    Development of a Measure to Evaluate the Quality of Dementia Care Provided by Caregivers at Unit Care Geriatric Health Service Facilities

    This study was conducted to develop a measure for evaluating the quality of dementia care provided by caregivers in unit care geriatric health service facilities in Japan. A secondary factor model consisting of the following factors was used in the study. Primary factors were: "Creating a safe environment with a home-like atmosphere," "Helping residents maintain their ordinary life," "Helping maintain and improve the lifestyles of individual residents," "Providing support in a way that respects each resident\u27s unique personality," "Encouraging social interaction with others" and "Maintaining family relationships," and one secondary factor, "Quality of dementia care." The construct validity of the above measure was assessed in terms of the relationship between the factor model and an external criterion using structural equation modeling (SEM) based on data from 984 caregivers. Job satisfaction was used as the external criterion. As a result, the fit of the factor model to the data and the relationship between dementia care quality and job satisfaction were statistically supported. The Cronbach alpha coefficient for the measure was within statistically permissible limits. Based on the above results, we concluded that the measure to evaluate dementia care quality that was developed in the study is effective for evaluating the quality of dementia care provided by caregivers in unit care geriatric health service facilities

    Relationships between Dementia Care Duties and the Job Satisfaction of Nursing Staff in Unit-type Geriatric Health Service Facilities

    This study examined the relationship between the frequency of high-quality dementia care duties and job satisfaction among nursing employees working in unit care geriatric health service facilities, including the perceived difficulty of performing care and responses to difficult work situations. Self-administered questionnaires were distributed to a total of 608 subjects. 324 effective responses were received and analyzed. A causal model was proposed in which the frequency of high-quality dementia care is influenced by the perceived difficulty of practicing such care, and that these two factors, mediated by control and escapes responses, affect job performance. Structural equation modeling was employed for assessment of fit and to evaluate the relationships between variables. After confirming that the proposed model fitted the data, it was found that high-quality dementia care is indeed associated with the perceived difficulty of practicing such care and that these two factors, mediated by control responses, are associated with job satisfaction. The results indicate that sustaining and increasing job satisfaction requires adopting control-oriented responses and improving the quality of dementia care

    Distribution of Lenticular Astigmatism in a Pre-Cataract Surgery Population

    Recently custom ablation of LASIK (laserin situ keratomileusis) has rapidly evolved. It could achieve supervision temporarily, but we suspect that the vision could deteriorate due to against-the rule astigmatism decades after the operation. To clarify this concern, we evaluated distribution of the total and corneal astigmatism of 101 eyes of 65 pre-cataract surgery patients (meanage:73 years). Then we calculated the lenticular astigrlatism by vector analysis. The mean amounts of total and corneal astigmatism were 1.22±1.50D and 0.97±0.84D each. The percentages of no astigmatism: oblique: with-the-rule: against-the-rule were 32: 4: 15: 50 and 7: 28: 26: 40, respectively. The mean amount of lenticular astigrlatism measured by vector analysis was 1.6± 1.4D. The percentage of no astigmatism: oblique: with-the-rule: against-the-rule was 2: 0: 39: 59. This biased distribution of astigmatism might have contributed to the biased distribution (no and against-the-rule) of total astigmatism. These data indicate that in a pre-cataract surgery population against-the-rule astigmatism is predominant in both corneal and lenticular astigmatism. We suspect that custom correction of adolescent eyes, in which with-the-rule astigmatism is predominant, might elicit more against-the-rule astigmatism when they reach pre-cataract surgery age population, leading to a decline in quality of vision

    Molecular phylogeny of the higher and lower taxonomy of the Fusarium genus and differences in the evolutionary histories of multiple genes

    <p>Abstract</p> <p>Background</p> <p>Species of the <it>Fusarium </it>genus are important fungi which is associated with health hazards in human and animals. The taxonomy of this genus has been a subject of controversy for many years. Although many researchers have applied molecular phylogenetic analysis to examine the taxonomy of <it>Fusarium </it>species, their phylogenetic relationships remain unclear only few comprehensive phylogenetic analyses of the <it>Fusarium </it>genus and a lack of suitable nucleotides and amino acid substitution rates. A previous stugy with whole genome comparison among <it>Fusairum </it>species revealed the possibility that each gene in <it>Fusarium </it>genomes has a unique evolutionary history, and such gene may bring difficulty to the reconstruction of phylogenetic tree of <it>Fusarium</it>. There is a need not only to check substitution rates of genes but also to perform the exact evaluation of each gene-evolution.</p> <p>Results</p> <p>We performed phylogenetic analyses based on the nucleotide sequences of the rDNA cluster region (rDNA cluster), and the β-tubulin gene (<it>β-tub</it>), the elongation factor 1α gene (<it>EF-1α</it>), and the aminoadipate reductase gene (<it>lys2</it>). Although incongruence of the tree topologies between <it>lys2 </it>and the other genes was detected, all genes supported the classification of <it>Fusarium </it>species into 7 major clades, I to VII. To obtain a reliable phylogeny for <it>Fusarium </it>species, we excluded the <it>lys2 </it>sequences from our dataset, and re-constructed a maximum likelihood (ML) tree based on the combined data of the rDNA cluster, <it>β-tub</it>, and <it>EF-1α</it>. Our ML tree indicated some interesting relationships in the higher and lower taxa of <it>Fusarium </it>species and related genera. Moreover, we observed a novel evolutionary history of <it>lys2</it>. We suggest that the unique tree topologies of <it>lys2 </it>are not due to an analytical artefact, but due to differences in the evolutionary history of genomes caused by positive selection of particular lineages.</p> <p>Conclusion</p> <p>This study showed the reliable species tree of the higher and lower taxonomy in the lineage of the <it>Fusarium </it>genus. Our ML tree clearly indicated 7 major clades within the <it>Fusarium </it>genus. Furthermore, this study reported differences in the evolutionary histories among multiple genes within this genus for the first time.</p

    Functional expression of thiocyanate hydrolase is promoted by its activator protein, P15K

    AbstractThiocyanate hydrolase (SCNase) is a cobalt-containing enzyme with a post-translationally modified cysteine ligand, γCys131-SO2H. When the SCNase α, β and γ subunits were expressed in Escherichia coli, the subunits assembled to form a hetero-dodecamer, (αβγ)4, like native SCNase but exhibited no catalytic activity. Metal analysis indicated that SCNase was expressed as an apo-form irrespective of the presence of cobalt in the medium. On the contrary, SCNase co-expressed with P15K, encoded just downstream of SCNase genes, in cobalt-enriched medium under the optimized condition (SCNase(+P15K)) possessed 0.86 Co atom/αβγ trimer and exhibited 78% of the activity of native SCNase. SCNase(+P15K) showed a UV–Vis absorption peak characteristic of the SCNase cobalt center. About 70% of SCNase(+P15K) had the γCys131-SO2H modification. These results indicate that SCNase(+P15K) is the active holo-SCNase. P15K is likely to promote the functional expression of SCNase probably by assisting the incorporation of cobalt ion

    Evolution of superconductivity in isovalent Te-substituted KxFe2-ySe2 crystals

    We report the evolution of superconductivity and the phase diagram of the KxFe2-ySe2-zTez (z=0-0.6) crystals grown by a simple one-step synthesis. No structural transition is observed in any crystals, while lattice parameters exhibit a systematic expansion with Te content. The Tc exhibits a gradual decrease with increasing Te content from Tconset = 32.9 K at z = 0 to Tconset = 27.9 K at z = 0.5, followed by a sudden suppression of superconductivity at z = 0.6. Upon approaching a Te concentration of 0.6, the shielding volume fraction decreases and eventually drops to zero. Simultaneously, hump positions in r-T curve shift to lower temperatures. These results suggest that isovalent substitution of Te for Se in KxFe2-ySe2 crystals suppresses the superconductivity in this system.Comment: 10 pages, 1 table, 8 figure

    Pancreatic ductal adenocarcinoma (PDAC) is caused by genetic mutations in four genes: KRAS proto-oncogene and GTPase (KRAS), tumor protein P53 (TP53), cyclin-dependent kinase inhibitor 2A (CDKN2A), and mothers against decapentaplegic homolog 4 (SMAD4), also called the big 4. The changes in tumors are very complex, making their characterization in the early stages challenging. Therefore, the development of innovative therapeutic approaches is desirable. The key to overcoming PDAC is diagnosing it in the early stages. Therefore, recent studies have investigated the multifaced characteristics of PDAC, which includes cancer cell metabolism, mesenchymal cells including cancer-associated fibroblasts and immune cells, and metagenomics, which extend to characterize various biomolecules including RNAs and volatile organic compounds. Various alterations in the KRAS-dependent as well as KRAS-independent pathways are involved in the refractoriness of PDAC. The optimal combination of these new technologies is expected to help treat intractable pancreatic cancer
