    Undifferentiated carcinoma of the pancreas with osteoclast-like giant cells reported in an asymptomatic patient: a rare case and literature review

    Undifferentiated carcinoma of the pancreas with osteoclast-like giant cells (UC-OGC) is a rare and poorly described pancreatic malignancy. It is comprised of mononuclear, pleomorphic, and undifferentiated cells as well as osteoclast-like giant cells (OGC’s). It constitutes less than 1% of pancreatic non-endocrine neoplasia and is twice as likely to occur in females as in males. Its histopathologic properties remain poorly understood. It is suspected that UC-OGC is of epithelial origin that can then transition to mesenchymal elements. As part of this study, we describe a case of a malignant pancreatic neoplasm that was discovered in a 69-year old patient as an incidental finding. We also provide an overview of previously published data to highlight UC-OGC’s clinical and pathologic features

    Energy and Exergy Analysis of Vapor Compression Refrigeration System with Low-GWP Refrigerants

    In this paper, a first- and second-law analysis of vapor compression refrigeration is presented to estimate and propose the replacement of R134 with working fluids having less global warming potential (GWP) and less exergy destruction and irreversibilities. Six different refrigerants were studied, namely, R717, R1234yf, R290, R134a, R600a, and R152a. A thermodynamic model was designed on Engineering Equation Solver (EES) software, and performance parameters were calculated. The model was deployed on all six refrigerants, while the used output parameters of performance were cooling capacity, coefficient of performance, discharge temperature, total exergy destruction, relative exergy destruction rates of different components, second-law efficiency, and efficiency defect of each component. The performance parameters were estimated at different speeds of the compressor (1000, 2000, and 3000 rpm) and fixed condenser and evaporator temperatures of 50 °C and 5 °C, respectively. The isentropic efficiency of the compressor was the same as the volumetric efficiency, and it was taken as 75%, 65%, and 55% at the compressor speeds of 1000 rpm, 2000 rpm, and 3000 rpm, respectively. A comparison of the performance parameters was presented by importing the results in MATLAB. It was found that the compressor had the highest exergy destruction compared to the other components. It was found that R152 was the refrigerant with zero ozone depletion potential (ODP) and a GWP value of 140 with less exergy destruction and irreversibilities. Moreover, it was easy to use R152a with good thermodynamic characteristics. It is estimated that R152a is a suitable replacement for R134a, as it can be used with few modifications

    Knowledge, attitudes and practices of parents regarding antibiotic use in children

    Background: Emerging resistance to antimicrobial chemotherapy is becoming a challenge for medicine in recent times. Un-prescribed use of antibiotics is a major contributor to development of this problem. In Pakistan access to antibiotics remains unchecked and hence results in it are over use. The aim of this study was to assess knowledge of parents regarding use of antibiotics, its associated problems, their source of information and their expectations from Paediatricians for prescription of anti-biotics.Methods: This is a questionnaire based cross sectional study conducted in Hayatabad town, District Peshawar. Parents who were consenting, had children aged between 0-16 years, and were not related to medical profession were included in study. Total number of participants interviewed was 400. Analysis was done using prevalence ratios.Results: Most of the participants were mothers. Majority of respondents were literate with education up till level of Graduation. 64% mentioned that they enjoyed a good access to healthcare. Most common source for use of antibiotics was Physician. 35% mentioned that antibiotics must be administered in any case of fever, 47% thought antibiotics to increase recovery time and 51% knew that antibiotics have their own side effect. The most common reason to administer un-prescribed antibiotics was same antibiotic being prescribed by a physician earlier followed by family member or pharmacist recommending use of antibiotic. Lack of resources was denied as a reason for self-administration of antibiotics by majority of parents.CONCLUSION: There is a need of intervention to increase awareness regarding judicious use of antibiotics and to check un-prescribed dispensing of antibiotics

    Pulmonary placental transmogrification associated with adenocarcinoma of the lung: a case report with a comprehensive review of the literature

    Pulmonary placental transmogrification (PT) is a rare entity with less than 40 cases reported in the literature. Most reported cases are associated with either bullous emphysema or with pulmonary fibrochondromatous hamartomas. We present only the second case of PT associated with adenocarcinoma of the lung. A 67-year-old female with multiple chronic medical ailments presented with shortness of breath and was found to have a 6-cm mass in the upper lobe of her right lung. A computed tomography (CT) guided core biopsy was performed that showed a well-differentiated adenocarcinoma. Interestingly the normal lung tissue showed placental villous architecture. A unique feature of our case is that the diagnosis was made on a needle core biopsy, unlike all the other cases in the literature. We also provide a comprehensive review of this rare entit

    Automated deep bottleneck residual 82-layered architecture with Bayesian optimization for the classification of brain and common maternal fetal ultrasound planes

    Despite a worldwide decline in maternal mortality over the past two decades, a significant gap persists between low- and high-income countries, with 94% of maternal mortality concentrated in low and middle-income nations. Ultrasound serves as a prevalent diagnostic tool in prenatal care for monitoring fetal growth and development. Nevertheless, acquiring standard fetal ultrasound planes with accurate anatomical structures proves challenging and time-intensive, even for skilled sonographers. Therefore, for determining common maternal fetuses from ultrasound images, an automated computer-aided diagnostic (CAD) system is required. A new residual bottleneck mechanism-based deep learning architecture has been proposed that includes 82 layers deep. The proposed architecture has added three residual blocks, each including two highway paths and one skip connection. In addition, a convolutional layer has been added of size 3 × 3 before each residual block. In the training process, several hyper parameters have been initialized using Bayesian optimization (BO) rather than manual initialization. Deep features are extracted from the average pooling layer and performed the classification. In the classification process, an increase occurred in the computational time; therefore, we proposed an improved search-based moth flame optimization algorithm for optimal feature selection. The data is then classified using neural network classifiers based on the selected features. The experimental phase involved the analysis of ultrasound images, specifically focusing on fetal brain and common maternal fetal images. The proposed method achieved 78.5% and 79.4% accuracy for brain fetal planes and common maternal fetal planes. Comparison with several pre-trained neural nets and state-of-the-art (SOTA) optimization algorithms shows improved accuracy
