    Conservative management of renal trauma, 10 years of experience

    Introducción: el tratamiento conservador es la opción más utilizada en el tratamiento del trauma renal porque la mayoría de estas lesiones evolucionan satisfactoriamente. Objetivo: describir los resultados obtenidos con el tratamiento conservador en los pacientes con trauma renal. Método: se realizó un estudio descriptivo, retrospectivo con 89 pacientes con diagnóstico de trauma renal tratados en el Servicio de Urgencia del Hospital “Arnaldo Milián Castro” desde el año 2005 hasta 2014. Resultados: el diagnóstico fue más frecuente en pacientes masculinos de 30-49 años de edad, las caídas se presentaron con mayor frecuencia y afectaron al 52,80%, el 76,86% fueron observados y el 14,60% intervenidos quirúrgicamente, la mayoría de los pacientes evolucionaron satisfactoriamente y no se presentó ningún fallecido. Conclusiones: la conducta conservadora en el trauma renal es conveniente y justificada en la mayoría de los casos.Introduction: conservative treatment is the most option used in the treatment of renal trauma because most of these lesions evolve satisfactorily. Objective: to describe the results obtained with conservative treatment in patients with renal trauma. Method: it was carried out a descriptive, retrospective study with 89 patients diagnosed with renal trauma treated at the Emergency Service of "Arnaldo Milián Castro" Hospital from 2005 to 2014. Results: the diagnosis was more frequent in male patients of 30- 49 years of age, falls occurred more frequently and affected 52.80%, 76.86% were observed and 14.60% underwent surgery, most of the patients evolved satisfactorily and no deceased appeared. Conclusions: conservative behavior in renal trauma is convenient and justified in most cases

    Experiencia inicial con cinta suburetral transobturatriz en la intervención quirúrgica de la incontinencia urinaria de esfuerzo

    Urinary incontinence is a common problem that increases with age. The incidence is variable, with a prevalence of 11%-72%; the procedure of transobturator suburethral tape, because of the simplicity, the easy learning, to effectiveness, to minimum invasion, to low morbidity, to limited surgical time, to short hospital stay and to faster recovery of patients has become in the treatment of choice in the treatment of urinary incontinence of effort. A cross-sectional study was performed to 23 patients diagnosed with urinary incontinence who were treated at the Urology Service of "Dr. Daniel Codorniu Pruna" University Hospital of Placetas and at "Arnaldo Milián Castro" University Provincial Hospital of Santa Clara during the period between September 2013 and July 2015 with the aim of describing the results of the implementation of the transobturator suburethral tape. The average age of the patients was 54.04 years; all were multiparous, with an average of 2.5 births, 4.04 pregnancies, 1.39 abortions (spontaneous or induced) and 11 (47.82%) had been previously operated by urinary incontinence of effort; all, except two fulfilled the criteria for healing. The use of transobturator suburethral tape in the treatment of genuine stress urinary incontinence for effort can give prompt solution, economical and effective to patients with this technique solutions recover quickly, they reduce hospitalization time and achieve a return to productive activity form earlier.La incontinencia urinaria es un problema frecuente que aumenta con la edad. La incidencia es variable, con una prevalencia del 11%-72%; el procedimiento de cinta suburetral transobturatriz, debido a la sencillez, al fácil aprendizaje, a la efectividad, a la mínima invasión, a la poca morbilidad, al escaso tiempo quirúrgico, a la corta estancia postoperatoria y a la rápida recuperación de las pacientes se ha convertido en el tratamiento de elección en el tratamiento de la incontinencia urinaria de esfuerzo. Se realizó un estudio transversal a 23 pacientes con diagnóstico de incontinencia urinaria de esfuerzo que fueron atendidas en el Servicio de Urología del Hospital Universitario “Dr. Daniel Codorniú Pruna” de Placetas y el Hospital Provincial Universitario “Arnaldo Milián Castro” de Santa Clara durante el período comprendido entre septiembre de 2013 y julio de 2015 con el objetivo de describir los resultados obtenidos en la implementación de la cinta suburetral transobturatriz. La edad media de las pacientes fue de 54.04 años; todas eran multíparas, con una media de 2.5 partos, 4.04 embarazos, 1.39 abortos (espontáneo o provocado) y 11 (47.82%) habían sido intervenidas previamente por incontinencia urinaria de esfuerzo; todas, excepto dos, cumplieron los criterios de curación. El uso de cinta suburetral transobturatriz en el tratamiento de la incontinencia urinaria de esfuerzo genuina permite dar soluciones prontas, económicas y efectivas a las pacientes que con esta técnica se recuperan rápidamente, disminuyen el tiempo de hospitalización y logran el retorno a la actividad productiva de forma más temprana

    Pediatric liver transplantation: caregivers’ quality of life

    Purpose: The survival rate of pediatric patients undergoing liver transplantation has increased considerably. Despite this, the period after transplantation is still complex and poses several challenges to the recipient’s family, which is responsible for care management. Recently, more attention has been paid to the impact of this complex procedure on the quality of life of caregivers. Hence, this study is aimed at assessing the quality of life of caregivers of patients who have undergone liver transplantation and the aspects that influence it. Methods: This was an observational and cross-sectional study. From November 2020 to January 2021, short-form-36 questionnaires and additional questions were given to the main caregivers of children and adolescents who underwent pediatric liver transplantation. Results: Thirty-eight questionnaires were completed and the results revealed a lower quality of life in comparison to Brazilian standards, primarily in the mental domains (41.8±14.1 vs. 51.1±2.8; p<0.001). It did not show a significant association with socioeconomic or transplant-related factors, but it did show a negative impact on parents’ perception of the child’s health. Parents who reported worse health status for their children had a lower mental quality of life (44.1±13.8 vs. 33.3±12.6; p<0.05). Conclusion: The caregivers of transplanted children have a lower quality of life than those of the local population. Psychological assistance should be routinely provided to parents for long-term follow-up to mitigate potential negative effects on the transplanted child’s care

    A PROBLEMATIC TEACHING MODEL FOR THE IMPLEMENTATION OF E-LEARNING FORMATIVE PATHWAYS

    The contemporary e-learning scenario is marked by a strong contradiction between Italian and international technical and pedagogical literature on one hand and the reality of the experiences carried out to date on the other. From a pedagogical perspective, analysis of the literature reveals four strong points in e-learning, corresponding to as many methodological-didactic categories characterizing contemporary innovative didactic practices. In brief, these categories relate to the concepts of individualization, personalization, constructive/socio-constructive approach, student/students and students/teachers educational interaction. Since we are dealing with complex categories, it is worth briefly analyzing their internal problems and the gap between theory and practice in e-learning settings. In any case, their usage must include structural integration and cannot stick to single educational experiences of the additive-linear kind. 1.1. Individualization The formative category of individualization seeks equal formative opportunities for the learners and deals with the need to use differentiated didactic strategies in order to allow all the students to achieve the same goal. The didactic concept which supports this category envisages the very same formative objective (knowledge or competence) for all the students, but differentiated didactic procedures (timing, materials, spaces, exercises…), reflecting student individuality. This allows everybody to reach the same objective. In theory, e-learning grants maximum individualization, since it can offer many didactic paths simultaneously, all aiming to achieve the same goal. In actual fact, this possibility is almost never exploited or is trivialized in a limited choice of alternatives which hardly exploit effective reading tools of entrance levels for single students, offer scanty formative evaluation feed-back and remedial sequences and strategies. 1.2.
