    Risk Factors for Positive Deep Pelvic Nodal Involvement in Patients with Palpable Groin Melanoma Metastases: Can the Extent of Surgery be Safely Minimized?: A Retrospective, Multicenter Cohort Study

    Background: Patients with palpable melanoma groin metastases have a poor prognosis. There is debate whether a combined superficial and deep groin dissection (CGD) is necessary or if superficial groin dissection (SGD) alone is sufficient. Aim: The aim of this study was to analyze risk factors for deep pelvic nodal involvement in a retrospective, multicenter cohort of palpable groin melanoma metastases. This could aid in the development of an algorithm for selective surgery in the future. Methods: This study related to 209 therapeutic CGDs from four tertiary centers in The Netherlands (1992–2013), selected based on complete preoperative imaging and pathology reports. Analyzed risk factors included baseline and primary tumor characteristics, total and positive number of inguinal nodes, inguinal lymph node ratio (LNR) and positive deep pelvic nodes on imaging (computed tomography [CT] ± positron emission tomography [PET], or PET − low-dose CT). Results: Median age was 57 years, 54 % of patients were female, and median follow-up was 21 months (interquartile range [IQR] 11–46 months). Median Breslow thickness was 2.10 mm (IQR 1.

    La alteración de la salud mental constituye un problema socio-sanitario. Las ciencias de la salud consideran modelos de causalidad multifactoriales con determinantes sociales. No se observa en esta investigación la inclusión de la Ciencia Administrativa como interdisciplina. Su incorporación permitiría integrar conocimientos administrativos para un eficaz y eficiente desempeño en las organizaciones de salud mental. Las fallas de planificación como proceso central en la formulación de estrategias constituyen un común denominador de los modelos descriptos. Los resultados encontrados, como indicadores del valor giro programa, 5 años de permanencia (rango 1 – 20); y producto del análisis cualitativo (respuestas a entrevistas) 6 años (máxima permanencia 26-27 años) afirman un bajo rendimiento de los programas existentes. Se observó contradicción en los modelos para compatibilizar el control del rendimiento con la autonomía de las diferentes divisiones, sistemas cerrados instrumentales de la sede central, responsables de las actividades dependientes de más de un coordinador con desempeño simultáneo en subproyectos sin conseguir resultados aceptables. La propuesta del nuevo modelo de gestión es una intervención de tipo social fragmentaria, sistematizada e incremental. Propone un rediseño de los procesos con la revisión de los recursos disponibles para alcanzar aumento del valor giro-paciente para la externación con reinserción social.Facultad de Ciencias Económica

    Quality of treatment and surgical approach for rectal gastrointestinal stromal tumour (GIST) in a large European cohort

    Background: Rectal gastrointestinal stromal tumours (GISTs) are rare tumours. Variability in the managementmay influence outcome, but there is a lack of understanding regarding contemporary variancein care. A multicenter, international, retrospective cohort study was performed to elucidate characteristicsand outcomes of rectal GIST in European practice, with particular reference to surgical approach.Methods: All rectal GIST patients diagnosed between 2009 and 2018 were identified from five Europeandatabases. Recurrence free survival (RFS) and overall survival (OS) were estimated using Kaplan-Meiermethod. Possible confounders were identified using Cox regression analyses.Results: From 210 patients, 155 patients had surgery. The three main types of surgery were local tumourresection (LTR, n ¼ 46), low anterior resection (LAR, n ¼ 31) and abdomino-perineal resection (APR,n ¼ 32). Most patients received neoadjuvant (65%) and/or adjuvant imatinib therapy (66%). Localrecurrence rate after surgery was 15% and overall recurrence rate 28%. No significant differences werefound in terms of RFS nor OS between LTR, LAR and APR. However, locally resected tumours weresmaller, while LAR and APR patients more often received perioperative imatinib. General hospitalstreated smaller GISTs, offered imatinib less frequently, and had a higher tumour rupture rate. In the multivariate analysis in the group having LTR, APR or LAR, the only significant prognostic factor for localrecurrence was higher age (HR 1.06, CI 1.00e1.12, p ¼ 0.048).Conclusions: In European clinical practice for rectal GIST, LTR, LAR and APR have comparable localcontrol. Multimodal approach is higher and tumour rupture less frequent in specialist centres comparedto general hospitals.Experimentele farmacotherapi
