    Type 2 Diabetes Mellitus Screening and Risk Factors Using Decision Tree: Results of Data Mining

    OBJECTIVES: The aim of this study was to examine a predictive model using features related to the diabetes type 2 risk factors. METHODS: The data were obtained from a database in a diabetes control system in Tabriz, Iran. The data included all people referred for diabetes screening between 2009 and 2011. The features considered as "Inputs" were: age, sex, systolic and diastolic blood pressure, family history of diabetes, and body mass index (BMI). Moreover, we used diagnosis as "Class". We applied the "Decision Tree" technique and "J48" algorithm in the WEKA (3.6.10 version) software to develop the model. RESULTS: After data preprocessing and preparation, we used 22,398 records for data mining. The model precision to identify patients was 0.717. The age factor was placed in the root node of the tree as a result of higher information gain. The ROC curve indicates the model function in identification of patients and those individuals who are healthy. The curve indicates high capability of the model, especially in identification of the healthy persons. CONCLUSIONS: We developed a model using the decision tree for screening T2DM which did not require laboratory tests for T2DM diagnosis

Se-dangkan risiko (risk) merupakan fungsi peluang terjadinya gangguan kesehatan dan kepara-han (severity) gangguan kesehatan oleh karena suatu bahaya.Risiko lingkungan merupakan risiko terhadap kesehatan manusia yang disebabkan oleh karena faktor lingkungan, baik lingkungan fisik, hayati maupun sosial-ekonomi-budaya. Salah satu bahaya yang berpotensi menimbulkan dampak bagi kesehatan manusia dan ling-kungan yakni bahaya kimia yang berupa keberadaan polutan di udara.Di Indonesia Analisis Risiko Kesehatan Lingkungan (ARKL) masih belum banyak dikenal dan digunakan sebagai metoda kajian dampak lingkungan terhadap kesehatan. Pa-dahal, di beberapa negara Uni Eropa, Amerika dan Australia ARKL telah menjadi proses central idea legislasi dan regulasi pengendalian dampak lingkungan.     The remuneration policy is a policy issued by the central government to give awards to civil servants on the professionalism of the work done. Remuneration is based on performance appraisal that is the target of employee and workplace behavior. This study aims to describe and analyze the implementation of the remuneration policy to improve the performance of civil servants at the Faculty of Social Sciences, State University of Malang, and to describe and analyze the factors that affect the implementation of the remuneration policy to improve the performance of civil servants at the Faculty of Social Sciences, State University of Malang. The results showed that the implementation of the remuneration policy at the Faculty of Social Sciences, State University of Malang can improve the performance of civil servants because the remuneration is based on the workload and responsibilities of employees. There are supporting factors and obstacles in the implementation of remuneration policies that affect the policy implementation process

    Clinical risk factors and bronchoscopic features of invasive aspergillosis in Intensive Care Unit patients

    Introduction. Invasive aspergillosis (IA) is an important cause of morbidity and mortality in immunocompromised patients. During recent years, a rising incidence of IA in Intensive Care Unit (ICU) patients has been reported. The patterns of IA related infection may differ according to the type of underlying disease. Unfortunately little is known about the characteristics of IA in ICU patients. In the present study we assessed IA related clinical and bronchoscopy findings in ICU patients. Materials and methods. This study was performed at the ICU units in Sari and Babul, Mazandaran from August 2009 through September 2010. We analysed 43 ICU patients with underlying predisposing conditions for IA. Bronchoalveolar lavage (BAL) samples were collected by bronchoscope twice a weekly. The samples were analyzed by direct microscopic examination, cul- ture and non-culture based diagnostic methods. Patients were assigned a probable or possible diagnosis of IA according to the consensus definition of the EORTC/MSG. Results. Out of 43 suspected patients to IA, 13 (36.1%) cases showed IA. According to criteria presented by EORTC/MSG, they were categorized as: 4 cases (30.8%) of possible IA and 9 (69.2%) of probable IA. The observed mortality was 69.2%. The main underlying predisposing conditions were neutropenia, hematologic malignancy, and COPD. The macroscopic finding in bronchoscopy included of Prulent secretion (46.6%), Mucosal bleeding (30.7%), Mucosal erythema (23%), Trachobronchomalasia (15.3%). Conclusion. The diagnosis of IA in patients with critical illness in ICU is even more difficult. The clinical diagnostic process is often dependent on indirect circumstantial data enhancing the proba- bility of IA. Bronchoscopy with inspection of the tracheobronchial tree, sampling of deep airway secretions and BAL can be helpful

    3D AMR hydrosimulations of a compact source scenario for the Galactic Centre cloud G2

    The nature of the gaseous and dusty cloud G2 in the Galactic Centre is still under debate. We present three-dimensional hydrodynamical adaptive mesh refinement (AMR) simulations of G2, modeled as an outflow from a "compact source" moving on the observed orbit. The construction of mock position-velocity (PV) diagrams enables a direct comparison with observations and allow us to conclude that the observational properties of the gaseous component of G2 could be matched by a massive (M˙w=5×107  Myr1\dot{M}_\mathrm{w}=5\times 10^{-7} \;M_{\odot} \mathrm{yr^{-1}}) and slow (50  km  s150 \;\mathrm{km \;s^{-1}}) outflow, as observed for T Tauri stars. In order for this to be true, only the material at larger (>100  AU>100 \;\mathrm{AU}) distances from the source must be actually emitting, otherwise G2 would appear too compact compared to the observed PV diagrams. On the other hand, the presence of a central dusty source might be able to explain the compactness of G2's dust component. In the present scenario, 5-10 years after pericentre the compact source should decouple from the previously ejected material, due to the hydrodynamic interaction of the latter with the surrounding hot and dense atmosphere. In this case, a new outflow should form, ahead of the previous one, which would be the smoking gun evidence for an outflow scenario.Comment: resubmitted to MNRAS after referee report, 16 pages, 11 figure

    The Post-Pericenter Evolution of the Galactic Center Source G2

    In early 2014 the fast-moving near-infrared source G2 reached its closest approach to the supermassive black hole Sgr A* in the Galactic Center. We report on the evolution of the ionized gaseous component and the dusty component of G2 immediately after this event, revealed by new observations obtained in 2015 and 2016 with the SINFONI integral field spectrograph and the NACO imager at the ESO VLT. The spatially resolved dynamics of the Brγ\gamma line emission can be accounted for by the ballistic motion and tidal shearing of a test-particle cloud that has followed a highly eccentric Keplerian orbit around the black hole for the last 12 years. The non-detection of a drag force or any strong hydrodynamic interaction with the hot gas in the inner accretion zone limits the ambient density to less than a few 103^3 cm3^{-3} at the distance of closest approach (1500 RsR_s), assuming G2 is a spherical cloud moving through a stationary and homogeneous atmosphere. The dust continuum emission is unresolved in L'-band, but stays consistent with the location of the Brγ\gamma emission. The total luminosity of the Brγ\gamma and L' emission has remained constant to within the measurement uncertainty. The nature and origin of G2 are likely related to that of the precursor source G1, since their orbital evolution is similar, though not identical. Both object are also likely related to a trailing tail structure, which is continuously connected to G2 over a large range in position and radial velocity.Comment: 17 pages, 12 figures; accepted for publication in Ap

    Comparing mindfulness-based group therapy with treatment as usual for opioid dependents: A pilot randomized clinical trial study protocol

    Background: In response to high burden of opioid abuse in Iran, Ministry of Health has launched a large-scale opioid maintenance treatment program, delivered through a network of certified drug treatment centers. To promote opioid pharmacotherapies, there is an urgent need to develop and introduce evidence-based psychosocial interventions into the network. Patients and Methods: This is a randomized clinical trial (RCT) to investigate feasibility and effectiveness of adding mindfulness-based group therapy to opioid pharmacotherapies as compared to opioid pharmacotherapies alone. The primary outcomes were treatment retention and percentage of weekly morphine, methamphetamine, and benzodiazepine negative tests. Discussion: This is the first RCT that explores the effectiveness of mindfulness-based relapse prevention group therapy among opioid dependent clients in Iran. The feasibility of group therapy and comparison of outcomes in intervention and control groups should be discussed in the outcome article. © 2015, Mazandaran University of Medical Sciences
