    Lower Ca2+ enhances the K+-induced force depression in normal and HyperKPP mouse muscles

    Hyperkalemic periodic paralysis (HyperKPP) manifests as stiffness or subclinical myotonic discharges before or during periods of episodic muscle weakness or paralysis. Ingestion of Ca2+ alleviates HyperKPP symptoms, but the mechanism is unknown because lowering extracellular [Ca2+] ([Ca2+]e) has no effect on force development in normal muscles under normal conditions. Lowering [Ca2+]e, however, is known to increase the inactivation of voltage-gated cation channels, especially when the membrane is depolarized. Two hypotheses were tested: (1) lowering [Ca2+]e depresses force in normal muscles under conditions that depolarize the cell membrane; and (2) HyperKPP muscles have a greater sensitivity to low Ca2+-induced force depression because many fibers are depolarized, even at a normal [K+]e. In wild type muscles, lowering [Ca2+]e from 2.4 to 0.3 mM had little effect on tetanic force and membrane excitability at a normal K+ concentration of 4.7 mM, whereas it significantly enhanced K+-induced depression of force and membrane excitability. In HyperKPP muscles, lowering [Ca2+]e enhanced the K+-induced loss of force and membrane excitability not only at elevated [K+]e but also at 4.7 mM K+. Lowering [Ca2+]e increased the incidence of generating fast and transient contractures and gave rise to a slower increase in unstimulated force, especially in HyperKPP muscles. Lowering [Ca2+]e reduced the efficacy of salbutamol, a beta2 adrenergic receptor agonist and a treatment for HyperKPP, to increase force at elevated [K+]e. Replacing Ca2+ by an equivalent concentration of Mg2+ neither fully nor consistently reverses the effects of lowering [Ca2+]e. These results suggest that the greater Ca2+ sensitivity of HyperKPP muscles primarily relates to (1) a greater effect of Ca2+ in depolarized fibers and (2) an increased proportion of depolarized HyperKPP muscle fibers compared with control muscle fibers, even at normal [K+]e

    Post-Release Dispersal in Animal Translocations: Social Attraction and the “Vacuum Effect”

    Animal translocations are human-induced colonizations that can represent opportunities to contribute to the knowledge on the behavioral and demographic processes involved in the establishment of animal populations. Habitat selection behaviors, such as social cueing, have strong implications on dispersal and affect the establishment success of translocations. Using modeling simulations with a two-population network model (a translocated population and a remnant population), we investigated the consequences of four habitat selection strategies on post-translocation establishment probabilities in short- and long-lived species. Two dispersal strategies using social cues (conspecific attraction and habitat copying) were compared to random and quality-based strategies. We measured the sensitivity of local extinctions to dispersal strategies, life cycles, release frequencies, remnant population and release group sizes, the proportion of breeders and the connectivity between populations. Our results indicate that social behaviors can compromise establishment as a result of post-release dispersal, particularly in long-lived species. This behavioral mechanism, the “vacuum effect”, arises from increased emigration in populations that are small relative to neighboring populations, reducing their rate of population growth. The vacuum effect can drive small remnant populations to extinction when a translocated group is large. In addition, the magnitude of the vacuum effect varies non-linearly with connectivity. The vacuum effect represents a novel form of the behaviorally mediated Allee effect that can cause unexpected establishment failures or population extinctions in response to social cueing. Accounting for establishment probabilities as a conditional step to the persistence of populations would improve the accuracy of predicting the fates of translocated or natural (meta)populations

    Community Assessment and Intervention Alignment to Reduce Incidence of Female-to-Female Non-Intimate Partner Violence

    Significance: According to the North Carolina Central University (NCCU) Office of Student Rights and Responsibilities (OSRR), 14 undergraduate female students were sanctioned for participating in non-intimate partner violence incidents during the fall semester of 2011, some involving deadly weapons. NCCU administrators and staff identified a marked increase in incidents of undergraduate female-to-female non-intimate partner violence (FTFNIPV) within the last five years. FTFNIPV among college women is an area of public health with increasing importance but remains an area about which little is known. Consequences of FTFNIPV include injury and disrupted academic settings, and may include psychological and behavioral problems that negatively impact school performance, increased risk of depression, and may lead to future violent behaviors. Ultimately these long-term consequences have implications for health status and quality of life. Prior to the 2012-2013 academic year, the Vice-Chancellor for Student Affairs tasked the Director of the Women's Center with creating an initiative to address the increased levels of violence, at which time she sought out and formed a partnership with UNC's School of Public Health Capstone program to do just that. Project Deliverables: Using a women-centered approach, this project intended to help NCCU add to a campus climate that encourages respect for self, others and community. Starting with a literature review, the NCCU Capstone team researched violence among female students occurring at historically black colleges and universities (HBCU). Though little data pertaining to FTFNIPV exists for this target population, we were able to corroborate increasing incidents of FTFNIPV at NCCU to national trends of violence. We completed a community social norms assessment to demonstrate the significance of the problem of FTFNIPV on NCCU's campus and to identify opportunities for intervention. Using anecdotal information from NCCU stakeholders as our guide, we developed our primary research questions, a focus group guide, and an individual interview guide. We then conducted six focus groups and five individual interviews with both female and male students on campus. Next, using the qualitative data, we created a community social norms and attitudinal questionnaire using both previously validated and originally developed items. The survey assessed attitudes towards and perceptions of using violence as a means to manage conflict. After compiling and analyzing the data, we wrote and distributed an Executive Summary to campus administrators, which outlined our findings and recommendations for intervention. Additionally, we used the formative research findings to develop a program plan outlining a campus-wide intervention to address the problem of FTFNIPV on NCCU's campus. Broader Implications: This project significantly impacted Capstone team members as well as the partner organization. The Capstone team was able to provide intervention recommendations based on exploratory data rather than anecdotal evidence. This project could initiate a cultural shift on campus that discourages interpersonal violence and encourages healthy conflict management. This will add to the already rich and engaging academic experience that NCCU provides for its students. The formative research completed by the Capstone team contributes to current literature related to aggression among racial/ethnic minority women, of which there is little. Because our target population shared unique social experiences that shaped their attitudes and perceptions of violence, we anticipate our formative research will be useful to other HBCU's or similar communities that experience comparable problems with violence among women.Master of Public Healt

Point-of-care tests (POCTs) for the management of community-acquired acute respiratory tract infections (CA-ARTI) have been developed to help optimize antibiotic prescribing. While some countries in Europe have adopted these tests in primary care settings, most have not. Stakeholders, such as policy-makers, regulators, the diagnostic industry, and scientific associations, have roles in the implementation of new diagnostics in primary care. The aim of this study is to explore these stakeholders’ views and experiences, and identify areas of unmet need relating to POCT implementation.Methods: Stakeholders were recruited using purposive sampling and snowballing. Between March 2021 and May 2022, semi-structured interviews were conducted online with stakeholders in Belgium, the UK and from European Union (EU) -level organizations. Interviews were audio recorded and transcribed verbatim. Transcripts were analysed inductively and deductively using thematic analysis.Results: Twenty-six stakeholders participated: eleven from EU-level organizations, seven from Belgium, and eight from the UK. Five themes were identified. Stakeholders felt a balance of top-down and bottom-up approaches were an optimal strategy to the implementation of POCTs. Stakeholders stressed the need to engage with clinicians to act as champions for tests to help raise awareness and generate new evidence on how tests are used. While acknowledging the potential of POCTs for improving patient outcomes and impacting antibiotic prescribing behavior, some raised concerns on how tests would be used in practice and wished to see national data on effectiveness. COVID-19 catalyzed the use of tests, but stakeholders were pessimistic that processes for approving diagnostics during the pandemic would be replicated in the future.Conclusion: Stakeholders provided recommendations for research and practice. Robust reimbursement policies could alleviate financial burden from clinicians and patients, encouraging practices to adopt POCTs. Industry is likely to benefit from engaging as early on as possible with other stakeholders.     One-year outcomes after transcatheter insertion of an interatrial shunt device for the management of heart failure with preserved ejection fraction

    Background—Heart failure with preserved ejection fraction has a complex pathophysiology and remains a therapeutic challenge. Elevated left atrial pressure, particularly during exercise, is a key contributor to morbidity and mortality. Preliminary analyses have demonstrated that a novel interatrial septal shunt device that allows shunting to reduce the left atrial pressure provides clinical and hemodynamic benefit at 6 months. Given the chronicity of heart failure with preserved ejection fraction, evidence of longer-term benefit is required. Methods and Results—Patients (n=64) with left ventricular ejection fraction ≄40%, New York Heart Association class II–IV, elevated pulmonary capillary wedge pressure (≄15 mm Hg at rest or ≄25 mm Hg during supine bicycle exercise) participated in the open-label study of the interatrial septal shunt device. One year after interatrial septal shunt device implantation, there were sustained improvements in New York Heart Association class (P&lt;0.001), quality of life (Minnesota Living with Heart Failure score, P&lt;0.001), and 6-minute walk distance (P&lt;0.01). Echocardiography showed a small, stable reduction in left ventricular end-diastolic volume index (P&lt;0.001), with a concomitant small stable increase in the right ventricular end-diastolic volume index (P&lt;0.001). Invasive hemodynamic studies performed in a subset of patients demonstrated a sustained reduction in the workload corrected exercise pulmonary capillary wedge pressure (P&lt;0.01). Survival at 1 year was 95%, and there was no evidence of device-related complications. Conclusions—These results provide evidence of safety and sustained clinical benefit in heart failure with preserved ejection fraction patients 1 year after interatrial septal shunt device implantation. Randomized, blinded studies are underway to confirm these observations
