Carbohydrate intake and cardiometabolic risk factors in high BMI African American children.
The aim of this study was to evaluate the relationship between intakes of subgroups of energy-providing carbohydrate, and markers of cardiometabolic risk factors in high BMI African American (AA) children.A cross sectional analysis was performed on data from a sample of 9-11 year old children (n = 95) with BMI greater than the 85th percentile. Fasting hematological and biochemical values for selected markers of cardiometabolic risk factors were related to intakes of carbohydrates and sugars.After adjusting for gender, pubertal stage and waist circumference, multivariate regression analysis showed that higher intakes of carbohydrate (with fat and protein held constant) were associated with higher plasma concentrations of triglycerides (TG), VLDL-C, IDL-C, and worse insulin resistance (homeostasis model assessment of insulin resistance, HOMA-IR). After dividing carbohydrate into non-sugar versus sugar fractions, sugars were significantly related to higher TG, VLDL-C, IDL-C, lower adipocyte fatty acid insulin sensitivity (ISI-FFA), and was closely associated with increased HOMA-IR. Similar trends were observed for sugars classified as added sugars, and for sugars included in beverages. Further dividing sugar according to the food group from which it was consumed showed that consuming more sugar from the candy/soda food group was highly significantly associated with increased TG, VLDL-C, IDL-C and closely associated with increased HOMA-IR. Sugars consumed in all fruit-containing foods were significantly associated with lower ISI-FFA. Sugars consumed as fruit beverages was significantly associated with VLDL-C, IDL-C and ISI-FFA whereas sugars consumed as fresh, dried and preserved fruits did not show significant associations with these markers.Sugars consumed from in all dairy foods were significantly associated with higher TG, VLDL-C and IDL-C, and with significantly lower HDL-C and ISI-FFA. These effects were associated with sugars consumed in sweetened dairy products, but not with sugars consumed in unsweetened dairy products. This analysis suggests that increases in carbohydrate energy, especially in the form of sugar, may be detrimental to cardiometabolic health in high BMI children

Assessment of Pesticide Use in Major Vegetables From Farmers\u27 Perception and Knowledge in Dhading District, Nepal
A field study was carried out to assess the pesticide use status in major vegetable crops from farmers\u27 perception and knowledge in Dhading, Nepal in 2019. Field study was carried with 100 commercial farmer\u27s using semi-structure questionnaire by face to face interview. This study was analyzed by categorization of farmers into small holder (51) and large holder (49) groups on the basis of mean area of vegetable cultivation (6.48 ropani). The highest amount of pesticides is needed in tomato in both large holders and small holders according to the farmer\u27s experience. Among the study farmer\u27s, 41% of them spray the pesticides by making a cocktail or mixed method and 56% follow the waiting period of 3-5 days in both of the cases. A significant positive correlation was found at 5% level of significance between the knowledge and practice pattern of waiting period of the pesticides and negative correlation was found between the Personal Protective equipment score and health hazard score. Headache was the major health hazards faced by the farmers which was higher in small holders (66.7%) as compared to the large holders (46.9%). Mask was the most used PPE by the farmer\u27s i.e. by 83% in overall. Fourty three percent of the farmer\u27s throw the pesticide containers in secret place after using of it.The use of PPE was seen lower in small holders as compared to the large holders. This study reveals the necessities of suitable program and policies regarding the knowledge, safe handling and use of pesticide among the farmer\u27s level
Satellite Image Classification using Clustering Algorithms with Edge Detection Operators
Image classification consists of image processing algorithms for grouping cells of similar characteristics together. Satellite image classification is essential to extract the information and identify the different components such as water dense region, roads, vegetation etc. from the classified image. In this paper, an attempt is made to locate and identify the different regions of interest using classification algorithms such as K means and Fuzzy-C Means. Comparison is done for both the algorithms in terms of computational time and memory requirements. Also, the algorithms are applied for the input image by considering different values of K and its discussion is presented in the paper. The algorithms are then applied for the given image with edge detection operators to obtain the better visual clarity of the edges
Stand structure of a primate rich rainforest region in the central Western Ghats of Southern India
The Western Ghats of southern India are one of the most important biodiversity regions in the world, not only due to their faunal diversity and abundance but also due to different habitat types, floral diversity and the presence of several endemic plant species. The rainforests in the central Western Ghats are inhabited by several primate species. We investigated the vegetation pattern and tree species occupancy of one of the prime primate habitats in the central Western Ghats. Lion-tailed Macaque (Macaca silenus), Bonnet Macaque (Macaca radiata), Hanuman Langur (Semnopithecus entellus achates) and Malabar Slender Loris (Loris lydekkerianus malabaricus) inhabit the study area. We studied the density, dominance, frequency and Importance Value Index (IVI) of different tree species, using the belt transect method on randomly selected plots covering 4.1ha. We found that all the plant species that emerged to be the most dominant species with high IVI in the forest were also used by the diurnal primates for foraging. Knema attenuata and Syzygium gardneri were found to be the 'keystone' species. Since the forests of the study area do not come under the 'protected area network' for wildlife, the data obtained during this study will be helpful in the forestry management practices with a view for wildlife conservation of the region
Surgical management of fractures of distal end radius using uniplanar external fixator augmented with percutaneous kirschner wire fixation
Background: Distal end radius fractures is one of the most common fractures of the upper limb especially in the elderly population, accounting for about 17% of all upper limb fractures. Surgical stabilization of these fractures remains a challenge even today. Although the recent trend is towards internal fixation with locking plates, the external fixator itself has its own advantages in the treatment of these fractures.Methods: This study is a prospective, time bound, hospital based study conducted in Kempegowda Institute of Medical Sciences and Research Center, Bangalore, between November 2014 to April 2016. The study included 30 cases of distal end radius fractures that were operated with the closed reduction and uniplanar external fixator augmented with k-wire for distal end radius by the principle of ligamentotaxis.Results: In our study, 14 (46.6 %) patients had excellent results. Whereas, 11 (36.7%) patients had good results and 3 (10%) had fair and only 2 (6.7%) patients had poor results. Most of the fractures united by 12 weeks. Complications associated with the study was stiffness, malunion, sudeck’s osteodystrophy and pin tract infection. Conclusions: The uniplanar external fixator augmented with k-wire is a good choice in the treatment of distal end radius fractures in terms of providing a good functional outcome if proper preoperative planning, good reduction and surgical technique are followed, leading to high rate of bone union, minimal soft tissue damage and complications
Prescribing pattern of antipsychotic medications in patients with schizophrenia in a tertiary care hospital
Background: The objective of thisstudy was to study the prescribing pattern of antipsychotic medications in patients with schizophrenia in a tertiary care hospital.Methods: An observational study was conducted in psychiatry outpatient department (OPD) of Bapuji and Chigateri hospital attached to Jagadguru Jayadeva Murugarajendra medical college, Davangere. The data collected, had information about age, gender, education, occupation, marital status and drug prescription that included trade name, generic name, dosage and frequency of administration of 100 outpatients who attended the psychiatry OPD.Results: A total of 100 prescriptions were analyzed of which 6% was monotherapy, where Asenapine was the only drug used and rest 94% was combination therapy. About 94% of the combination therapy included the use of single antipsychotic with anticholinergic drug and multiple antipsychotics with anticholinergic drug. A combination of multiple antipsychotics with anticholinergic was most commonly used, that is trifluoperazine and chlorpromazine (36%), followed by trifluoperazine and risperidone (13%), trifluoperazine and quetiapine (8%), risperidone and quetiapine (7%), trifluoperazine, chlorpromazine along with quetiapine (3%). Among single antipsychotic with anticholinergic, risperidone (20%) was most commonly used, followed by Trifluoperazine (7%). Use of the anticholinergic drug, Trihexyphenidyl accounts to 94% of the total prescriptions.Conclusions: Schizophrenia is mostly seen in males, middle age group and unemployed people. The present study showed that combination therapy is preferred for the treatment of Schizophrenia. Despite several side-effects, typical antipsychotics, especially trifluoperazine was the most commonly used drug, followed by chlorpromazine either alone or in combination. Among atypical antipsychotics, risperidone was commonly used followed by quetiapine and asenapine. Most of the patients received trihexyphenidyl, an anticholinergic drug along with antipsychotics to reduce extra pyramidal side-effects
Prescribing pattern of antidepressants in psychiatric unit of a tertiary care hospital
Background: The objective was to study the prescribing pattern of antidepressants in psychiatric unit of a tertiary care hospital.Methods: An observational study was carried out at psychiatry out-patient department (OPD). The data which were collected included information about age, gender, education, occupation, marital status and drug prescription included trade name, generic name, dosage, and frequency of 100 outpatients who attended the psychiatry OPD.Results: Among 100Â patients with major depression 66% were females and 34% were males. Depression was more commonly seen between patients with age group 21-40Â years. Depression was more common among housewives (44%) and next was students (18%). Percentage of depression was more in educated people with (72%) than in uneducated with (28%). Depression was more commonly seen in married people with (77%) than in unmarried people (23%). Most commonly prescribed antidepressant as monotherapy was fluoxetine and as combination therapy was fluoxetine and escitalopram.Conclusions: Depression is more commonly seen in married people predominantly in females and housewives. Fluoxetine is more commonly used followed by escitalopram. Selective serotonin reuptake inhibitors are preferred over other antidepressant because of their relative lesser side effects
Macronutrient intakes and cardio metabolic risk factors in high BMI African American children
<p>Abstract</p> <p>Background</p> <p>The aim of this study was to evaluate the relationship between intakes of energy-providing macronutrients, and markers of cardio metabolic risk factors in high BMI African American (AA) children.</p> <p>Methods</p> <p>A cross sectional analysis of a sample of 9-11 year old children (n = 80) with BMI greater then the 85<sup>th </sup>percentile. Fasting hematological and biochemical measurements, and blood pressure were measured as selected markers of cardio metabolic risk factors and their relationships to dietary intakes determined.</p> <p>Results</p> <p>After adjusting for gender, pubertal stage and waist circumference (WC), multivariate regression analysis showed that higher total energy intakes (when unadjusted for source of energy) were associated with higher plasma concentrations of intermediate density lipoprotein cholesterol (IDL-C) and very low density lipoprotein cholesterol (VLDL-C). Higher intakes of carbohydrate energy (fat and protein held constant) were associated with higher IDL-C, VLDL-C, triglycerides (TG) and homeostasis model assessment of insulin resistance (HOMA-IR). Higher intakes of fat (carbohydrate and protein held constant), however, were associated with lower IDL-C; and higher protein intakes (fat and carbohydrate held constant) were associated with lower HOMA-IR.</p> <p>Conclusion</p> <p>The specific macronutrients that contribute energy are significantly associated with a wide range of cardio metabolic risk factors in high BMI AA children. Increases in carbohydrate energy were associated with undesirable effects including increases in several classes of plasma lipids and HOMA-IR. Increases in protein energy were associated with the desirable effect of reduced HOMA-IR, and fat energy intakes were associated with the desirable effect of reduced IDL-C. This analysis suggests that the effect of increased energy on risk of developing cardio metabolic risk factors is influenced by the source of that energy.</p
