Comparision of Hook Phlebectomy and Endovenous Laser Therapy for Below Knee Varicose Veins
Background: Varicose vein is a common surgical problem having multiple modalities available for their treatment. We studied the treatment modalities available for the below knee varicose veins comparing Endovenous Laser Therapy (EVLT) and Hook Phlebectomy at Acharya Vinoba Bhave Rural Hospital, Sawangi (Meghe), Wardha.Methods: All patients with varicose veins presenting to our hospital were included in the study. All the patients in our study were assessed by clinico-etiological anatomical and pathological (CEAP) classification. Diagnosis was further confirmed by Duplex ultrasonography.Results and Conclusions: of the 76 patients studied in this study, 15 patients underwent EVLT whereas 55 patients underwent Hook Phlebectomy for below knee varicose veins. 6 patients underwent EVLT for above knee varicose veins in combination with Hook Phlebectomy for below knee varicose veins.Conclusions: We find Hook Phlebectomy to be much better option than EVLT for below knee varicose veins
Minimizing the postoperative complications of severe hypospadias using a simple technique
Background The aim of this study was to decrease complication rates in proximal hypospadias surgery.Methods A simple method of stenting using a polypropylene stent has been developed for the most severe form of hypospadias during the period from January 2008 to January 2011 in the Department of Pediatric Surgery. The total number of patients was 46. The patients were classified into group 1 (n= 23), in which a polypropylene stent was used, and group 2 (n= 23), in which a polypropylene stent was not used.Results In group 1, complications occurred in three patients (13.04%), whereas in group 2 it occurred in 12 patients (52.2%). The difference in the total number of complications between groups was highly significant (P < 0.001). In group 1, no patient needed redo surgery, and in group 2 four patients (17.39%) needed redo surgery (P < 0.05). All other patients responded to repeated dilatation in the follow-up.Conclusion Although the sample size was small, this simple modification can decrease the complication rate significantly in the most severe form of hypospadias. Pediatric carcinoma of rectum - Varanasi experience
Background : Primary gastrointestinal system malignancies constitute
approximately 2% of pediatric neoplasm and of these; colorectal
carcinoma is the second most common malignancy. This is one of the
rarer diseases in children. Aim : We reviewed our records to study the
clinical features, outcome and the follow-up of this condition with
evaluation of the prognostic factors. Settings and Design: Tertiary
care Pediatric Surgery centre. Retrospective study. Materials and
Methods : The data of all patients with diagnosis of colorectal
carcinoma from January 1986 to January 2006 were reviewed. The
confirmation of the diagnosis was by biopsy from the lesion. The age,
sex, family history, clinical features, response to the treatment and
follow-up were studied. Results : There were four male patients. All
had bleeding per rectum as the presenting complaint. Three patients had
advanced disease at presentation. All the patients had signet cell
adenocarcinoma. The two patients expired and one was lost to follow-up.
One patient is alive after one year of follow up and is receiving
treatment. Conclusion : Colorectal cancer in children though rare can
be a reality, hence any children presenting with pain in abdomen along
with doubtful history of constipation and rectal bleeding should be
examined carefully with special emphasis on digital rectal examination
Double coin in esophagus at same location and same alignment - a rare occurrence: a case report
Coin is the most common foreign body swallowed by pediatric age group. The multiple coin swallowing is extremely rare and very few cases had been reported in English literature. Most of them were present at different site and had different alignment in the esophagus. The location of the coin (trachea vs. esophagus) is commonly determined by the alignment of the coin on radiographic studies. A 4-year-girl was presented to us with history of coin ingestions one day back without any respiratory distress. On radiological study there was suspicion of two coins on same location and alignment. The diagnosis was confirmed after removal. The both coin was removed successfully by esophagoscopy. Unexpected second foreign bodies in pediatric esophageal coin ingestions are rare and it is mandatory to do post operative radiography after removal to exclude duplex coin or tracheal coin. We are presenting this case because of its rarity, difficulty in diagnosis especially when proper history is not available
Single-Stage Repair versus Traditional Repair of High Anorectal Malformations, Functional Results’ Correlation with Kelly’s Score and Postoperative Magnetic Resonance Imaging Findings
Introduction: Posterior sagittal anorectoplasty (PSARP) is the standard treatment for anorectal malformations. In the present study, the clinical evaluation of anal continence was carried out using Kelly’s scoring system and the results of primary PSARP or abdomino-PSARP were compared with the traditional three-stage procedure and the functional outcome was correlated with the findings of MRI, which was used as an objective method of evaluation.Patients and methods: A total of 40 patients with intermediate and high anorectal malformations were studied and were divided into two groups on the basis of a random number table. The patients in group A were treated with a single-stage operation, whereas the patients in group B were treated with a standard staged operation (either PSARP or abdominoperineal pull-through). After clinical evaluation using the Kelly score, patients were divided into three clinical groups irrespective of whether they were operated in one stage or in three stages. All patients were subjected to MRI at the age of 3 years and the findings were correlated with the clinical scoring system.Result: Patients were categorized according to their Kelly’s scores as follows: group 1: clinically good (score 5–6); group 2: clinically fair (score 3–4); and group 3: clinically poor (score 0–2). The proportions of good development of the muscles (puborectalis, external sphincter muscle, and levator muscle hammock) were 78.9% in group 1, 40% in group 2, and none in group 3. Development of muscles was found to be a significant factor for anal continence. Other significant factors for anal continence are rectal diameter and anorectal angle.Conclusion: Clinical assessment using the Kelly score was similar for the single-stage operation and the staged procedure, and this was supported by MRI findings. Therefore, we recommend the single-stage procedure to achieve a better outcome in intermediate and high anorectal malformation.Keywords: Anorectal Malformations, MRI, Posterior Sagittal Anorectoplast
Order-Revealing Encryption and the Hardness of Private Learning
An order-revealing encryption scheme gives a public procedure by which two
ciphertexts can be compared to reveal the ordering of their underlying
plaintexts. We show how to use order-revealing encryption to separate
computationally efficient PAC learning from efficient -differentially private PAC learning. That is, we construct a concept
class that is efficiently PAC learnable, but for which every efficient learner
fails to be differentially private. This answers a question of Kasiviswanathan
et al. (FOCS '08, SIAM J. Comput. '11).
To prove our result, we give a generic transformation from an order-revealing
encryption scheme into one with strongly correct comparison, which enables the
consistent comparison of ciphertexts that are not obtained as the valid
encryption of any message. We believe this construction may be of independent

Illiteracy, low educational status, and cardiovascular mortality in India
Background: Influence of education, a marker of SES, on cardiovascular disease (CVD) mortality has not been evaluated in low-income countries. To determine influence of education on CVD mortality a cohort study was performed in India. Methods: 148,173 individuals aged ≥ 35 years were recruited in Mumbai during 1991-1997 and followed to ascertain vital status during 1997-2003. Visibility Representations of Boxes in 2.5 Dimensions
We initiate the study of 2.5D box visibility representations (2.5D-BR) where
vertices are mapped to 3D boxes having the bottom face in the plane and
edges are unobstructed lines of sight parallel to the - or -axis. We
prove that: Every complete bipartite graph admits a 2.5D-BR; The
complete graph admits a 2.5D-BR if and only if ; Every
graph with pathwidth at most admits a 2.5D-BR, which can be computed in
linear time. We then turn our attention to 2.5D grid box representations
(2.5D-GBR) which are 2.5D-BRs such that the bottom face of every box is a unit
square at integer coordinates. We show that an -vertex graph that admits a
2.5D-GBR has at most edges and this bound is tight. Finally,
we prove that deciding whether a given graph admits a 2.5D-GBR with a given

Flavor conversion of cosmic neutrinos from hidden jets
High energy cosmic neutrino fluxes can be produced inside relativistic jets
under the envelopes of collapsing stars. In the energy range E ~ (0.3 - 1e5)
GeV, flavor conversion of these neutrinos is modified by various matter effects
inside the star and the Earth. We present a comprehensive (both analytic and
numerical) description of the flavor conversion of these neutrinos which
includes: (i) oscillations inside jets, (ii) flavor-to-mass state transitions
in an envelope, (iii) loss of coherence on the way to observer, and (iv)
oscillations of the mass states inside the Earth. We show that conversion has
several new features which are not realized in other objects, in particular
interference effects ("L- and H- wiggles") induced by the adiabaticity
violation. The neutrino-neutrino scattering inside jet and inelastic neutrino
interactions in the envelope may produce some additional features at E > 1e4
GeV. We study dependence of the probabilities and flavor ratios in the
matter-affected region on angles theta13 and theta23, on the CP-phase delta, as
well as on the initial flavor content and density profile of the star. We show
that measurements of the energy dependence of the flavor ratios will, in
principle, allow to determine independently the neutrino and astrophysical

Lower Left Ventricular Ejection Fraction Relates to Cerebrospinal Fluid Biomarker Evidence of Neurodegeneration in Older Adults
BACKGROUND: Subclinical cardiac dysfunction is associated with decreased cerebral blood flow, placing the aging brain at risk for Alzheimer's disease (AD) pathology and neurodegeneration. OBJECTIVE: This study investigates the association between subclinical cardiac dysfunction, measured by left ventricular ejection fraction (LVEF), and cerebrospinal fluid (CSF) biomarkers of AD and neurodegeneration. METHODS: Vanderbilt Memory & Aging Project participants free of dementia, stroke, and heart failure (n = 152, 72±6 years, 68% male) underwent echocardiogram to quantify LVEF and lumbar puncture to measure CSF levels of amyloid-β42 (Aβ42), phosphorylated tau (p-tau), and total tau (t-tau). Linear regressions related LVEF to CSF biomarkers, adjusting for age, sex, race/ethnicity, education, Framingham Stroke Risk Profile, cognitive diagnosis, and apolipoprotein E ɛ4 status. Secondary models tested an LVEF x cognitive diagnosis interaction and then stratified by diagnosis (normal cognitive (NC), mild cognitive impairment (MCI)). RESULTS: Higher LVEF related to decreased CSF Aβ42 levels (β= -6.50, p = 0.04) reflecting greater cerebral amyloid accumulation, but this counterintuitive result was attenuated after excluding participants with cardiovascular disease and atrial fibrillation (p = 0.07). We observed an interaction between LVEF and cognitive diagnosis on CSF t-tau (p = 0.004) and p-tau levels (p = 0.002), whereas lower LVEF was associated with increased CSF t-tau (β= -9.74, p = 0.01) and p-tau in the NC (β= -1.41, p = 0.003) but not MCI participants (p-values>0.13). CONCLUSIONS: Among cognitively normal older adults, subclinically lower LVEF relates to greater molecular evidence of tau phosphorylation and neurodegeneration. Modest age-related changes in cardiovascular function may have implications for pathophysiological changes in the brain later in life
