    Eurorack to VCV Rack: Modular Synthesis as Compositional Performance

    An auto-ethnographic approach is applied to explore the practice of specific patching techniques in post-digital modular synthesis. This is conceptualised as a performance, an embodied approach to creative musical synthesis that emphases the sound and touch over the visual dominance of the currently ubiquitous computer screen. Music performance includes the re-assemblage and re-configuration of a hardware modular system, in this case Eurorack, hereby differentiating it from its virtual version, VCV Rack

    Stop, think SCORTCH: rethinking the traditional 'TORCH' screen in an era of re-emerging syphilis

    BACKGROUND: The epidemiology of congenital infections is ever changing, with a recent resurgence in syphilis infection rates seen in the UK. Identification of congenital infection is often delayed; early recognition and management of congenital infections is important. Testing modalities and investigations are often limited, leading to missed diagnostic opportunities. METHODS: The SCORTCH (syphilis, cytomegalovirus (CMV), 'other', rubella, toxoplasmosis, chickenpox, herpes simplex virus (HSV) and blood-borne viruses) acronym increases the awareness of clinicians to the increased risk of congenital syphilis, while considering other infectious aetiologies including: zika, malaria, chagas disease, parvovirus, enterovirus, HIV, hepatitis B and C, and human T-lymphotropic virus 1, in addition to the classic congenital infections recognised in the 'TORCH screen' (toxoplasmosis, 'other', rubella, CMV, HSV). The SCORTCH diagnostic approach describes common signs present in infants with congenital infection, details serological testing for mother and infant and important direct diagnostics of the infant. Direct diagnostic investigations include: radiology, ophthalmology, audiology, microbiological and PCR testing for both the infant and placental tissue, the latter also warrants histopathology. CONCLUSION: The traditional 'TORCH screen' focuses on serology-specific investigations, often omits important direct diagnostic testing of the infant, and fails to consider emerging and re-emerging congenital infections. In recognition of syphilis as a re-emerging pathogen and the overlapping clinical presentations of various infectious aetiologies, we advocate for a broader outlook using the SCORTCH diagnostic approach

    Late-onset bloodstream infection and perturbed maturation of the gastrointestinal microbiota in premature infants

    Late-onset bloodstream infection (LO-BSI) is a common complication of prematurity, and lack of timely diagnosis and treatment can have life-threatening consequences. We sought to identify clinical characteristics and microbial signatures in the gastrointestinal microbiota preceding diagnosis of LO-BSI in premature infants.Daily faecal samples and clinical data were collected over two years from 369 premature neonates (<32 weeks gestation). We analysed samples from 22 neonates who developed LO-BSI and 44 matched control infants. Next-generation sequencing of 16S rRNA gene regions amplified by PCR from total faecal DNA was used to characterise the microbiota of faecal samples preceding diagnosis from infants with LO-BSI and controls. Culture of selected samples was undertaken, and bacterial isolates identified using MALDI-TOF. Antibiograms from bloodstream and faecal isolates were compared to explore strain similarity.From the week prior to diagnosis, infants with LO-BSI had higher proportions of faecal aerobes/facultative anaerobes compared to controls. Risk factors for LO-BSI were identified by multivariate analysis. Enterobacteriaceal sepsis was associated with antecedent multiple lines, low birth weight and a faecal microbiota with prominent Enterobacteriaceae. Staphylococcal sepsis was associated with Staphylococcus OTU faecal over-abundance, and the number of days prior to diagnosis of mechanical ventilation and of the presence of centrally-placed lines. In 12 cases, the antibiogram of the bloodstream isolate matched that of a component of the faecal microbiota in the sample collected closest to diagnosis.The gastrointestinal tract is an important reservoir for LO-BSI organisms, pathogens translocating across the epithelial barrier. LO-BSI is associated with an aberrant microbiota, with abundant staphylococci and Enterobacteriaceae and a failure to mature towards predominance of obligate anaerobes

    An Exploration of the Metaphors and Images Used to Describe Leadership in Two Different Cultural Contexts

    © Copyright © 2020 Randell and Yerbury. This paper is developed from qualitative data exploring the metaphors used to describe women’s leadership in differing cultural contexts. Metaphors are a useful communication tool, allowing us to understand an idea or concept through some other phenomenon. Understandably, studies of metaphor tend to focus on metaphors deriving from the English language and from Western cultures. Our everyday language literature abounds with metaphors that evoke images of the masculine – including of machines, war and fighting, competition, games and sport. Leadership is generally thought of as “a good thing,” as something important, carried out by people with desirable attributes, such as courage and insight, or with attractive personalities and good communication skills. Metaphors used in discussing women’s leadership in many countries may support this approach, but they do so by highlighting the obstacles women face, for example, the glass ceiling, glass cliff, sticky floors, and the labyrinth. This study attempts to break the mold, investigating the understanding of women’s leadership as expressed in metaphors that is contextualized differently across the continents. Taking an interpretive approach, this study seeks to present leadership through the understanding of female leaders involved in the field of education broadly defined, from Rwanda and Bangladesh, who gave accounts of their metaphorical conceptualisation of leadership. Using narrative analysis, these accounts were analyzed to identify and interpret the metaphors emerging from descriptions of leadership experiences. The analysis shows that these leaders used metaphors determined by a dynamic interplay of personal, situational, and cultural factors. Somewhat surprisingly, the metaphors used to present leadership are not always based on a local cultural context, with some explicitly including references to European plants, which could be considered exotic. However, they are not metaphors that fit neatly into the taxonomies recognized in the literature. This study makes an important contribution to the literature on leadership, especially leadership in education. It is one of the few studies focussing on the stories of women from a non-Western background. The pool of case studies presented here is limited, but these metaphors represent thinking tools enabling a scholarly and imaginative understanding of women’s leadership in education

    Challenges Facing Airway Epithelial Cell-Based Therapy for Cystic Fibrosis

    Mutations in the cystic fibrosis transmembrane conductance regulator (CFTR) gene cause the life-limiting hereditary disease, cystic fibrosis (CF). Decreased or absent functional CFTR protein in airway epithelial cells leads to abnormally viscous mucus and impaired mucociliary transport, resulting in bacterial infections and inflammation causing progressive lung damage. There are more than 2000 known variants in the CFTR gene. A subset of CF individuals with specific CFTR mutations qualify for pharmacotherapies of variable efficacy. These drugs, termed CFTR modulators, address key defects in protein folding, trafficking, abundance, and function at the apical cell membrane resulting from specific CFTR mutations. However, some CFTR mutations result in little or no CFTR mRNA or protein expression for which a pharmaceutical strategy is more challenging and remote. One approach to rescue CFTR function in the airway epithelium is to replace cells that carry a mutant CFTR sequence with cells that express a normal copy of the gene. Cell-based therapy theoretically has the potential to serve as a one-time cure for CF lung disease regardless of the causative CFTR mutation. In this review, we explore major challenges and recent progress toward this ambitious goal. The ideal therapeutic cell would: (1) be autologous to avoid the complications of rejection and immune-suppression; (2) be safely modified to express functional CFTR; (3) be expandable ex vivo to generate sufficient cell quantities to restore CFTR function; and (4) have the capacity to engraft, proliferate and persist long-term in recipient airways without complications. Herein, we explore human bronchial epithelial cells (HBECs) and induced pluripotent stem cells (iPSCs) as candidate cell therapies for CF and explore the challenges facing their delivery to the human airway

    We prove the minimality of the CW-complex structure for complements of hyperplane arrangements in Cn\mathbb C^n by using the theory of Lefschetz pencils and results on the variation maps within a pencil of hyperplanes. This also provides a method to compute the Betti numbers of complements of arrangements via global polar invariants

    Self-repair ability of evolved self-assembling systems in cellular automata

    Self-repairing systems are those that are able to reconfigure themselves following disruptions to bring them back into a defined normal state. In this paper we explore the self-repair ability of some cellular automata-like systems, which differ from classical cellular automata by the introduction of a local diffusion process inspired by chemical signalling processes in biological development. The update rules in these systems are evolved using genetic programming to self-assemble towards a target pattern. In particular, we demonstrate that once the update rules have been evolved for self-assembly, many of those update rules also provide a self-repair ability without any additional evolutionary process aimed specifically at self-repair
