Impact of international organic markets on small U.S. producers
Rapid growth of the organic agricultural sector in the U.S. and implementation of the U.S. Department of Agriculture's national organic standards in 2002 have lead to concerns that organic production could become increasingly concentrated on larger U.S. and international farms, disrupting the market access of small domestic organic producers. However, data on the U.S. organic agriculture show that the smallest-scale farms continue to hold a small but stable piece of the organic sector and that U.S. organic farm size has grown slowly. The amount of land under organic production worldwide is growing rapidly, particularly in developing countries producing commodities for export, many of which are not widely grown in the U.S. Small-scale producers using direct markets have likely been least impacted from increased organic imports, while producers of organic oilseeds and cotton have likely been most impacted. Federal and State government agencies and the private sector have launched initiatives to sustain small-farm participation in the U.S. organic sector. Novices Outperform Experienced Laparoscopists on Virtual Reality Laparoscopy Simulator
BACKGROUND AND OBJECTIVES: Virtual reality has been poorly studied among gynecologic surgeons. The aim of this study was to evaluate whether performance on the Minimally Invasive Surgery Trainer-Virtual Reality (MIST-VR) laparoscopic trainer reflects laparoscopic experience among gynecologic surgeons and trainees. METHODS: Twenty-six medical students, residents, and attending gynecologic surgeons completed a MIST-VR training program. A new simulated task was then presented to each participant, who repeated the task until proficiency was reached. RESULTS: Attending physicians performed poorly when compared with medical students, requiring more than twice the number of attempts to reach proficiency (Mann-Whitney P\u3c0.01). Among medical students and residents, there was an association between years of live laparoscopy experience and poor simulator performance (Spearman r P=0.01). CONCLUSION: Increased operating room experience and age were associated with worsening simulator performance. Several potential explanations for this trend are discussed, including lack of tactile and contextual feedback. Caution should be exercised when considering current virtual reality simulator technology as a measure of experience or ability among gynecologic surgeons
The guide dog as a mobility aid part 2: perceived changes to travel habits
This article describes the second of a two-part study that examined the effects of a guide dog as an aid to mobility. The first part, which is also published in this issue, showed that dogs were perceived to significantly improve travel performance, irrespective of the participants' orientation and mobility skills before receiving the
dog. The second part of the study describes the changes a dog makes to travel habits. In this second part, the travel habits of 50 people who were blind or vision impaired
were examined retrospectively before and after they received a dog. The results indicate that dogs were used more frequently than other mobility aids except when
it was more convenient to use a human guide or a long cane, as for example on a very short journey. People travelled independently more often and went further, with greater ease and enjoyment when travelling with a dog. The use of a dog appeared to reduce problems with access and the need to avoid certain journeys. However, dogs also caused difficulties, especially in social situations where they were no/welcomed,and in crowded, cramped or dog-populated environments. Design of a Robotic Apparatus for Simulated Motion of the Human Hand
Background: The hand is complex, in that any small disturbance to the flexor tendons, extensor tendons, and intrinsic muscles can result in dysfunction of the entire structure. We designed a robotic device to consistently load a native thumb carpometacarpal (CMC) joint in assessing the effects of ligamentous damage on stability of the thumb CMC joint.
Methods: The device consisted of a mechanical plate in which to fixate a cadaveric hand, a tendon-suture routing system, a bracket to couple multiple suture lines to a cable to maintain equal force among sutures and tendons, and the finger-thumb force measurement devices. To apply force to the sutures, a cable was run from the suture coupling device to the tendon actuator and from the finger-thumb force measurement devices to the control system. The device was controlled using a Beaglebone Black microcontroller, load cells, rotary encoders, and a liquid crystal display (ie, LCD) touchscreen interface.
Results: The design worked as intended in terms of basic communication, signal processing, and control functions. Cyclic loading resulted in web creep of the tissue. Using closed-loop control, the system was able to settle to a desired load.
Conclusions: Use of the current device may result in improved understanding of joint movement within the hand, which may help surgeons in treating associated injuries. Future revisions to the device will aim to improve the hardware and software to accelerate the time to converging to the desired force and displacement

First Foods Nutrition Curriculum for New Immigrant Families: A Pilot Study
Background: Immigrant families arrive in the US from a variety of nutritional landscapes and educational experiences. Early childhood is a key time to intervene to set children on a healthy path. Creating nutritional education programs tailored for immigrant families may improve nutrition and health outcomes.
Objective: To evaluate the First Foods curriculum as a tool for knowledge and behavior change for new immigrant families of young children.
Methods: Immigrant caregivers of children less than 2 years old were invited to attend First Foods, a 4-class series. Each series was offered in 1 of 5 different languages (Arabic, Dari, Somali, Burmese, and Nepali). Recruitment occurred through community organizations, primary care clinics and Women, Infants, and Children (WIC), and classes were held in King County, Washington. The curriculum was developed and taught by a registered pediatric dietitian with input from general pediatricians, all experienced in the care of immigrant families. Classes were interpreted in the relevant language and course materials were translated. The classes were based on 4 themes -- 1) Child Eating and Development, 2) Eating Together, 3) Food Safety, and 4) Health Living -- and incorporated positive parenting and child development. Attendees completed pre- and post-surveys in their respective languages or in English. Descriptive statistics, chi-squared analyses, t-tests, and a multi-level linear regression model were conducted in Stata v14.0.
Results: Participants in the classes included 47 caregivers (91% mothers). Nearly one-third had previously lived in a refugee camp. They had lived in the US a mean 5.5 years (95% CI: 3.8-7.2 years), attended a mean 8.6 years of school (95% CI: 7.1-10.1 years), and had a mean of 2.8 children (95% CI: 2.3-3.3 children). Classes ranged in size from 5 to 14 caregivers. Caregivers reported an improved understanding of 2 out of 4 methods to decrease risk of dental caries (drinking tap water, p = \u3c0.001; going to the dentist, p=0.02). They reported a decreased use of food as a reward from the pre- to the post-survey (p=0.027). Additionally, the caregivers reported increased frequency of considering sugar content in family foods (p=0.033), and decreased frequency of purchasing food at a convenience store, after participating in the curriculum (p=0.001). Conversely, there were several domains where caregivers did not show a change in their response.
Conclusion: First Foods, a community-tailored, early childhood feeding curriculum for immigrant parents of young children, improved knowledge and behavior among caregivers from a variety of immigrant communities in some domains. In the other domains, there may be opportunities to further optimize the educational messages and approach
Disaster Vulnerability
Vulnerability drives disaster law, yet the literature lacks both an overarching analysis of the different aspects of vulnerability and a nuanced examination of the factors that shape disaster outcomes. Though central to disaster law and policy, vulnerability often lurks in the shadows of a disaster, evident only once the worst is past and the bodies have been counted. The COVID-19 pandemic is a notable exception to this historical pattern: from the beginning of the pandemic, it has been clear that the virus poses different risks to different people, depending on vulnerability variables. This most recent pandemic experience thus provides a useful vantage point for analyzing vulnerability. Drawing on empirical data from the pandemic and experiences from past disasters, this Article identifies and discusses the policy implications of three dimensions of disaster vulnerability: the geography of vulnerability, competing or conflicting vulnerabilities, and political vulnerability. First, it explores the geography of vulnerability, using statistical analysis and geographic information system (GIS) mapping. The Article presents an innovative COVID-19 vulnerability index that identifies the country’s most vulnerable counties and the leading driver of vulnerability for each county. It demonstrates how this index could have informed voter accommodations during the 2020 elections and mask mandates throughout the pandemic.
The Article also shows how, going forward, similar modeling could make disaster management more proactive and better able to anticipate needs and prioritize disaster mitigation and response resources. Second, this Article explores competing or conflicting vulnerabilities––situations where policy-makers must prioritize one vulnerable group or one aspect of vulnerability over another. To illustrate this, it considers two other policy challenges: school closures and vaccine distribution. Finally, the Article explores political vulnerability, analyzing how disasters make already-vulnerable groups even more vulnerable to certain harms, including political neglect, stigmatization, disenfranchisement, and displacement. In sum, this Article draws upon the costly lessons of COVID-19 to suggest a more robust framework for policy-makers to assess and respond to vulnerability in future disasters
