    There are a vast number of operations carried out every year, with a small proportion of patients being at highest risk of mortality and morbidity. There has been considerable work to try and identify these high-risk patients. In this paper, we look in detail at the commonly used perioperative risk prediction models. Finally, we will be looking at the evolution and evidence for functional assessment and the National Surgical Quality Improvement Program (in the USA), both topical and exciting areas of perioperative prediction

    Nonlinear Cosmological Power Spectra in Real and Redshift--Space

    We present an expression for the nonlinear evolution of the cosmological power spectrum based on following Lagrangian trajectories. This is simplified using the Zel'dovich approximation to trace particle displacements, assuming Gaussian initial conditions. The model is found to exhibit the transfer of power from large to small scales expected in self- gravitating fields. We have extended this analysis into redshift-space and found a solution for the nonlinear, anisotropic redshift-space power spectrum in the limit of plane--parallel redshift distortions. The quadrupole-to- monopole ratio is calculated for the case of power-law initial spectra. We find that the shape of this ratio depends on the shape of the initial spectrum, but when scaled to linear theory depends only weakly on the redshift-space distortion parameter, β\beta. The point of zero-crossing of the quadrupole, k0k_0, is found to obey a scaling relation. This model is found to be in agreement with NN-body simulations on scales down to the zero-crossing of the quadrupole, although the wavenumber at zero-crossing is underestimated. These results are applied to the quadrupole--monopole ratio found in the merged QDOT+1.2 Jy IRAS redshift survey. We have estimated that the distortion parameter is constrained to be β>0.5\beta>0.5 at the 95%95 \% level. The local primordial spectral slope is not well constrained, but analysis suggests n≈−2n \approx -2 in the translinear regime. The zero-crossing scale of the quadrupole is k0=0.5±0.1h/Mpck_0=0.5 \pm 0.1 h/Mpc and from this we infer the amplitude of clustering is σ8=0.7±0.05\sigma_8=0.7 \pm 0.05. We suggest that the success of this model is due to nonlinear redshift--space effects arising from infall onto caustics and is not dominated by virialised cluster cores.Comment: 13 pages, uufiles, Latex with 6 postscript figures, submitted to MNRA

    This report is the second year evaluation of the person centred counselling service for male victims and perpetrators of domestic violence at HMP Doncaster in 2006/7. This report follows on from the previous year's evaluation, Raging Anger Within Me, which evaluated this project in 2005/6. Initiated by the Doncaster Rape and Sexual Abuse Counselling Centre (DRSACC), the second year of counselling service delivery was also funded by Lloyds TSB and the Tudor Trust. The report provides a background to the service offered, details the key factors in the service's success and makes recommendations for the future sustainability of this innovative service. The report also contains a particularly interesting section detailing how the appropriateness of the service is perceived by experienced prison staff

    Generational Aspects of U.S. Public Opinion on Renewable Energy and Climate Change

    The topics of climate change and renewable energy often are linked in policy discussions and scientific analysis, but public opinion on these topics exhibits both overlap and divergence. Although renewable energy has potentially broader acceptance than anthropogenic climate change, it can also sometimes face differently-based opposition. Analyses of U.S. and regional surveys, including time series of repeated surveys in New Hampshire (2010–2018) and northeast Oregon (2011–2018), explore the social bases of public views on both issues. Political divisions are prominent, although somewhat greater regarding climate change. Such divisions widen with education, an interaction effect documented in other studies as well. We also see robust age and temporal effects. Younger adults more often prioritize renewable energy development, and agree with scientists on the reality of anthropogenic climate change (ACC). Across all age groups and both regional series, support for renewable energy and recognition of ACC have been gradually rising. These trends, together with age-cohort replacement and possible changes in age-group voting participation, suggest that public pressure for action on these issues could grow

    Trust in scientists on climate change and vaccines

    On climate change and other topics, conservatives have taken positions at odds with a strong scientific consensus. Claims that this indicates a broad conservative distrust of science have been countered by assertions that while conservatives might oppose the scientific consensus on climate change or evolution, liberals oppose scientists on some other domains such as vaccines. Evidence for disproportionately liberal bias against science on vaccines has been largely anecdotal, however. Here, we test this proposition of opposite biases using 2014 survey data from Oregon and New Hampshire. Across vaccine as well as climate change questions on each of these two surveys, we find that Democrats are most likely to say they trust scientists for information, and Tea Party supporters are least likely, contradicting the proposition of opposite bias. Moreover, partisan divisions tend to widen with education. Theoretical explanations that have been offered for liberal trust or conservative distrust of science in other specific domains such as climate change or environmental protection fit less well with these results on vaccines. Given the much different content of climate change and vaccine issues, the common political pattern appears more consistent with hypotheses of broader ideological divisions on acceptance of science

    Tools for Risk Analysis: Updating the 2006 WHO guidelines

    This chapter reviews developments since the WHO Guidelines for the safe use of wastewater in agriculture were published in 2006. The six main developments are: the recognition that the tolerable additional disease burden may be too stringent for many developing countries; the benefits of focusing on single-event infection risks as a measure of outbreak potential when evaluating risk acceptability; a more rigorous method for estimating annual risks; the availability of dose-response data for norovirus; the use of QMRA to estimate Ascaris infection risks; and a detailed evaluation of pathogen reductions achieved by produce-washing and disinfection. Application of the developments results in more realistic estimates of the pathogen reductions required for the safe use of wastewater in agriculture and consequently permits the use of simpler wastewater treatment processes
