PT.

Systematic associations between germ-line mutations and human cancers

YesThe revolution in Big Data has opened the gate for new research challenges in biomedical science. The aim of this study was to investigate whether germ-line gene mutations are a significant factor in 29 major primary human cancers. Using data obtained from multiple biological databases, we identified 424 genes from 8879 cancer mutation records. By integrating these gene mutation records a human cancer map was constructed from which several key results were obtained. These include the observations that missense/nonsense and regulatory mutations might play central role in connecting cancers/genes, and tend to be distributed in all chromosomes. This suggests that, of all mutation classes missense/nonsense and regulatory mutation classes are over-expressed in human genome and so are likely to have a significant impact on human cancer aetiology and pathomechanism. This offers new insights into how the distribution and interconnections of gene mutations influence the development of cancers

Robotic partial nephrectomy for posterior tumors through a retroperitoneal approach offers decreased length of stay compared with the transperitoneal approach: A propensity-matched analysis

INTRODUCTION: We sought to compare surgical outcomes between transperitoneal and retroperitoneal robotic partial nephrectomy (RPN) for posterior tumors. PATIENTS AND METHODS: Using our multi-institutional RPN database, we reviewed 610 consecutive cases for posterior renal masses treated between 2007 and 2015. Primary outcomes were complications, operative time, length of stay (LOS), surgical margin status, and estimated glomerular filtration rate (eGFR) preservation. Secondary outcomes were estimated blood loss, warm ischemia time (WIT), disease recurrence, and disease-specific mortality. Due to significant differences in treatment year and tumor size between approaches, retroperitoneal cases were matched 1:4 to transperitoneal cases based on propensity scores using the greedy algorithm. Outcomes were compared between approaches using the chi-square and Mann-Whitney U tests. RESULTS: After matching, 296 transperitoneal and 74 retroperitoneal cases were available for analysis, and matched groups were well balanced in terms of treatment year, age, gender, race, American Society of Anesthesiologists physical status classification (ASA) score, body mass index, tumor laterality, tumor size, R.E.N.A.L. (radius, exophytic/endophytic properties, nearness of tumor to the collecting system or sinus, anterior/posterior, location relative to polar lines) score, and hilar location. Compared with transperitoneal, the retroperitoneal approach was associated with significantly shorter mean LOS (2.2 vs 2.6 days, p = 0.01), but longer mean WIT (21 vs 19 minutes, p = 0.01). Intraoperative (p = 0.35) and postoperative complications (p = 0.65), operative time (p = 0.93), positive margins (p = 1.0), and latest eGFR preservation (p = 0.25) were not significantly different between approaches. No differences were detected in the other outcomes. CONCLUSIONS: Among high-volume surgeons, transperitoneal and retroperitoneal RPN achieved similar outcomes for posterior renal masses, although with slight differences in LOS and WIT. Retroperitoneal RPN may be an effective option for the treatment of certain small posterior renal masses

Immigrant become an important issue because some state used it for atool to fight for their interest. Tanjungpinang UNHCR discriminativepolicy for the sake of the interest of the donor state resulted a loss inUNHCR independency in giving a humanitarian aid. This researchobjective is to know United States through UNHCR within RefugeeStatus Determination giving in Rumah Detensi Imigrasi PusatTanjungpinang. In knowing that interest, used a theory called realismtheory. The type of this research is mixed methods that analyzed theTanjungpinang UNHCR discriminative policy that favor theAfghanistan nationality immigrant in Rumah Detensi Imigrasi PusatTanjungpinang. The discriminative policy that occur is to favor anAfghanistan nationality immigrants in term of giving the refugeestatus determination. The obtained result by this research shows thatUNHCR discriminative policy are not to bring United States interestto reform their image, but more to fight for their other interest

Performance monitoring of a secant piled wall using distributed fiber optic strain sensing

    Get PDF
An optical fiber strain sensing technique, based on Brillouin Optical Time Domain Reflectometry (BOTDR), was used to monitor the performance of a secant pile wall subjected to multiple props during construction of an adjacent basement in London. Details of the installation of sensors as well as data processing are described. Distributed strain profiles were obtained by deriving strain measurements from optical fibers installed on opposite sides of the pile to allow monitoring of both axial and lateral movements along the pile. Methods for analyzing the thermal strain and temperature compensation are also presented. Measurements obtained from the BOTDR were found to be in good agreement with inclinometer data from the adjacent piles. The relative merits of the two different techniques are discussed
