    Bi-directional Photogoniometer for the Assessment of the Luminous Properties of Fenestration Systems

    Most energy saving applications of advanced fenestration systems (solar blinds, novel types of glazing and daylight redirecting devices) require a precise knowledge of their directional light transmission features. These photometric properties can be described by a Bi-directional Transmission Distribution Function (BTDF) whose experimental assessment requires appropriate equipment. A novel bi-directional transmission photogoniometer, based on digital imaging techniques, was designed and set up for that purpose. The apparatus takes advantage of a modern video image capturing device (CCD digital camera) as well as of powerful image analysis software (pattern recognition) to considerably reduce the scanning time of a BTDF measurement, in comparison to existing devices that use a conventional approach (mobile photometer). A detailed calibration and validation procedure was used to obtain optimal experimental accuracy for the device during the assessment of BTDF data. It included a spectral, a photometric and a geometrical calibration of the digital video system, as well as several additional corrections, leading to an overall relative accuracy better than 11% for BTDF data. A special effort was made to improve the user-friendliness of BTDF measurement by facilitating the data acquisition and treatment (definition of a data acquisition and electronic data format) and by offering different possibilities of BTDF visualisation (hemispherical representation, axonometric view of photometric solids, C-planes). Overall, the photometric equipment was used to assess the BTDFs of more than 20 novel fenestration products of the industrial partner of the project (Baumann-Hüppe Storen AG). The experimental data produced was successfully used by the company to optimise the visual and energy saving performance of their products, which confirms the adequacy of the novel bi-directional photogoniometer for practical building applications

    The concept of solidarity: emerging from the theoretical shadows?

    The concept of solidarity has been relatively neglected by social scientists since Durkheim's pioneering work in the late 19th century. The discipline of politics has been guilty of overlooking this 'subjective' element of community life, but recent works by Stjernø and Brunkhorst reflect a growing awareness of the theoretical significance of the concept. Whereas early liberal attempts to theorise solidarity took the nation state to be the appropriate community for its realisation, the emergence of globalisation raises the possibility of human solidarity developing in the global community. Traditional forms of solidarity have been dissipated by the social changes accompanying globalisation, but they were often locked into the defence of particular interests. New forms may be emerging to rekindle the broader vision of human solidarity. Recent work by writers such as Habermas, Honneth, Rorty and Touraine focuses on widening and deepening democratic participation and/or the articulation of our ethical obligations in various ways. It is argued here that these perspectives need to be supplemented by a radical humanist approach grounded in a normative theory of human self-realisation

    Civil Society Activism in Italy Across Different Fields: A Multifaceted Picture of Solidarity in Hard Times

    Over the last years, Italian civil society organisations have been working on a daily basis to mitigate the impact of both the global economic crisis and the refugee crisis, which have increased social vulnerabilities. Relying on the data gathered through 30 in-depth interviews with transnational solidarity organisations’ representatives, this chapter analyses solidarity practices in three fields of activity: disability, unemployment and migration. Results show that solidarity attitudes, practices and discourses are strongly influenced by the policy domain in which the organisations are active. Furthermore, the crisis led organisations to search for new strategies and approaches, even though it has been an ineffective vector of transnationalisation due to lack of resources, and the necessity to cope with pressing needs at national and local level/s.Results presented in book have been obtained through the project ‘European paths to transnational solidarity at times of crisis: Conditions, forms, role-models and policy responses’ (TransSOL).     The nephrology eHealth-system of the metropolitan region of Hannover for digitalization of care, establishment of decision support systems and analysis of health care quality

    Background Even though a high demand for sector spanning communication exists, so far no eHealth platform for nephrology is established within Germany. This leads to insufficient communication between medical providers and therefore suboptimal nephrologic care. In addition, Clinical Decision Support Systems have not been used in Nephrology until now. Methods The aim of NEPHRO-DIGITAL is to create a eHealth platform in the Hannover region that facilitates integrated, cross-sectoral data exchange and includes teleconsultation between outpatient nephrology, primary care, pediatricians and nephrology clinics to reduce communication deficits and prevent data loss, and to enable the creation and implementation of an interoperable clinical decision support system. This system will be based on input data from multiple sources for early identification of patients with cardiovascular comorbidity and progression of renal insufficiency. Especially patients will be able to enter and access their own data. A transfer to a second nephrology center (metropolitan region of Erlangen-Nuremburg) is included in the study to prove feasibility and scalability of the approach. Discussion A decision support system should lead to earlier therapeutic interventions and thereby improve the prognosis of patients as well as their treatment satisfaction and quality of life. The system will be integrated in the data integration centres of two large German university medicine consortia (HiGHmed (highmed.org) and MIRACUM (miracum.org)).     RF Sources for the ITER Ion Cyclotron Heating and Current Drive System

    The RF source requirements for the ITER ion cyclotron (IC) heating and current drive system are very challenging ? 20 MW CW power into an antenna load with a VSWR of up to 2 over the frequency range of 35-65 MHz. For the two present antenna designs under consideration, 8 sources providing 2.5 MW each are to be employed. For these sources, the outputs of two final power amplifiers (FPAs), using the high power CPI 4CM2500KG tube, are combined with a 180? hybrid combiner to easily meet the ITER IC source requirements ? 2.5 MW is supplied at a VSWR of 2 at ? 70% of the maximum tube power available in class B operation. The cylindrical cavity configuration for the FPAs is quite compact so that the 8 combined sources fit into the space allocated at the ITER site with room to spare. The source configuration is described in detail and its projected operating power curves are presented. Although the CPI tube has been shown to be stable under high power operating conditions on many facilities, a test of the combined FPA source arrangement is in preparation using existing high power 30 MHz amplifiers to assure that this configuration can be made robustly stable for all phases at a VSWR up to 2. The possibility of using 12 sources to feed a suitably modified antenna design is also discussed in the context of providing flexibility for specifying the final IC antenna design

    Practice guidelines are systematically developed statements and recommendations that assist the physicians and patients in making decisions about appropriate health care measures for specific clinical circumstances taking into account specific national health care structures. The 1st revision of the S-2k guideline of the German Sepsis Society in collaboration with 17 German medical scientific societies and one self-help group provides state-of-the-art information (results of controlled clinical trials and expert knowledge) on the effective and appropriate medical care (prevention, diagnosis, therapy and follow-up care) of critically ill patients with severe sepsis or septic shock. The guideline had been developed according to the “German Instrument for Methodological Guideline Appraisal” of the Association of the Scientific Medical Societies (AWMF). In view of the inevitable advancements in scientific knowledge and technical expertise, revisions, updates and amendments must be periodically initiated. The guideline recommendations may not be applied under all circumstances. It rests with the clinician to decide whether a certain recommendation should be adopted or not, taking into consideration the unique set of clinical facts presented in connection with each individual patient as well as the available resources

    Use of IFN gamma/IL10 Ratio for Stratification of Hydrocortisone Therapy in Patients With Septic Shock

    Large clinical trials testing hydrocortisone therapy in septic shock have produced conflicting results. Subgroups may benefit of hydrocortisone treatment depending on their individual immune response. We performed an exploratory analysis of the database from the international randomized controlled clinical trial Corticosteroid Therapy of Septic Shock (CORTICUS) employing machine learning to a panel of 137 variables collected from the Berlin subcohort comprising 83 patients including demographic and clinical measures, organ failure scores, leukocyte counts and levels of circulating cytokines. The identified theranostic marker was validated against data from a cohort of the Hellenic Sepsis Study Group (HSSG) (n = 246), patients enrolled in the clinical trial of Sodium Selenite and Procalcitonin Guided Antimicrobial Therapy in Severe Sepsis (SISPCT, n = 118), and another, smaller clinical trial (Crossover study, n = 20). In addition, in vitro blood culture experiments and in vivo experiments in mouse models were performed to assess biological plausibility. A low serum IFNγ/IL10 ratio predicted increased survival in the hydrocortisone group whereas a high ratio predicted better survival in the placebo group. Using this marker for a decision rule, we applied it to three validation sets and observed the same trend. Experimental studies in vitro revealed that IFNγ/IL10 was negatively associated with the load of (heat inactivated) pathogens in spiked human blood and in septic mouse models. Accordingly, an in silico analysis of published IFNγ and IL10 values in bacteremic and non-bacteremic patients with the Systemic Inflammatory Response Syndrome supported this association between the ratio and pathogen burden. We propose IFNγ/IL10 as a molecular marker supporting the decision to administer hydrocortisone to patients in septic shock. Prospective clinical studies are necessary and standard operating procedures need to be implemented, particularly to define a generic threshold. If confirmed, IFNγ/IL10 may become a suitable theranostic marker for an urging clinical need
