    Transtibial amputee gait efficiency : energy storage and return versus solid ankle cushioned heel prosthetic feet

    Energy storage and return (ESR) feet have long been assumed to promote metabolically efficient amputee gait. However, despite being prescribed for approximately thirty years, there is limited evidence that they achieve this desired function. Here, we report a meta-analysis of data from ten studies which met our selection criteria to determine whether amputee walking with ESR feet is more efficient than with conventional solid ankle cushioned heel (SACH) feet. Additionally, the data were tested for a relationship with walking speed; since it has been suggested ESR feet might perform better at higher speeds. The raw data are highly variable due to differences in study protocols; therefore we normalised the data and found a statistically significant difference (p < 0.001) between ESR and SACH feet. However, the magnitude of this difference is small, with the cost of transport (COT) with ESR feet being 97.3% of the cost with SACH feet. No relationship between ESR COT and speed was found (p = 0.191). In the authors’ opinion, these results suggest that ESR feet are not effective at improving amputee COT to a clinically relevant degree. We hypothesise this is due to limited push-off at the end of stance compared with able-bodied ankles

    Adaptive working memory strategy training in early Alzheimer's disease: randomised controlled trial.

    BACKGROUND: Interventions that improve cognitive function in Alzheimer's disease are urgently required. AIMS: To assess whether a novel cognitive training paradigm based on 'chunking' improves working memory and general cognitive function, and is associated with reorganisation of functional activity in prefrontal and parietal cortices (trial registration: ISRCTN43007027). METHOD: Thirty patients with mild Alzheimer's disease were randomly allocated to receive 18 sessions of 30 min of either adaptive chunking training or an active control intervention over approximately 8 weeks. Pre- and post-intervention functional magnetic resonance imaging (fMRI) scans were also conducted. RESULTS: Adaptive chunking training led to significant improvements in verbal working memory and untrained clinical measures of general cognitive function. Further, fMRI revealed a bilateral reduction in task-related lateral prefrontal and parietal cortex activation in the training group compared with controls. CONCLUSIONS: Chunking-based cognitive training is a simple and potentially scalable intervention to improve cognitive function in early Alzheimer's disease

    The importance of sustained attention in early Alzheimer's disease

    INTRODUCTION: There is conflicting evidence regarding impairment of sustained attention in early Alzheimer's disease (AD). We examine whether sustained attention is impaired and predicts deficits in other cognitive domains in early AD. METHODS: Fifty-one patients with early AD (MMSE > 18) and 15 healthy elderly controls were recruited. The sustained attention to response task (SART) was used to assess sustained attention. A subset of 25 patients also performed tasks assessing general cognitive function (ADAS-Cog), episodic memory (Logical memory scale, Paired Associates Learning), executive function (verbal fluency, grammatical reasoning) and working memory (digit and spatial span). RESULTS: AD patients were significantly impaired on the SART compared to healthy controls (total error β = 19.75, p = 0.027). SART errors significantly correlated with MMSE score (Spearman's rho = -0.338, p = 0.015) and significantly predicted deficits in ADAS-Cog (β = 0.14, p = 0.004). DISCUSSIONS: Patients with early AD have significant deficits in sustained attention, as measured using the SART. This may impair performance on general cognitive testing, and therefore should be taken into account during clinical assessment, and everyday management of individuals with early AD

    The HATNet and HATSouth Exoplanet Surveys

    The Hungarian-made Automated Telescope Network (HATNet) has been in operation since 2003, with the key science goal being the discovery and accurate characterization of transiting extrasolar planets (TEPs) around bright stars. Using six small, 11\,cm\ aperture, fully automated telescopes in Arizona and Hawaii, as of 2017 March, it has discovered and accurately characterized 67 such objects. The HATSouth network of telescopes has been in operation since 2009, using slightly larger, 18\,cm diameter optical tubes. It was the first global network of telescopes using identical instrumentation. With three premier sites spread out in longitude (Chile, Namibia, Australia), the HATSouth network permits round-the-clock observations of a 128 square arcdegree swath of the sky at any given time, weather permitting. As of this writing, HATSouth has discovered 36 transiting exoplanets. Many of the altogether ~100 HAT and HATSouth exoplanets were the first of their kind. They have been important contributors to the rapidly developing field of exoplanets, motivating and influencing observational techniques, theoretical studies, and also actively shaping future instrumentation for the detection and characterization of such objects.Comment: Invited review chapter, accepted for publication in "Handbook of Exoplanets", edited by H.J.     Performance of optimised prosthetic ankle designs that are based on a hydraulic variable displacement actuator (VDA)

    Current energy storage and return (ESR) prosthetic feet only marginally reduce the cost of amputee locomotion compared to basic solid ankle cushioned heel (SACH) feet, possibly due to their lack of push-off at the end of stance. To our knowledge, a prosthetic ankle that utilises a hydraulic variable displacement actuator (VDA) to improve push-off performance has not previously been proposed. Therefore, here we report a design optimisation and simulation feasibility study for a VDA based prosthetic ankle. The proposed device stores the eccentric ankle work done from heel strike to maximum dorsiflexion in a hydraulic accumulator and then returns the stored energy to power push-off. Optimisation was used to establish the best spring characteristic and gear ratio between ankle and VDA. The corresponding simulations show that, in level walking, normal push-off is achieved and, per gait cycle, the energy stored in the accumulator increases by 22% of the requirements for normal push-off. Although the results are promising, there are many unanswered questions and, for this approach to be a success, a new miniature, low-losses, lightweight VDA would be required that is half the size of the smallest commercially available device

    Interventions for subjective cognitive decline: systematic review and meta-analysis

    OBJECTIVES: This review provides a broad overview of the effectiveness of interventions for subjective cognitive decline (SCD) in improving psychological well-being, metacognition and objective cognitive performance. METHODS: Databases including PubMed, Web of Science and Cochrane Systematic Reviews were searched up to August 2017 to identify randomised controlled trials evaluating interventions for SCD. Interventions were categorised as psychological, cognitive, lifestyle or pharmacological. Outcomes of interest included psychological well-being, metacognitive ability and objective cognitive performance. To assess the risk of bias, three authors independently rated study validity using criteria based on the Critical Appraisal Skills Programme. Random-effects meta-analyses were undertaken where three or more studies investigated similar interventions and reported comparable outcomes. RESULTS: Twenty studies met inclusion criteria and 16 had sufficient data for inclusion in the meta-analyses. Of these, only seven were rated as being high quality. Group psychological interventions significantly improved psychological well-being (g=0.40, 95% CI 0.03 to 0.76; p=0.03) but the improvement they conferred on metacognitive ability was not statistically significant (g=0.26, 95% CI -0.22 to 0.73; p=0.28). Overall, cognitive training interventions led to a small, statistically significant improvement in objective cognitive performance (g=0.13, 95% CI 0.01 to 0.25; p=0.03). However, the pooled effect sizes of studies using active control groups (g=0.02, 95% CI -0.19 to 0.22; p=0.85) or reporting global cognitive measures (g=0.06, 95% CI -0.19 to 0.31; p=0.66) were non-significant. CONCLUSIONS: There is a lack of high-quality research in this field. Group psychological interventions improve psychological well-being and may also improve metacognition. A large, high-quality study is indicated to investigate this further. There is no evidence to suggest that cognitive interventions improve global cognitive performance and the clinical utility of small improvements in specific cognitive domains is questionable. There is a lack of research considering lifestyle interventions and poor quality evidence for pharmacological interventions. PROSPERO REGISTRATION NUMBER: CRD42017079391

    Simulated performance of an energy storage and return prosthetic ankle based on cams and miniature hydraulics

    Prosthetic feet are limited in their ability to mimic the energy-recycling behaviour of an intact ankle, negatively affecting lower-limb amputees’ gait in terms of metabolic cost and walking speed. To overcome these weaknesses, a novel prosthetic ankle based on hydraulics is described here. The ankle joint drives two cams, which in turn drive two hydraulic rams. One cam-ram system captures the negative work done from foot-flat until maximum dorsiflexion, by pumping oil into an accumulator, while the other returns positive work during push-off providing forward propulsion through fluid flowing from the accumulator to the ram. Simulation results are promising: of the total negative work done by the prosthetic ankle over the gait cycle (i.e., the maximum amount of energy available to be stored), 78% is returned, mainly during push-off; 14% is carried forward for future gait cycles; and 8% is lost. The estimated prosthesis height and mass are approximately 26.5cm and 2.3kg. Nonetheless, further work is necessary to realise a prototype for bench and in-vivo testing. By mimicking intact ankle torque and efficiently storing and returning energy at the ankle joint, this new design may contribute to reducing amputees’ metabolic cost of walking

    The Changing Epidemiology of Malaria in Ifakara Town, Southern Tanzania.

    Between 1995 and 2000 there were marked changes in the epidemiology of malaria in Ifakara, southern Tanzania. We documented these changes using parasitological and clinical data from a series of community- and hospital-based studies involving children up to the age of 5 years. There was a right shift and lowering in the age-specific parasite prevalence in the community-based cohort studies. The incidence of clinical malaria in placebo-receiving infants in additional study cohorts dropped from 0.8 in 1995 to 0.43 episodes per infant per year in 2000, an incidence rate ratio of 0.53 (95% confidence interval: 0.404, 0.70, P<0.0001). At the same time, there was an increase in the total number of malaria admissions and a marked right shift in the age pattern of these admissions (median age in 1995 1.55 years vs. 2.33 in 2000, P<0.0001). However, the burden of malaria deaths remained in infants. We discuss how these dramatic changes in the epidemiology of malaria may have arisen from the use of currently available malaria control tools. Caution is required in the interpretation of hospital-based data as it is likely to underestimate the impact of anaemia on mortality in the community, where most paediatric deaths occur. Even in low/moderate malaria transmission settings, where older children suffer most malaria episodes, targeting effective malaria control at infants may produce important reductions in infant mortality caused by malaria
