    Prevalence study of yaws in the Democratic Republic of Congo using the lot quality assurance sampling method.

    BACKGROUND: Until the 1970s the prevalence of non-venereal trepanomatosis, including yaws, was greatly reduced after worldwide mass treatment. In 2005, cases were again reported in the Democratic Republic of the Congo. We carried out a survey to estimate the village-level prevalence of yaws in the region of Equator in the north of the country in order to define appropriate strategies to effectively treat the affected population. METHODOLOGY/PRINCIPAL FINDINGS: We designed a community-based survey using the Lot Quality Assurance Sampling method to classify the prevalence of active yaws in 14 groups of villages (lots). The classification into high, moderate, or low yaws prevalence corresponded to World Health Organization prevalence thresholds for identifying appropriate operational treatment strategies. Active yaws cases were defined by suggestive clinical signs and positive rapid plasma reagin and Treponema pallidum hemagglutination serological tests. The overall prevalence in the study area was 4.7% (95% confidence interval: 3.4-6.0). Two of 14 lots had high prevalence (>10%), three moderate prevalence (5-10%) and nine low prevalence (<5%.). CONCLUSIONS/SIGNIFICANCE: Although yaws is no longer a World Health Organization priority disease, the presence of yaws in a region where it was supposed to be eradicated demonstrates the importance of continued surveillance and control efforts. Yaws should remain a public health priority in countries where previously it was known to be endemic. The integration of sensitive surveillance systems together with free access to effective treatment is recommended.     Effectiveness of ready-to-use therapeutic food compared to a corn/soy-blend-based pre-mix for the treatment of childhood moderate acute malnutrition in Niger.

    Standard nutritional treatment of moderate acute malnutrition (MAM) relies on fortified blended flours though their importance to treat this condition is a matter of discussion. With the newly introduced World Health Organization growth standards, more children at an early stage of malnutrition will be treated following the dietary protocols as for severe acute malnutrition, including ready-to-use therapeutic food (RUTF). We compared the effectiveness of RUTF and a corn/soy-blend (CSB)-based pre-mix for the treatment of MAM in the supplementary feeding programmes (SFPs) supported by Médecins Sans Frontières, located in the Zinder region (south of Niger). Children measuring 65 to <110 cm, newly admitted with MAM [weight-for-height (WHM%) between 70% and <80% of the NCHS median] were randomly allocated to receive either RUTF (Plumpy'Nut®, 1000 kcal day(-1)) or a CSB pre-mix (1231 kcal day(-1)). Other interventions were similar in both groups (e.g. weekly family ration and ration at discharge). Children were followed weekly up to recovery (WHM% ≥ 85% for 2 consecutive weeks). In total, 215 children were recruited in the RUTF group and 236 children in the CSB pre-mix group with an overall recovery rate of 79.1 and 64.4%, respectively (p < 0.001). There was no evidence for a difference between death, defaulter and non-responder rates. More transfers to the inpatient Therapeutic Feeding Centre (I-TFC) were observed in the CSB pre-mix group (19.1%) compared to the RUTF group (9.3%) (p = 0.003). The average weight gain up to discharge was 1.08 g kg(-1) day(-1) higher in the RUTF group [95% confidence interval: 0.46-1.70] and the length of stay was 2 weeks shorter in the RUTF group (p < 0.001). For the treatment of childhood MAM in Niger, RUTF resulted in a higher weight gain, a higher recovery rate, a shorter length of stay and a lower transfer rate to the I-TFC compared to a CSB pre-mix. This might have important implications on the efficacy and the quality of SFPs

The high-symmetry phase is described by a hexagonal rank-4 superspace group. Pretransitional phenomena in this crystallographic superspace reveal competing short-range-ordering phenomena within the high-symmetry phase. Very high-resolution diffraction data show that critical scattering appears at inequivalent points within the four-dimensional Brillouin zone, although the first phase transition at T-c1 near 158 K implies the condensation at only one of those points. The resulting superspace group remains of dimension 4. Two other phase transitions are reported at T-c2 = 152.8(4) K and T-c3 = 109(4) K in n-octadecane/urea-d(4). The two low-symmetry phases that arise are described by rank-5 superspace groups

    Conductance switching at the nanoscale of diarylethene derivatives self-assembled monolayers on La0.7_{0.7}Sr0.3_{0.3}MnO3_3

    We report on the phosphonic acid route for the grafting of functional molecules, optical switch (dithienylethene diphosphonic acid, DDA), on La0.7Sr0.3MnO3 (LSMO). Compact self-assembled monolayers (SAMs) of DDA are formed on LSMO as studied by topographic atomic force microscopy (AFM), ellipsometry, water contact angle and X-ray photoemission spectroscopy (XPS). The conducting AFM measurements show that the electrical conductance of LSMO/DDA is about 3 decades below that of the bare LSMO substrate. Moreover, the presence of the DDA SAM suppresses the known conductance switching of the LSMO substrate that is induced by mechanical and/or bias constraints during C-AFM measurements. A partial light-induced conductance switching between the open and closed forms of the DDA is observed for the LSMO/DDA/C-AFM tip molecular junctions (closed/open conductance ratio of about 8). We show that, in the case of long-time exposition to UV light, this feature can be masked by a non-reversible decrease (a factor of about 15) of the conductance of the LSMO electrode.Comment: Full paper with supporting informatio

At room temperature, it presents a quasi-one dimensional liquid-like state for the confined guest molecules within the rigid, hexagonal framework of the urea host. At lower temperatures, we report the existence of two other phases. Below T-c=248 K there appears a phase with rank four superspace group P6(1)22(00 gamma), the one typically observed at room temperature in n-alkane/urea compounds with longer guest molecules. A misfit parameter, defined by the ratio gamma=C-h/C-g (C-host/C-guest), is found to be 0.632 +/- 0.005. Below T-c1=123 K, a monoclinic modulated phase is created with a constant shift along c of the guest molecules in adjacent channels. The maximal monoclinic space group for this structure is P12(1)1(alpha 0 gamma). Analogies and differences with n-heptane/urea, which also presents a monoclinic, modulated low-temperature phase, are discussed

    Long-range modulation of a composite crystal in a five-dimensional superspace

Physical Review B, 91(18), 7. doi:10.1103/PhysRevB.91.184101The intergrowth crystal of n-tetracosane/urea presents a misfit parameter, defined by the ratio gamma = c(h)/c(g) (c(host)/c(guest)), that is very close to a commensurate value (gamma congruent to 1/3). High-resolution diffraction studies presented here reveal an aperiodic misfit parameter of gamma = 0.3369, which is found to be constant at all temperatures studied. A complex sequence of structural phases is reported. The high temperature phase (phase I) exists in the four-dimensional superspace group P6(1)22(00 gamma). At T-c1 = 179(1) K, a ferroelastic phase transition increases the dimension of the crystallographic superspace. This orthorhombic phase (phase II) is characterized by the five-dimensional (5D) superspace group C222(1)(00 gamma)(10 delta) with a modulation vector a(o)* + c(m)* = a(o)* + delta . c(h)*, in which the supplementary misfit parameter is delta = 0.025(1) in host reciprocal units. This corresponds to the appearance of a modulation of very long period (about 440 +/- 16 angstrom). At T-c2 = 163.0(5) K, a 5D to 5D phase transition leads to the crystallographic superspace group P2(1)2(1)2(1)(00 gamma)(00 delta) with a very similar value of delta. This phase transition reveals a significant hysteresis effect

    Changes in Health and Physical Fitness Parameters After 6 Months of High-intensity Group Exercise in Firefighters: Preliminary Data

    The physical demands of firefighting require the men and women employed in this profession to be in optimal physical condition to perform their jobs proficiently, as well as to mitigate the risk of injury. Every year, the city of Addison, TX, budgets many thousands of dollars to the compensation plan for first responders. Most of the funding is used for rehabilitation services due to work related injuries. PURPOSE: While many of these injuries are unavoidable due to the inherent risks of the profession, ensuring proper physical fitness is one of the most effective methods to reduce many of these impairments. The purpose of this preliminary study was to characterize health and fitness parameters in 18 professional firefighters from the city of Addison, TX, prior to a 6-month training program. METHODS: Upon arrival, all participants underwent testing in the following order: body composition, range of motion, anaerobic power, muscular endurance, and cardiorespiratory fitness. All participants also completed a detailed health history questionnaire and answered questions specifically addressing chronic low back pain. RESULTS: The following values were attained from testing: total body fat: 30.1±9.7%; flexibility: 24.9±6.3 cm; peak power: 1068.7±272.9 W; mean power: 636.9±143.4 W; time to peak power 0.57±0.35 sec; pushup: 28.8±14.9; curl up: 22.1±15.8; VO2max: 34.1±5.1 ml/kg/min. CONCLUSION: Based off the comparison of reported means and ACSM’s fitness norms, it can be concluded that improvements are necessary in body composition, muscular endurance, range of motion and cardiorespiratory fitness. With improvements in these physiological variables, tactical performance may be optimally performed in a safer manner

    Who I Am: The Meaning of Early Adolescents’ Most Valued Activities and Relationships, and Implications for Self-Concept Research

    Self-concept research in early adolescence typically measures young people’s self-perceptions of competence in specific, adult-defined domains. However, studies have rarely explored young people’s own views of valued self-concept factors and their meanings. For two major self domains, the active and the social self, this mixed-methods study identified factors valued most by 526 young people from socioeconomically diverse backgrounds in Ireland (10-12 years), and explored the meanings associated with these in a stratified subsample (n = 99). Findings indicate that self-concept scales for early adolescence omit active and social self factors and meanings valued by young people, raising questions about content validity of scales in these domains. Findings also suggest scales may under-represent girls’ active and social selves; focus too much on some school-based competencies; and, in omitting intrinsically salient self domains and meanings, may focus more on contingent (extrinsic) rather than true (intrinsic) self-esteem
