    Social farming as an innovative approach to promote mental health, social inclusion and community engagement

    I fattori contestuali astratti basati sulla natura vengono riconosciuti come fondamentali per la prevenzione e la promozione della salute mentale . Le aree rurali, infatti, sono sempre più riconosciute come luogo elettivo per la promozione della salute mentale. Negli ultimi anni c'è stata un'ondata di modelli di governance ibrida con base rurale in cui enti pubblici, comunità locali e attori economici uniscono le forze per creare soluzioni di welfare innovative per facilitare le sfide finanziarie (e organizzative) affrontate dai sistemi sanitari nazionali. Utilizzando risorse agricole, come animali e piante, l'agricoltura sociale è in grado di soddisfare esigenze sociali specifiche, tra cui riabilitazione, occupazione protetta, istruzione permanentee altre attività che contribuiscono all'inclusione sociale. Allo stesso tempo, l'agricoltura sociale è in grado di rafforzare la redditività economica e sociale delle comunità rurali. Abbiamo studiato i fattori alla base delle potenzialità delle fattorie sociali per fornire programmi di inserimento lavorativo e riabilitazione per le persone con disturbi mentali. Utilizzando nuove metodologie e strumenti adeguati, abbiamo raccolto dati che indicano gli effetti positivi delle attività agricole sul funzionamento sociale dell'individuo, nonché l'impatto del networking delle aziende agricole sulla sostenibilità del sistema

    Predictors of walking capacity in peripheral arterial disease patients

    OBJECTIVE: To estimate walking capacity in intermittent claudication patients through a prediction model based on clinical characteristics and the walking impairment questionnaire. METHODS: The sample included 133 intermittent claudication patients of both genders aged between 30 and 80 years. Data regarding clinical characteristics, the walking impairment questionnaire and treadmill walking test performance were obtained. Multiple regression modeling was conducted to predict claudication onset distance and total walking distance using clinical characteristics (age, height, mass, body mass index, ankle brachial index lower, gender, history of smoking and co-morbid conditions) and walking impairment questionnaire responses. Comparisons of claudication onset distance and total walking distance measured during treadmill tests and estimated by a regression equation were performed using paired t-tests. RESULTS: Co-morbid conditions (diabetes and coronary artery disease) and questions related to difficulty in walking short distances (walking indoors - such as around your house and walking 5 blocks) and at low speed (walking 1 block at average speed - usual pace) resulted in the development of new prediction models high significant for claudication onset distance and total walking distance (p;0.05) were observed. CONCLUSION: The current study demonstrated that walking capacity can be adequately estimated based on co-morbid conditions and responses to the walking impairment questionnaire

    Efeito agudo do salbutamol no sistema cardiovascular durante o exercício físico em pacientes com asma moderada ou grave: estudo aleatorizado, duplo-cego e cruzado

    Salbutamol é um β2-agonista de curta duração frequentemente utilizado em pacientes com asma para prevenir os sintomas durante ou após exercício físico. Alterações hemodinâmicas em repouso estão bem descritas. Contudo são escassos os dados sobre os efeitos na frequência cardíaca (FC) e pressão arterial (PA) durante o exercício e na fase de recuperação em pacientes com asma moderada ou grave. Foi realizado um estudo aleatorizado, duplo-cego e cruzado, em que foram inclusos 15 indivíduos com asma moderada e grave, com média de idade de 46,4±9,3 anos. Os pacientes realizaram um teste de esforço máximo em dois dias não consecutivos, com administração de 400mcg de salbutamol ou 4 “puffs” de placebo. Durante todo o protocolo foi monitorada a FC, a PA, a percepção de esforço e o pico de fluxo expiratório (PFE). Após o uso do salbutamol, o valor do PFE aumentou em média de 28,0±47,7L/m, permanecendo maior nos tempos de 5, 10 e 15 minutos de recuperação passiva em relação ao placebo (p;0,05). Esses resultados sugerem que o uso de salbutamol é seguro, e que a FC não necessita de ser ajustada para prescrever a intensidade do exercício após a administração de salbutamol em indivíduos com asma moderada ou grave.Salbutamol es un agonista β2 de corta duración frecuentemente utilizado en pacientes con asma para prevenir los síntomas durante o después del ejercicio físico. Sin embargo, son escasos los datos sobre los efectos en la frecuencia cardíaca (FC) y la presión arterial (PA) durante el ejercicio y en la fase de recuperación en pacientes con asma moderada o grave. Se realizó un estudio aleatorizado, doble ciego y cruzado, donde fueron incluidos 15 individuos con asma moderada y grave, con una media de edad de 46,4 ± 9,3 años. Los pacientes realizaron una prueba de esfuerzo máximo en 2 días no consecutivos, con administración de 400mcg de salbutamol o 4 «puffs» de placebo. Durante el protocolo se supervisaron la FC, PA, percibe el esfuerzo y el Pico flujo espiratorio (PEF). Después del uso del salbutamol, el valor del PFE aumentó en promedio de 28,0 ± 47,7 L/m, permaneciendo mayor en los tiempos 5, 10 y 15 minutos de recuperación pasiva con relación al placebo (p ; 0,05). Los resultados sugieren que el uso de salbutamol es seguro y que la FC no necesita ser ajustada para prescribir la intensidad del ejercicio después de la administración de salbutamol en individuos con asma moderada o grave.Salbutamol is a β2-agonist of short duration commonly used in patients with asthma to prevent symptoms during or after exercise. After the use of salbutamol, the PEF value increased by a mean of 28.0±47.7L/m, remaining increased at 5, 10 and 15 minutes of passive recovery compared to placebo (p;0.05). These results suggest that the use of salbutamol is safe and that HR does not need to be adjusted to prescribe exercise intensity following salbutamol administration in subjects with moderate or severe asthma

    Nature-Based Interventions for Mental Health Care: Social Network Analysis as a Tool to Map Social Farms and their Response to Social Inclusion and Community Engagement

    Social farming represents a hybrid governance model in which public bodies, local communities, and economic actors act together to promote health and social inclusion in rural areas. Although relational variables are crucial to foster social farm performance, the relational system in which farms are embedded has still not been fully described. Using social network analysis, here we map the nature of the links of a selected sample of social farms operating in Northern Italy. We also explore possible network variations following specific actions taken to potentiate local social farming initiatives. The results show a certain degree of variability in terms of the extension and features of the examined networks. Overall, the actions taken appear to be significant to enlarge and diversify farms’ networks. Social farming has the potential to provide important benefits to society and the environment and to contrast vulnerability in rural areas. Being able to create social and economic networks of local communities, social farming may also represent an innovative way to respond to the cultural shift from institutional psychiatry to community-based mental health care.     Comparative study of tissue response to polyglecaprone 25, polyglactin 910 and polytetrafluorethylene suture materials in rats

    The authors evaluated the biocompatibility of three suture materials, polyglecaprone 25, polyglactin 910 and polytetrafluorethylene, implanted into subcutaneous tissue in the dorsal region of 20 Wistar albinus rats. After periods of 2, 7, 14 and 21 days, the rats were sacrificed and the specimens were processed for routine histotechnical analysis and stained with hematoxylin and eosin. The rate of fibrosis, angioblastic and fibroblastic proliferation, and also the intensity of inflammation were observed with the optic microscope. The results showed that polyglecaprone 25 suture material induced a mild inflammatory reaction, followed by polyglactin 910 and polytetrafluorethylene, respectively. Such biological behavior must be considered during the selection of the suture material to be used in oral surgery

    FUNDAMENTO: O Walking Impairment Questionnaire (WIQ) tem sido utilizado para a obtenção de informações sobre a percepção de locomoção dos indivíduos com claudicação intermitente (CI). Entretanto, esse questionário ainda não foi traduzido para o idioma português, o que limita sua utilização em amostra brasileira. OBJETIVO: Traduzir e verificar a validade e reprodutibilidade do WIQ em brasileiros com CI. MÉTODOS: Participaram deste estudo 42 indivíduos com CI, diagnosticados pelo índice tornozelo-braço < 0,90. Após os procedimentos de tradução e retradução do questionário, realizado por dois tradutores independentes, a validade de constructo do WIQ foi analisada correlacionando os escores do WIQ com os escores do Medical Outcome Study Questionnaire Short Form 36 (SF-36) e com o desempenho em testes de aptidão física (marcha e de força máxima). A reprodutibilidade foi analisada em duas aplicações do WIQ com 7 dias de intervalo. RESULTADOS: Foi encontrada correlação significativa entre os domínios do WIQ, os domínios do SF-36 (capacidade funcional, aspectos físicos, dor e aspectos emocionais) e o desempenho nos testes de aptidão física (marcha e força máxima). Além disso, verificaram-se coeficientes de correlação intraclasse que variaram de 0,72 a 0,81, e não houve diferença entre os escores do WIQ entre as duas aplicações do questionário. CONCLUSÃO: A versão em português do WIQ é válida e reprodutível em brasileiros com CI.BACKGROUND: The Walking Impairment Questionnaire (WIQ) has been used to evaluate walking impairment in subjects with intermittent claudication (IC). However, this questionnaire has yet to be translated to Brazilian Portuguese, which limits its use in Brazilian subjects. OBJECTIVE: To translate and analyze the validity and reliability of WIQ in Brazilian subjects with IC. METHODS: Forty-two patients with IC, determined by the ankle-brachial index < 0.90, participated in the study. After translation and re-translation, carried out by two independent translators, the construct validity of the WIQ was analyzed by correlating the WIQ scores with the Medical Outcome Study Questionnaire Short Form 36 (SF-36) scores and the physical fitness performance (treadmill and strength tests). The reliability was analyzed with a 7-day interval between two questionnaire applications. RESULTS: Significant correlations between the WIQ domains and the SF-36 (functional capacity, physical aspects, bodily pain and emotional aspects) and physical fitness performance (treadmill and strength tests) were observed. Moreover, the intraclass coefficient correlation ranged from 0.72 to 0.81, and there were no differences in WIQ scores between the two questionnaire applications.     Effect of salbutamol on the cardiovascular response in healthy subjects at rest, during physical exercise, and in recovery phase: a randomized, double-blind, crossover study

    <div><p>ABSTRACT Aim To evaluate the effect of the short-acting beta agonists (SABAs) salbutamol on cardiovascular response rest, exercise and recovery phase. Methods This study was conducted as a randomized, double-blind, placebo controlled, crossover study in 15 healthy adults, with a mean age of 30.2±6.6 years. Participants underwent a maximal effort test on two non-consecutive days with 400 mcg of salbutamol or placebo. Throughout the protocol, the variables HR, blood pressure (BP), perceived rate of effort (modified Borg scale) and peak expiratory flow (PEF) were monitored. Results After salbutamol, baseline HR and PEF had increase from 71±8 to 80±11 bpm (p<0.05) and 454.0±64.5 to 475.3±71.4 L/min (p < 0.05), respectively. The variables HR, BP and Borg were similar between interventions during all the protocol phases (p>0.05). Conclusion Administration of salbutamol increased rest heart rate; however, did not change heart rate, blood pressure and perceived exertion during exercise or recovery. This suggests that the salbutamol administration is safe and does not affect exercise intensity prescription in healthy subjects.</p></div
