    TRAINING OF HUMAN CAPITAL IN HEALTH AND SOCIAL REPRESENTATIONS: CASE STUDY IN HOSPITALS OF 2° LEVEL OF THE AREA EAST OF THE STATE OF MEXICO

    The focus of this work is to analyze from a medical perspective the multiple aspects that guide the criteria that assumes the training of health professionals. This research is based on the theory of social representations; this theory is referred to an epistemology of the sense common, which allows to interpret the reality-the knowledge every day, product of social interactions. Within this theoretical framework, it worked with the approach process, which focuses on the content of the representation in terms of its meaning and significance. It was a descriptive study, qualitative, cross-not experimental, with interviews semistructured with three axes categorical (training, bonding and regulation). The sample was obtained in a manner not probabilistic, they interviewed 12 subject, six heads of teaching and six medical in training attached to the institutions that agreed to participate in the research. From the crossroads of knowledge, the experiences and opinions of education managers and doctors in training, is develop that the training of the professional of the health still framed in the paradigm positivist (hegemonic biomedical). Of this form is evidence a worldview in the training of the medical and an approach to this reality training, that allows carry to the reflection all those aspects forgotten that dan by made a conformation infinite and dogmatic of medical and thus, placed in the context to the actors main (universities, institutions of health, decision makers of decisions, executing or responsible of the training).The focus of this work is to analyze from a medical perspective the multiple aspects that guide the criteria that assumes the training of health professionals.     BeCAPTCHA-Type: Biometric Keystroke Data Generation for Improved Bot Detection

    This work proposes a data driven learning model for the synthesis of keystroke biometric data. The proposed method is compared with two statistical approaches based on Universal and User-dependent models. These approaches are validated on the bot detection task, using the keystroke synthetic data to improve the training process of keystroke-based bot detection systems. Our experimental framework considers a dataset with 136 million keystroke events from 168 thousand subjects. We have analyzed the performance of the three synthesis approaches through qualitative and quantitative experiments. Different bot detectors are considered based on several supervised classifiers (Support Vector Machine, Random Forest, Gaussian Naive Bayes and a Long Short-Term Memory network) and a learning framework including human and synthetic samples. The experiments demonstrate the realism of the synthetic samples. The classification results suggest that in scenarios with large labeled data, these synthetic samples can be detected with high accuracy. However, in few-shot learning scenarios it represents an important challenge.     Mechanical thrombectomy with a novel device: initial clinical experience with the ANA thrombectomy device

    Mechanical thrombectomy; New device; StrokeTrombectomia mecànica; Nou dispositiu; IctusTrombectomía mecánica; Nuevo dispositivo; IctusIntroduction The ANA™ (Anaconda Biomed) thrombectomy system is a novel stroke thrombectomy device comprising a self-expanding funnel designed to reduce clot fragmentation by locally restricting flow while becoming as wide as the lodging artery. Once deployed, ANA allows distal aspiration in combination with a stentretriever (SR) to mobilize the clot into the funnel where it remains copped during extraction. We investigate safety and efficacy of ANA™ in a first-in-man study. Methods Prospective data was collected on 35 consecutive patients treated as first line with ANA™ at a single centre. Outcome measures included per-pass reperfusion scores, symptomatic intracerebral hemorrhage (sICH), NIHSS at day 5, and mRS at 90 days. Results Median NIHSS was 12(9−18). Sites of primary occlusion were: 5 ICA, 15 M1-MCA, 15 M2-MCA. Primary performance endpoint, mTICI 2b-3 within 3 passes without rescue therapy was achieved in 91.4% (n = 32) of patients; rate of complete recanalization (mTICI 2c-3) was 65.7%. First pass complete recanalization rate was 42.9%, and median number of ANA passes 1(IQR: 1−2). In 17.1% (n = 6) rescue treatment was used; median number of rescue passes was 2(1–7), leading to a final mTICI2b-3rate of 94.3% (n = 33). There were no device related serious adverse events, and rate of sICH was 5.7% (n = 2). At 5 days median NIHSS was 1 (IQR 1−6) and 90 days mRS 0−2 was achieved in 60% of patients. Conclusions In this initial clinical experience, the ANA™ device achieved a high rate of complete recanalization with a good safety profile and favourable 90 days clinical outcomes.The study was funded by Anaconda Biomed S.L

    Lethal Concentration of Carbonate OF Ca as a Function of the Osmotic Potential of the Solution in Sunflower (Heliantusannuus L.)

    In order to know the effect of CaCO3 in solution, sunflower seedlings cv. Victoria, an experiment was completely randomized, where five concentrations of calcium carbonate were evaluated to determine the lethal concentration (LC50), pH and EC of the solution under laboratory conditions in the Universidad Tecnologica de Tehuacan, to simulate of excess Ca++ in the soils or nutrient solution. The results indicate, the LC50 was 62.8 mg CaCO3 L-1, so maximum values for pH, EC and calcium absorption, They were achieved at concentrations of 120 and 160 mg L-1 of CaCO3. This work can be concluded, Sunflower can absorb the high levels of calcium and used as an alternative, for remediation of agricultural soils affected hard water and Ca++ salts

    Hábitos alimenticios del pez sable Trichiurus lepturus en el Golfo de Urabá, Caribe colombiano

    We studied the feeding habits of the commercial fish Trichiurus lepturus, a mesopredator with an important role in the energy flow in the coastal trophic webs, in order to contribute to the knowledge of its ecology, tool for its conservation, and an input for the management of fisheries in the Gulf of Urabá, northwestern Colombia. Using gillnets, 320 fish were captured, from which 208 stomachs containing food were extracted, and their contents separated and identified. The food spectrum of the species is integrated by 26 elements, belonging to three great taxonomic groups: fish, crustaceans and mollusks. Engraulid fishes are the main food component at all sampling sites, mosly Centengraulis edentulus and several Anchoa species. There was a trophic overlap among sites, and the importance of mangrove areas for feeding was evident. However, no seasonal or sex variation in diet composition was found, since Engraulids are one of the most abundant fish resources throughout the year in the Colombian Caribbean.Se estudiaron los hábitos alimenticios del pez comercial Trichiurus lepturus, un mesodepredador con un papel importante en el flujo de energía en las redes tróficas costeras, con el objeto de contribuir al conocimiento de su ecología, herramienta para su conservación e insumo para el ordenamiento pesquero del Golfo de Urabá, en el noroeste de Colombia. Usando redes de enmalle se capturaron 320 individuos, de los que se obtuvieron 208 estómagos con contenido. El espectro alimenticio de la especie está integrado por 26 ítems, pertenecientes a tres grandes grupos taxonómicos: peces, crustáceos y moluscos. Entre estos, los engráulidos (Engraulidae) son el principal componente en todos los sitios de muestreo, sobresaliendo Centengraulis edentulus y varias especies del género Anchoa. Se encontró un traslape de presas entre los sitios, siendo evidente la importancia de las áreas de manglar para alimentación. Sin embargo, no hubo variación estacional ni diferencias entre sexos en la composición de la dieta, dado que los engráulidos, su principal presa, son uno de los recursos pesqueros más abundantes en el Caribe colombiano

    3D printing for surgical planning in bone grafts for cleft-palate: a case report

    3D printing has been used for teaching purposes. Creating models for simulating surgeries. Gong et al presented a workflow for digital planning for surgery and Rendon et al previously presented a low-cost method with acceptable precision.  This paper aims to present a case where 3D printing surgical planning was applied for bone graft shape and dimensions. A 16-year-old female patient with a history of bilateral cleft lip and palate has received 6 surgical interventions to treat her congenital pathology. She begins orthodontic management at the age of 6 years and is referred to the plastic surgery service 10 years later, presenting oral and nasal fistula on the nasal floor with mainly liquid leakage. A bone graft was taken and applied from the patient's left iliac crest. the iliac crest is taken and the bone graft is molded Assisted with 3D printed model which is fixed in the premaxilla with a 14-hole linear plate. We proposed a new application for low-cost 3D printed models. Patient specific models have applications in cleft palate bone grafting. We present a case and more studies are required to measure variables as time, graft integration, and patient satisfaction

    Use of Amniotic Membrane Transplant and Deep Anterior Keratoplasty in a Patient with Bilateral Acanthamoeba Infectious Keratitis: A Case Report

    The aim of this study was to report a case with the use of amniotic membrane transplant and deep anterior keratoplasty in a patient with bilateral Acanthamoeba infectious keratitis as a treatment. A 20-year-old male presented with bilateral Acanthamoeba keratitis (AK) who was initially diagnosed with herpetic keratitis receiving full antiviral and corticosteroid topical treatment without any improvement. Corneal biopsy was performed to confirm the suspected diagnosis, and Acanthamoeba stromal cysts were identified in the sample. Treatment was initiated with 0.02% chlorhexidine, 0.1% propamidine isethionate, neomycin, and tropicamide/phenylephrine. Symptoms and clinical improvement were achieved between the 8th and 10th weeks, so corticosteroids were initiated. Treatment was continued until we observed a poor response in the left eye; therefore, an epithelial scraping and amniotic membrane placement were performed. Lately, the right eye underwent a deep anterior lamellar keratoplasty. A challenging case of bilateral AK managed with topical medications, amniotic membrane, and corneal keratoplasty. The earlier the disease is diagnosed, the better the outcome. If the diagnosis is delayed, the amoebas have penetrated deep into the corneal stroma, and successful therapy becomes difficult. A surgical option can be an early solution with a good prognosis for these cases

    Combined technique as first approach in mechanical thrombectomy: Efficacy and safety of REACT catheter combined with stent retriever

Here, we evaluated the effectiveness and safety of the REACT aspiration catheter used with a stent retriever. Methods This prospective study included consecutive adult patients who underwent MT with a combined technique using REACT 68 and/or 71 between June 2020 and July 2021. The primary endpoints were final and first pass mTICI 2b-3 and mTICI 2c-3 recanalization. Analysis was performed after first pass and after each attempt. Secondary safety outcomes included procedural complications, symptomatic intracranial hemorrhage (sICH) at 24 h, in-hospital mortality, and 90-day functional independence (modified Rankin Scale [mRS] 0–2). Results A total of 102 patients were included (median age 78; IQR: 73–87; 50.0% female). At baseline, median NIHSS score was 19 (IQR: 11–21), and ASPECTS was 9 (IQR: 8–10). Final mTICI 2b-3 recanalization was achieved in 91 (89.2%) patients and mTICI 2c-3 was achieved in 66 (64.7%). At first pass, mTICI 2b-3 was achieved in 55 (53.9%) patients, and mTICI 2c-3 in 37 (36.3%). The rate of procedural complications was 3.9% (4/102), sICH was 6.8% (7/102), in-hospital mortality was 12.7% (13/102), and 90-day functional independence was 35.6% (36/102). Conclusion A combined MT technique using a stent retriever and REACT catheter resulted in a high rate of successful recanalization and first pass recanalization in a sample of consecutive patients with AIS due to LVO in clinical use
