    SODIUM AESCINATE INJECTION FOR SKIN FLAP TRANSPLANTATION OF HAND OR FOOT IN CHILDREN

    Background: The purpose of this study is to evaluate the efficiency and safety of sodium aescinate injection for treating children suffering transplanted flap in children hand or foot. Materials and Methods: Ninety children patients with transplanted cutaneous nerve nutrition vascular flaps in hand or foot were selected and divided into “treatment” and “control” groups randomly by computer. The treatment group was prescribed intravenous sodium aescine injection and conventional therapy. The control group was only offered conventional therapy. Seven days following treatment, cumulative wound drainage, swelling of flap and adverse reactions were recorded. One month subsequent to treatment, two-point discrimination of flap was recorded. At the outset of treatment (0-d) and 1, 3, 7-days following treatment, the concentrations of C-reactive protein (CRP) and malondialdehyde(MDA) in venous blood were tested. Results: At the outset of treatment, there was no statistically significant difference between the two groups in terms of CRP and MDA (P>0.05).     Obesity and endocrine-related cancer: The important role of IGF-1

    Obesity is increasingly becoming a global epidemic of concern and is considered a risk factor for several endocrine-related cancers. Moreover, obesity is associated with cancer development and poor prognosis. As a metabolic abnormality, obesity leads to a series of changes in insulin, IGF-1, sex hormones, IGFBPs, and adipokines. Among these factors, IGF-1 plays an important role in obesity-related endocrine cancers. This review describes the role of obesity in endocrine-related cancers, such as prostate cancer, breast cancer and pancreatic cancer, focusing on the mechanism of IGF-1 and the crosstalk with estrogen and adipokines. In addition, this review briefly introduces the current status of IGF-1R inhibitors in clinical practice and shows the prospect of IGF-1R inhibitors in combination with other anticancer drugs

    Developing a framework leveraging building information modelling to validate fire emergency evacuation

    In fire emergency management, a delayed execution will cause a significant number of casualties. Conventional fire drills typically only identify a certain percentage of evacuation bottlenecks after the building has been constructed, which is hard to improve. This paper proposes an innovative framework to validate fire emergency evacuation at the early design stage. According to the experience and knowledge of fire emergency evacuation design, the proposed framework also introduces a seamless two-way information channel to embed fire emergency evacuation simulations into a BIM-based design environment. Several critical factors for fire evacuation have been reviewed in relevant domain knowledge, which is used to build virtual characters to test in experimental scenarios. The results are analyzed to validate fire emergency evacuation factors, and the feedback knowledge is stored as a knowledge model for further applications

    Sustainable building design has become a hot topic over the past decades. Many standards, databases, and tools have been developed for achieving a sustainable building. Not until recently have the importance of structural engineering and its contribution to sustainable building design been full recognised. However, due to the highly fragmented and diversity of knowledge across building and infrastructure domains, there is a lack of approach that can address all the sustainable issues within the structural design. This paper reviews the sustainable design from the perspective of structural engineering: (1) reviewing the current situation; (2) identifying the gaps and difficulties; and (3) making recommendations for future improvements. The strategies and indicators, as well as BIM-enabled methodology, for sustainable structural design (SSD) are also discussed in a holistic way. The results of this investigation show that most of the methods are not doing well in terms of delivering a successful sustainable structural design. It is expected that the future BIM could probably provide such a platform to address these issues

    AD-BERT: Using Pre-trained contextualized embeddings to Predict the Progression from Mild Cognitive Impairment to Alzheimer's Disease

    Objective: We develop a deep learning framework based on the pre-trained Bidirectional Encoder Representations from Transformers (BERT) model using unstructured clinical notes from electronic health records (EHRs) to predict the risk of disease progression from Mild Cognitive Impairment (MCI) to Alzheimer's Disease (AD). Materials and Methods: We identified 3657 patients diagnosed with MCI together with their progress notes from Northwestern Medicine Enterprise Data Warehouse (NMEDW) between 2000-2020. The progress notes no later than the first MCI diagnosis were used for the prediction. We first preprocessed the notes by deidentification, cleaning and splitting, and then pretrained a BERT model for AD (AD-BERT) based on the publicly available Bio+Clinical BERT on the preprocessed notes. The embeddings of all the sections of a patient's notes processed by AD-BERT were combined by MaxPooling to compute the probability of MCI-to-AD progression. For replication, we conducted a similar set of experiments on 2563 MCI patients identified at Weill Cornell Medicine (WCM) during the same timeframe. Results: Compared with the 7 baseline models, the AD-BERT model achieved the best performance on both datasets, with Area Under receiver operating characteristic Curve (AUC) of 0.8170 and F1 score of 0.4178 on NMEDW dataset and AUC of 0.8830 and F1 score of 0.6836 on WCM dataset. Conclusion: We developed a deep learning framework using BERT models which provide an effective solution for prediction of MCI-to-AD progression using clinical note analysis

    Research Progress on Spatiotemporal Distribution Characteristics of Carbon Stable Isotopes in Wines from New and Old World Countries

    With the continuous development of economic globalization, the production and consumption of wine in New and Old World countries have shown a steady growth trend. Wines of different grades and styles have been developed, which are related to geographical origin. Carbon stable isotopes are important indicators for geographical origin identification and have been of great concern to researchers due to their stability and objectivity. This paper summarizes the spatiotemporal distribution characteristics of carbon stable isotopes in ethanol, glycerol, total carbon and other components in wines from the Old (France, Italy, Spain, Germany, Switzerland, Croatia, and Romania) and New Worlds (Australia, South Africa, Chile, Argentina, Brazil, and China). In general, there are small differences in the carbon stable isotope ratio of various components between New and Old World wines, so it is impossible to achieve reliable results when only carbon stable isotope data is used for geographical origin identification. The combined use of carbon stable isotope data with other isotope data and mineral elements can improve the accuracy of geographical origin identification. Furthermore, the application of carbon stable isotope technology is also summarized over the past 15 years with the aim to provide a reference for the establishment of database

    Ganoderma Lucidum Polysaccharide Peptide Alleviates Hepatoteatosis via Modulating Bile Acid Metabolism Dependent on FXR-SHP/FGF

    Background/Aims: Non-alcoholic fatty liver disease (NAFLD) encompasses a series of pathologic changes ranging from steatosis to steatohepatitis, which may progress to cirrhosis and hepatocellular carcinoma. The purpose of this study was to determine whether ganoderma lucidum polysaccharide peptide (GLPP) has therapeutic effect on NAFLD. Methods: Ob/ ob mouse model and ApoC3 transgenic mouse model were used for exploring the effect of GLPP on NAFLD. Key metabolic pathways and enzymes were identified by metabolomics combining with KEGG and PIUmet analyses and key enzymes were detected by Western blot. Hepatosteatosis models of HepG2 cells and primary hepatocytes were used to further confirm the therapeutic effect of GLPP on NAFLD. Results: GLPP administrated for a month alleviated hepatosteatosis, dyslipidemia, liver dysfunction and liver insulin resistance. Pathways of glycerophospholipid metabolism, fatty acid metabolism and primary bile acid biosynthesis were involved in the therapeutic effect of GLPP on NAFLD. Detection of key enzymes revealed that GLPP reversed low expression of CYP7A1, CYP8B1, FXR, SHP and high expression of FGFR4 in ob/ob mice and ApoC3 mice. Besides, GLPP inhibited fatty acid synthesis by reducing the expression of SREBP1c, FAS and ACC via a FXR-SHP dependent mechanism. Additionally, GLPP reduced the accumulation of lipid droplets and the content of TG in HepG2 cells and primary hepatocytes induced by oleic acid and palmitic acid. Conclusion: GLPP significantly improves NAFLD via regulating bile acid synthesis dependent on FXR-SHP/FGF pathway, which finally inhibits fatty acid synthesis, indicating that GLPP might be developed as a therapeutic drug for NAFLD

    Identification of avian polyomavirus and its pathogenicity to SPF chickens

    The research aimed to study an Avian polyomavirus strain that was isolated in Shandong, China. To study the pathogenicity of APV in SPF chickens, and provide references for epidemiological research and disease prevention and control of APV. The genetic characterization of APV strain (termed APV-20) was analyzed and the pathogenicity of APV was investigated from two aspects: different age SPF chickens, and different infection doses. The results revealed that the APV-20 exhibits a nucleotide homology of 99% with the other three APV strains, and the evolution of APV In China was slow. In addition, the APV-20 infection in chickens caused depression, drowsiness, clustering, and fluffy feathers, but no deaths occurred in the infected chickens. The main manifestations of necropsy, and Hematoxylin and Eosin staining (HE) showed that one-day-old SPF chickens were the most susceptible, and there was a positive correlation between viral load and infection dose in the same tissue. This study showed that SPF chickens were susceptible to APV, and an experimental animal model was established. This study can provide a reference for the pathogenic mechanism of immune prevention and control of APV
