Universal cures for idiosyncratic illnesses: a genealogy of therapeutic reasoning in the mental health field
Over the past decades, there has been a significant increase in prescriptions of psychotropic drugs for mental disorders. So far, most of the explanations of the phenomenon have focused on the process of medicalization, but little attention has been cast towards physicians' day-to-day clinical reasoning, and the way it affects therapeutic decision-making. This article addresses the complex relationship between aetiology, diagnosis and drug treatment by examining the style of reasoning underlying prescribing practices through an historical lens. A genealogy of contemporary prescribing practices is proposed, that draws significant comparisons between 19th-century medicine and modern psychiatry. Tensions between specific, standardized cures and specific, idiosyncratic patients have been historically at play in clinical reasoning - and still are today. This inquiry into the epistemological foundations of contemporary drug prescription reveals an underlying search for scientific legitimacy
Beyond health:medicines, food supplements, energetics, and the commodification of self-performance in Maputo
With an increasing range of products in global and local markets, more options are available for individuals to enhance their image and their (cognitive, social and physical) performance. These âperformance consumptionsâ relate to ideals of wellâbeing and improvement, and are based on constructed desires, expectations and needs that go beyond the (often blurred) dichotomy of health and illness. Drawing from mixedâmethods research in Maputo, Mozambique, this paper discusses individualsâ use of medicines and other substances â pharmaceuticals, food supplements, traditional herbs, cosmetics and energy drinks â for managing different aspects of their everyday lives. Through an overview of the main consumption practices, we explore the underlying purposes and strategies of users, and the perceived legitimacy and risks involved when using a variety of products accessible through formal and informal exchange channels. From tiredness to sexual and aesthetic management, we show how the body becomes the locus of experimentation and investment to perform in accordance with socially expected roles, individual aspirations and everyday tasks. With insights from individualsâ accounts in Maputo, we aim to add to discussions on pharmaceuticalisation of body management by showing how the emergence of new performance consumptions is articulated with the reconfiguration of more âtraditionalâ consumption practices.info:eu-repo/semantics/acceptedVersio
Secondary structure of rhBMP-2 in a protective biopolymeric carrier material
Efficient delivery of growth factors is one of the great challenges of tissue engineering. Polyelectrolyte multilayer films (PEM) made of biopolymers have recently emerged as an interesting carrier for delivering recombinant human bone morphogenetic protein 2 (rhBMP-2 noted here BMP-2) to cells in a matrix-bound manner. We recently showed that PEM made of poly(l-lysine) and hyaluronan (PLL/HA) can retain high and tunable quantities of BMP-2 and can deliver it to cells to induce their differentiation in osteoblasts. Here, we investigate quantitatively by Fourier transform infrared spectroscopy (FTIR) the secondary structure of BMP-2 in solution as well as trapped in a biopolymeric thin film. We reveal that the major structural elements of BMP-2 in solution are intramolecular ÎČ-sheets and unordered structures as well as α-helices. Furthermore, we studied the secondary structure of rhBMP-2 trapped in hydrated films and in dry films since drying is an important step for future applications of these bioactive films onto orthopedic biomaterials. We demonstrate that the structural elements were preserved when BMP-2 was trapped in the biopolymeric film in hydrated conditions and, to a lesser extent, in dry state. Importantly, its bioactivity was maintained after drying of the film. The analysis of components of the remedies given to Gouro babies in Ivory Coast to keep them chubby (to put on weight/child/drug) and of the diseases that these remedies are supposed to protect them from, show that, although drugs or diseases vary according to families, this variability allows -for each entity (nosological category or plant)- a space of acceptability in which variations occur by contiguity according to the polythetic mode. The analysis of the symptomatology of the diseases on one hand and the range of activity on the other hand, shows congruent structures if the analysis is considered from a relevant point of view, that of body deterioration and patients. The aim is to avoid impairment of the vital fluids after a loss of fluids due to diarrhea or dryness due to fever. The ethnopharmacological survey must use the symptomatology and the semiology of the studied medicine. (Résumé d'auteur
