    A study of the regional economic development impact of a university led entrepreneurship education programme for small business owners

    The health and performance of the small- and medium-sized enterprise (SME) sector is crucial to regional and national economies of most developed countries (Jones, Macpherson and Thorpe, 2010; Thorpe, Cope, Ram and Pedler, 2009). In recognition of this, Higher Education Institutions (HEIs) have been supported through government policy to provide training programmes for SMEs aimed at developing a higher level of skills that will support small business growth (European Commission, 2003; Lambert Review, 2003; Leitch Review of Skills, 2006; DIUS, 2007, 2008; Zhang and Hamilton, 2010). In this paper we explore the relationship between entrepreneurship education and regional development by examining a programme delivered by a higher education institution for growth-oriented small business owners. We concern ourselves with addressing the overarching questions: What is the impact of HEI and SME engagement on entrepreneurship education? And How does HEI/SME engagement impact on a regional economy? To deal with these questions a qualitative approach to the research was adopted to examine the situations of five SME owner/managers over a five year period. Findings show that entrepreneurship education delivered a range of economic benefits to SMEs and the region. Moreover, through engaging owner/managers have the opportunity to interact with others and this supports the growth and development of the business

    Individual Nurse Productivity in Preparing Patients for Discharge Is Associated with Patient Likelihood of 30-Day Return to Hospital

    Objective: Applied to value-based health care, the economic term “individual productivity” refers to the quality of an outcome attributable through a care process to an individual clinician. This study aimed to (1) estimate and describe the discharge preparation productivities of individual acute care nurses and (2) examine the association between the discharge preparation productivity of the discharging nurse and the patient’s likelihood of a 30-day return to hospital [readmission and emergency department (ED) visits]. Research Design: Secondary analysis of patient-nurse data from a cluster-randomized multisite study of patient discharge readiness and readmission. Patients reported discharge readiness scores; postdischarge outcomes and other variables were extracted from electronic health records. Using the structure-process-outcomes model, we viewed patient readiness for hospital discharge as a proximal outcome of the discharge preparation process and used it to measure nurse productivity in discharge preparation. We viewed hospital return as a distal outcome sensitive to discharge preparation care. Multilevel regression analyses used a split-sample approach and adjusted for patient characteristics. Subjects: A total 522 nurses and 29,986 adult (18+ y) patients discharged to home from 31 geographically diverse medical-surgical units between June 15, 2015 and November 30, 2016. Measures: Patient discharge readiness was measured using the 8-item short form of Readiness for Hospital Discharge Scale (RHDS). A 30-day hospital return was a categorical variable for an inpatient readmission or an ED visit, versus no hospital return. Results: Variability in individual nurse productivity explained 9.07% of variance in patient discharge readiness scores. Nurse productivity was negatively associated with the likelihood of a readmission (−0.48 absolute percentage points, P\u3c0.001) and an ED visit (−0.29 absolute percentage points, P=0.042). Conclusions: Variability in individual clinician productivity can have implications for acute care quality patient outcomes

    Feasibility randomised controlled trial comparing TRAK-ACL digital rehabilitation intervention plus treatment as usual versus treatment as usual for patients following anterior cruciate ligament reconstruction

    Objectives: To evaluate the feasibility of trialling taxonomy for the rehabilitation of knee conditions-ACL (TRAK-ACL), a digital health intervention that provides health information, personalised exercise plans and remote clinical support combined with treatment as usual (TAU), for people following ACL reconstruction. Methods: The study design was a two-arm parallel randomised controlled trial (RCT). Eligible participants were English-speaking adults who had undergone ACL reconstruction within the last 12 weeks, had access to the internet and could provide informed consent. Recruitment took place at three sites in the UK. TRAK-ACL intervention was an interactive website informed by behaviour change technique combined with TAU. The comparator was TAU. Outcomes were: recruitment and retention; completeness of outcome measures at follow-up; fidelity of intervention delivery and engagement with the intervention. Individuals were randomised using a computer-generated random number sequence. Blinded assessors allocated groups and collected outcome measures. Results: Fifty-nine people were assessed for eligibility at two of the participating sites, and 51 were randomised; 26 were allocated to TRAK-ACL and 25 to TAU. Follow-up data were collected on 44 and 40 participants at 3 and 6 months, respectively. All outcome measures were completed fully at 6 months except the Client Service Receipt Inventory. Two patients in each arm did not receive the treatment they were randomised to. Engagement with TRAK-ACL intervention was a median of 5 logins (IQR 3-13 logins), over 18 weeks (SD 12.2 weeks). Conclusion: TRAK-ACL would be suitable for evaluation of effectiveness in a fully powered RCT

    Ricci flow for homogeneous compact models of the universe

    Using quaternions, we give a concise derivation of the Ricci tensor for homogeneous spaces with topology of the 3-dimensional sphere. We derive explicit and numerical solutions for the Ricci flow PDE and discuss their properties. In the collapse (or expansion) of these models, the interplay of the various components of the Ricci tensor are studied.     Case-finding in clinical practice: An appropriate strategy for dementia identification?

As screening is not generally recommended, case-finding in clinical practice is encouraged as an alternative dementia identification strategy. The approaches of screening and case-finding are often confused, with uncertainty about what case-finding should involve and under what circumstances it is appropriate. We propose a formal definition of dementia case-finding with a clear distinction from screening. We critically examine case-finding policy and practice and propose evidence requirements for implementation in clinical practice. Finally, we present a case-finding pathway and discuss the available evidence for best practice at each stage, with recommendations for research and practice.     Local bulk operators in AdS/CFT: a boundary view of horizons and locality

    We develop the representation of local bulk fields in AdS by non-local operators on the boundary, working in the semiclassical limit and using AdS_2 as our main example. In global coordinates we show that the boundary operator has support only at points which are spacelike separated from the bulk point. We construct boundary operators that represent local bulk operators inserted behind the horizon of the Poincare patch and inside the Rindler horizon of a two dimensional black hole. We show that these operators respect bulk locality and comment on the generalization of our construction to higher dimensional AdS black holes.Comment: 28 pages, 4 figures, late

    A direct D-bar reconstruction algorithm for recovering a complex conductivity in 2-D

    A direct reconstruction algorithm for complex conductivities in W2,(Ω)W^{2,\infty}(\Omega), where Ω\Omega is a bounded, simply connected Lipschitz domain in R2\mathbb{R}^2, is presented. The framework is based on the uniqueness proof by Francini [Inverse Problems 20 2000], but equations relating the Dirichlet-to-Neumann to the scattering transform and the exponentially growing solutions are not present in that work, and are derived here. The algorithm constitutes the first D-bar method for the reconstruction of conductivities and permittivities in two dimensions.     Herschel Observations of Cataclysmic Variables

    We have used the PACS instrument on the Herschel Space Observatory to observe eight cataclysmic variables at 70 and 160 μm. Of these eight objects, only AM Her was detected. We have combined the Herschel results with ground-based, Spitzer, and WISE observations to construct spectral energy distributions for all of the targets. For the two dwarf novae in the sample, SS Cyg and U Gem, we find that their infrared luminosities are completely dominated by their secondary stars. For the two highly magnetic "polars" in our survey, AM Her and EF Eri, we find that their mid-infrared excesses, previously attributed to circumbinary dust emission, can be fully explained by cyclotron emission. The WISE light curves for both sources show large, orbitally modulated variations that are identically phased to their near-IR light curves. We propose that significant emission from the lowest cyclotron harmonics (n ≤ 3) is present in EF Eri and AM Her. Previously, such emission would have been presumed to be optically thick, and not provide significant orbitally modulated flux. This suggests that the accretion onto polars is more complicated than assumed in the simple models developed for these two sources. We develop a model for the near-/mid-IR light curves for WZ Sge with an L2 donor star that shows that the ellipsoidal variations from its secondary star are detected. We conclude that none of the targets surveyed have dusty circumbinary disks
