    Risk factors for septic arthritis of hip in neonates and infants

    Background: Suppurative arthritis of hip is pyogenic inflammation of synovial membrane of the hip, usually due to bacterial infection. The main aim of early diagnosis and treatment of septic arthritis of hip is to prevent damage to articular cartilage and growth plate, give stable and painless hip and to resume normal development and prevent sequelae.Methods: Prospective interventional study consisted of 34 patients with 36 Hips septic arthritis of hip less than one year of age carried out at Department of Orthopedics of a tertiary care centre during August 2011 to December 2013. Risk factors and related laboratory investigations were carried out. Data entry and analysis was done by MS Excel 3.0.Results: In our study more than 70% patients presented after one week. Left sided hip involvement (59%) was more common followed by right hip (35%). Sex ratio of the study population was   M: F-4:6.  NICU admission was present in 94% cases.  In many of our patients there were more than one risk factors like septicemia, low birth weight, jaundice and ventilator support. Conclusions: All NICU neonates should be handled with strict aseptic precaution

    Numerical Diagonalisation Study of the Trimer Deposition-Evaporation Model in One Dimension

    We study the model of deposition-evaporation of trimers on a line recently introduced by Barma, Grynberg and Stinchcombe. The stochastic matrix of the model can be written in the form of the Hamiltonian of a quantum spin-1/2 chain with three-spin couplings given by H= \sum\displaylimits_i [(1 - \sigma_i^-\sigma_{i+1}^-\sigma_{i+2}^-) \sigma_i^+\sigma_{i+1}^+\sigma_{i+2}^+ + h.c]. We study by exact numerical diagonalization of HH the variation of the gap in the eigenvalue spectrum with the system size for rings of size up to 30. For the sector corresponding to the initial condition in which all sites are empty, we find that the gap vanishes as L−zL^{-z} where the gap exponent zz is approximately 2.55±0.152.55\pm 0.15. This model is equivalent to an interfacial roughening model where the dynamical variables at each site are matrices. From our estimate for the gap exponent we conclude that the model belongs to a new universality class, distinct from that studied by Kardar, Parisi and Zhang.Comment: 11 pages, 2 figures (included

    The Irreducible String and an Infinity of Additional Constants of Motion in a Deposition-Evaporation Model on a Line

    We study a model of stochastic deposition-evaporation with recombination, of three species of dimers on a line. This model is a generalization of the model recently introduced by Barma {\it et. al.} (1993 {\it Phys. Rev. Lett.} {\bf 70} 1033) to q≥3q\ge 3 states per site. It has an infinite number of constants of motion, in addition to the infinity of conservation laws of the original model which are encoded as the conservation of the irreducible string. We determine the number of dynamically disconnected sectors and their sizes in this model exactly. Using the additional symmetry we construct a class of exact eigenvectors of the stochastic matrix. The autocorrelation function decays with different powers of tt in different sectors. We find that the spatial correlation function has an algebraic decay with exponent 3/2, in the sector corresponding to the initial state in which all sites are in the same state. The dynamical exponent is nontrivial in this sector, and we estimate it numerically by exact diagonalization of the stochastic matrix for small sizes.     In silico screening of phytoconstituents of Cissus quadrangularis and Chromolaena odorata against proteins of antimicrobial resistance and wound healing

    In silico screening is a methodological approach, which is invaluable for rational drug design and the identification of potential therapeutic agents. In the context of antibiotic-resistant infectious wounds, molecular docking can provide a deeper understanding of how phytocompounds might interfere with bacterial virulence and antibiotic resistance. In this study, proteins involved in antimicrobial resistance and wound healing were docked against major phytoconstituents of ethyl acetate extract of Cissus quadrangularis (EACQ) and ethanol extract of Chromolaena odorata (EECO), two medicinal plants that have been traditionally used. Receptor structures for interleukin 6 (PDB id: 1n26) IL6, of human and mice, IL6 (Uniprot id p 20607) of rat, vascular endothelial growth factor (VEGFR, PDB id: 2ctw) for human, mice, rat and penicillin binding protein 2a (PBP2a, PDB id: 1vqq) of S. aureus were downloaded from the database of the RCSB protein data bank. The ligand structures were downloaded from PubChem compound database in structure data file (.SDF) format. The docking studies were conducted using Autodock4. and the results of the docking analysis were visualised using Discovery Studio Visualizer. The docking log (dlg) file, featuring an RMSD table, provides binding energy values in Kcal/mol for each molecule at its optimal docked postures, offering insights into structural accuracy and ligand-receptor interaction strength in molecular docking simulations. In silico analysis of ligands showed that squalene of EACQ and epilupeol of EECO had the least binding energy towards proteins of antimicrobial resistance and wound healing. Thus, these compounds could emerge as promising lead molecules against infectious wounds

    Effects of oxalate on the re-initiation of DNA synthesis in LLC-PK1 cells do not involve p42/44 MAP kinase activation.BackgroundOxalate interaction with renal epithelial cells results in a program of events that include alterations in gene expression, re-initiation of DNA synthesis, cell growth and apoptosis. Our studies focused on understanding the mechanisms involved in the oxalate-induced re-initiation of the DNA synthesis. The effects of oxalate alone or in combination with epidermal growth factor (EGF), platelet-derived growth factor (PDGF) and insulin were investigated to determine whether oxalate utilized the p42/44 mitogen activated protein (MAP) kinase pathway, which is a common pathway used by a majority of the mitogens.MethodsLLC-PK1 cells (a renal epithelial cell line of porcine origin) were exposed to oxalate in the presence or absence of three established growth factors, EGF, insulin and PDGF, and of the transcription/translation inhibitors, actinomycin-D and cycloheximide. DNA synthesis was assessed by [3H]-thymidine incorporation. p42/44 MAP kinase activity was assessed by super-shift analysis as well as by immunocomplex kinase assay.ResultsExposure of growth-arrested LLC-PK1 cells to oxalate resulted in the re-initiation of the DNA synthesis that had been abolished earlier by pretreatment with transcription/translation inhibitors. Oxalate (1mmol/L), EGF (50 ng/mL) and insulin (100 ng/mL) stimulated DNA synthesis in growth-arrested LLC-PK1 cells, while PDGF (50 ng/mL) had no effect. Effects of EGF and oxalate on DNA synthesis were additive. In contrast, oxalate and insulin had antagonistic effects on DNA synthesis. Additionally, oxalate did not activate the p42/44 MAP kinase pathway while EGF stimulated this pathway.ConclusionsThese findings demonstrate that oxalate does not activate the p42/44 MAP kinase pathway, and the effects of oxalate are mediated by pathways that are distinct from those of EGF, PDGF and insulin

    Functional outcome of arthroscopic labral repair in adults for traumatic shoulder instability: a prospective study

    Background: Anterior shoulder instability is often treated either by open procedure or arthroscopic method. present study to assess the functional outcome of the patients with recurrent dislocation of shoulder with Bankart lesion, treated with arthroscopic stabilization to evaluate shoulder function with labral tear and after repair of labral tear, to study epidemiology of labral tear and to study epidemiology of associated lesions. Methods: Prospective study in the 25 consented patients admitted in the orthopaedics department in new civil hospital Surat with different shoulder injury after IEC approval. Statistical analyses done in MS excel. Results: Mean age at time of first dislocation was 30 year and most (50%) were of 20-30 years. Most patients were males (92%). 84% of the cases, the dominant side is involved, right shoulder. Number of episodes of recurrent dislocation prior to surgery averaged 5 times. most common mechanism was Accidental fall (48%) followed by sports (32%), RTA (20%). Association of Hill-Sachs lesions was 80% with labral-tear secured with extra suture anchor. Preoperative parameters; number of dislocations, chronicity, reduction method do not have any significance on outcome. Rowe-score improved from 40.2% Preoperatively to 89.8% postoperatively at 12 weeks. 80% of patients is having excellent outcome. Conclusions: Arthroscopic Bankart’s repair is gold standard for labral tear due to recurrent shoulder dislocation which led to short surgery time, less intraoperative and postoperative complications, increased shoulder function among patients. Associated Hill Sach’s lesion should be repaired for better functional outcome

    Studies on the effects of Adansonia Digitata Lin's fruits on diabetes

    The point of this study is to look into how Adansonia digitata Linn plants can help people with diabetes. The bioactive extract was chosen by using the Haemoglobin Glycosylation Inhibition Assay and α-Amylase Inhibition Assay in a lab setting.So, the ethanolic solution was the most effective at fighting diabetes. In vivo study was done on it because of this. The extract was determined to be safe up to a level of 5000 mg/kg in a trial on acute poisoning. Anti-diabetic action was tested using a model of diabetes mellitus caused by streptozotocin. The factors that were looked at were changes in body weight and blood sugar levels. It was determined that following significant necrotic alterations, a damaged β-cell population, and smaller cells, the cells in the diabetes control group developed atrophy and fibrosis. The group that got the test amount, on the other hand, had normal pancreatic cells, more and bigger islets, and no necrosis or fibrotic changes. These were about the same amounts as those found in people who took Glibenclamide, which is the common drug. The analysis of phytochemicals showed that flavonoids exist. These chemicals may be what give Adansonia digitata Linn's fruits their ability to help people with diabetes. The current study says that ethanolic extract greatly lowers the chance of getting diabetes in both lab-based and real-life setting

    Conservation Laws and Integrability of a One-dimensional Model of Diffusing Dimers

    We study a model of assisted diffusion of hard-core particles on a line. The model shows strongly ergodicity breaking : configuration space breaks up into an exponentially large number of disconnected sectors. We determine this sector-decomposion exactly. Within each sector the model is reducible to the simple exclusion process, and is thus equivalent to the Heisenberg model and is fully integrable. We discuss additional symmetries of the equivalent quantum Hamiltonian which relate observables in different sectors. In some sectors, the long-time decay of correlation functions is qualitatively different from that of the simple exclusion process. These decays in different sectors are deduced from an exact mapping to a model of the diffusion of hard-core random walkers with conserved spins, and are also verified numerically.
