced by a lack of consultation time and limitations in communication. An asthma diagnosis CDSS could provide structure and an interface to prompt dialogue, provide visuals about asthma to aid understanding and encourage patient involvement. Patient and Public Contribution Prespecified semistructured interview topic guides (young person and adult versions) were developed by the research team and piloted with members of the Asthma UK Centre for Applied Research Patient and Public Involvement (PPI) group. Findings were regularly discussed within the research group and with PPI colleagues to aid the interpretation of data

    Evaluation of an Online Platform for Multiple Sclerosis Research: Patient Description, Validation of Severity Scale, and Exploration of BMI Effects on Disease Course

    Objectives: To assess the potential of an online platform, PatientsLikeMe.com (PLM), for research in multiple sclerosis (MS). An investigation of the role of body mass index (BMI) on MS disease course was conducted to illustrate the utility of the platform. Methods: First, we compared the demographic characteristics of subjects from PLM and from a regional MS center. Second, we validated PLM’s patient-reported outcome measure (MS Rating Scale, MSRS) against standard physician-rated tools. Finally, we analyzed the relation of BMI to the MSRS measure. Results: Compared with 4,039 MS Center patients, the 10,255 PLM members were younger, more educated, and less often male and white. Disease course was more often relapsing remitting, with younger symptom onset and shorter disease duration. Differences were significant because of large sample sizes but small in absolute terms. MSRS scores for 121 MS Center patients revealed acceptable agreement between patient-derived and physician-derived composite scores (weighted kappa = 0.46). The Walking domain showed the highest weighted kappa (0.73) and correlation (rs = 0.86) between patient and physician scores. Additionally, there were good correlations between the patient-reported MSRS composite and walking scores and physician-derived measures: Expanded Disability Status Scale (composite rs = 0.61, walking rs = 0.74), Timed 25 Foot Walk (composite rs = 0.70, walking rs = 0.69), and Ambulation Index (composite rs = 0.81, walking rs = 0.84). Finally, using PLM data, we found a modest correlation between BMI and cross-sectional MSRS (rho = 0.17) and no association between BMI and disease course. Conclusions: The PLM population is comparable to a clinic population, and its patient-reported MSRS is correlated with existing clinical instruments. Thus, this online platform may provide a venue for MS investigations with unique strengths (frequent data collection, large sample sizes). To illustrate its applicability, we assessed the role of BMI in MS disease course but did not find a clinically meaningful role for BMI in this setting
