    Spatial Patterns Associating Low Birth Weight with Environmental and Behavioral Factors

    Low birth weight (LBW) is a significant public health problem in the world. It was estimated globally by the World Health Organization (WHO) that prevalence of LBW was 15% of all births. In Murung Raya district LBW cases remain high. This paper aimed to identify and discuss the relationship between environmental risk factors with LBW in Murung Raya.A spatial analysis was conducted with 150 women as the total participantswho were recruited through the incidence data in 2013-2014. The questionnaires, medical records, and geographic data were measured by Stata software, ArcGis, SatScan, and Geoda. The study results indicated there was significant correlation between health behavior and environmental variables with the strength of external neighborhood effect across LBW risk factors. More intense clustering of high values (hot spots) was found through the spatial analysis showing that most of the cases were located near the defined buffer zone. This research demonstrates that the spatial pattern analysis provided greater statistical power to detect an effect that was not apparent in the previous epidemiology studies

    Comparative analysis of genetic crossover operators in knapsack problem

    The Genetic Algorithm (GA) is an evolutionary algorithms and technique based on natural selections of individuals called chromosomes. In this paper, a method for solving Knapsack problem via GA (Genetic Algorithm) is presented. We compared six different crossovers: Crossover single point, Crossover Two point, Crossover Scattered, Crossover Heuristic, Crossover Arithmetic and Crossover Intermediate. Three different dimensions of knapsack problems are used to test the convergence of knapsack problem.     Factors affecting pregnant women's participation in HIV screening test in public health care centers of YogyakartaPurposeThis study aimed to explore the behavioral factors that influence pregnant women's participation in HIV testing in public health centers of Yogyakarta. MethodsThis research used quantitative and qualitative methods with a quantitative analytical descriptive design and cross-sectional observational approach to determine any association between independent variables and the dependent variable. Qualitative in-depth interviews were conducted to support the results of the quantitative research. ResultsResults showed 92.94% of respondents had been HIV tested with an average age of 25-34 years. Almost all pregnant women had an HIV test in a clinic although not all mothers have the knowledge, perceived susceptibility, perceived severity, and perceived benefits. While the exposure information, and support of health workers were high, and there was a perception of low resistance, since the HIV test is a test done by health workers and an initiative of the government program. ConclusionThis study recommends that counseling and information on HIV and HIV testing from health workers and support from related agencies are needed. Subsequent research could examine factors that affect healthcare workers' performance in providing education to health-care users

    Kasus-Kasus Maternal di Berita Online Menyangkut Hak Asasi yang Patut Menjadi Pelajaran dalam Pendidikan Bidan di Indonesia

    Abstrak Gerakan menghormati hak asasi manusia dalam pelayanan kesehatan maternal di berbagai negara mendorong petugas kesehatan bertindak lebih manusiawi. Perempuan selama ini diam karena tidak ingin memutus hubungan harmonis dengan bidan serta jarangnya isu ini diangkat dalam pertemuan profesi dan publikasi ilmiah menyebabkan perhatian yang rendah terhadap hak asasi manusia dalam pelayanan kesehatan maternal. Penelitian ini bertujuan untuk mengidentifikasi kegagalan dalam pemenuhan hak kesehatan ibu hamil dan bersalin serta mengeksplorasi alasan-alasan yang menyebabkan hal tersebut terjadi. Metode penelitian dilakukan dengan melakukan penelusuran berita online kompas.com, tribunnews.com, dan detiknews.com tahun 2016-2018 dengan kata kunci “malpraktik” dan “bidan”, tentang persoalan hak asasi manusia dalam pelayanan bidan. Kami mengikuti kasus dalam lebih dari satu media online, disertai perkembangan berita tentang kasus tersebut. Hasil penelitian menunjukkan bahwa dominasi bidan dalam pelayanan kesehatan maternal di Indonesia, berdampak pada pengabaian hak asasi perempuan dan keselamatan. Kejadian ini pada kelompok orang dengan pendapatan rendah dan kelompok yang mampu. Didapatkan praktik bidan di luar kewenangan, kemungkinan motivasi mendapatkan keuntungan, dominasi bidan, posisi sosial perempuan yang rendah dalam pelayanan kesehatan maternal, dan keterlibatan masyarakat yang rendah untuk mendukung perempuan yang mengalami ketidakadilan dalam layanan kesehatan. Kepercayaan dan ketergantungan perempuan pada bidan, berdampak pengabaian hak asasi dan keselamatan ibu, tindakan di luar kewenangan, untuk pencarian keuntungan dalam praktik pribadi. Organisasi profesi dan pendidik bidan harus memasukkan penerapan hak asasi manusia dalam praktik kebidanan melalui kasus-kasus dari berita online dalam pertemuan berkala asosiasi profesi, serta kurikulum pendidikan, untuk mencegah dampak buruk pengabaian hak asasi ibu di masa depan. Low attention to human rights issue in maternal health services due to the silent of the victims-because women do not want to break the harmonious relationship with midwives, and this issue is rarely raised in professional meetings and scientific publications. This study aims to identify failures in fulfilling the health rights of pregnant women and childbirth and explore the reasons for this. The research method is done by searching online news kompas.com, tribunnews.com, and detiknews.com in 2016-2018 with the keywords “malpractice” and “midwife”, about human rights issues in midwifery services. We followed the case in more than one online media, accompanied by the case progress report. The results of the study show that the dominance of midwives in maternal health services in Indonesia has an impact on neglecting women’s human rights and safety. This event is in the group of people with low income and groups who are able. The practice of midwive’s out of authority, possible motivations for profit, dominance of midwives, low social position of women in maternal health services, and low community involvement in supporting women who experience inequality in maternal health services. Trust and dependence of women on midwives, impact on neglecting human rights and maternal safety, actions that are beyond authority, for seeking profit in private practice. Midwife professional organizations and educators must incorporate the application of human rights in midwifery practice through cases from online news in periodic meetings of professional associations, as well as educational curricula, to prevent the adverse effects of neglecting maternal rights in the future.     Validation of the Patient Perception of Intensity of Urgency Scale in Patients with Lower Urinary Tract Symptoms Associated with Benign Prostatic Hyperplasia

    AbstractObjectiveTo assess the reliability and validity of scores derived from the Patient Perception of Intensity of Urgency Scale (PPIUS) in patients with lower urinary tract symptoms (LUTS) associated with benign prostatic hyperplasia (BPH).MethodsA post hoc analysis of the phase II Solifenacin and Tamsulosin in Males with Lower Urinary Tract Symptoms Associated with Benign Prostatic Hyperplasia trial (NCT00510406), a 12-week clinical trial in men with LUTS associated with BPH, assessed the measurement properties of six PPIUS-derived scores: mean score; maximum urgency score; total urgency and frequency score (TUFS; average sum of urgency scores over 3 days); and numbers of urgency episodes, urgency episodes of grade 3 or 4, and urgency incontinence episodes. Test-retest reliability, presence of floor/ceiling effects, responsiveness to change, known-group validity, and concurrent validity were assessed for each score.ResultsA total of 901 patients had at least one valid PPIUS assessment after baseline. TUFS demonstrated good test-retest reliability (intraclass correlation coefficient >0.8), discriminated between groups defined based on International Prostate Symptom Score storage score severity (known-groups validity), had high concurrent validity, and had high responsiveness to change (Guyatt’s responsiveness statistic 0.88), with an absence of floor or ceiling effects. The psychometric properties of other PPIUS-derived scores were not as consistently robust and showed either low-to-moderate responsiveness, presence of a floor or ceiling effect, or low-to-moderate test-retest reliability.ConclusionsThis study shows that the PPIUS is reliable and valid in patients with LUTS associated with BPH. TUFS provided the best combination of psychometric properties of the six scores derived from the PPIUS and appeared to be an appropriate measure of urgency and frequency

    Fixed-angle plates in patella fractures - a pilot cadaver study

    <p>Abstract</p> <p>Objective</p> <p>Modified anterior tension wiring with K-wires and cannulated lag screws with anterior tension wiring are currently the fixation of choice for patellar fractures. Failure of fixation, migration of the wires, postoperative pain and resulting revision surgery, however, are not uncommon. After preliminary biomechanical testing of a new fixed-angle plate system especially designed for fixation of patella fractures the aim of this study was to evaluate the surgical and anatomical feasibility of implanting such a plate-device at the human patella.</p> <p>Methods</p> <p>In six fresh unfixed female cadavers without history of previous fractures around the knee (average age 88.8 years) a bilateral fixed-angle plate fixation of the patella was carried out after previous placement of a transverse central osteotomy. Operative time, intra-operative problems, degree of retropatellar arthritis (following Outerbridge), quality of reduction and existence of any intraarticular screw placement have been raised. In addition, lateral and anteroposterior radiographs of all specimens were made.</p> <p>Results</p> <p>Due to the high average age of 88.8 years no patella showed an unimpaired retropatellar articular surface and all were severely osteoporotic, which made a secure fixation of the reduction forceps during surgery difficult. The operation time averaged 49 minutes (range: 36-65). Although in postoperative X-rays the fracture gap between the fragments was still visible, the analysis of the retropatellar surface showed no residual articular step or dehiscence > 0.5 mm. Also in a total of 24 inserted screws not one intraarticular malposition was found. No intraoperative complications were noticed.</p> <p>Conclusions</p> <p>Osteosynthesis of a medial third patella fracture with a bilateral fixed-angle plate-device is surgically and anatomically feasible without difficulties. Further studies have to depict whether the bilateral fixed-angle plate-osteosynthesis of the patella displays advantages over the established operative procedures.</p

    Evolution of microstructural and mechanical properties of nanocrystalline Co2FeAl Heusler alloy prepared by mechanical alloying

    Mechanical alloying (MA) has been used to fabricate the Co2FeAl Heusler alloy with a nanocrystalline structure. The formation mechanism of the alloy has been investigated. Rietveld analysis showed that all samples that were milled for more than 15 hours had an L21 structure with a space group of Fm3m. The crystallite size and internal strain of the samples were calculated using the Williamson-Hall equation. With mechanical alloying of up to 20 hours the crystallite size of Co2FeAl increased, after which the crystallite size started to decrease. In contrast, internal strain first decreased during the process and then increased with the increase of milling time. The powder obtained after 20 hours of MA was split into three parts and separately annealed at 300, 500 and 700 oC for 5 hours. A considerable increase was observed in the hardness value of powder particles with the increase of annealing temperature up to 500 oC. However, the hardness value of the sample annealed at 700 oC decreased. It seems that this feature is related to parameters such as increase of crystallite size, enhancement of lattice ordering, change in density of defects and impurities and nonstoichiometric effects
