    Critical Educational Policy Analysis: a Discussion of Perspectives

    Education policy is the medium through which society and educational institutions interact. Currently major policy thrusts of devolution and participation are being promoted within the educational community. It becomes essential that these poliCces be analysed and evaluated by educators. This paper supports the role of radical humanist perspective in the development of a critical model of educational policy analysis, and stresses the necessity for teacher educators to expose student teachers to critical and radical perspectives

    Rounding and uncertainties in parameters determined from fits to experimental data, or a failure to round data-analysis fit parameters properly may make them useless

    Almost no physically interesting physico/chemical parameter is determined directly from a measurement. Rather, they are determined by performing a least-squares fit of some model to a set of data. Unfortunately, there seems to be no commonly accepted set of `best practices' for determining how to round off such fitted parameter values to a minimum number of significant digits while ensuring that they retain the ability to reproduce the experimental data within their uncertainties. This sometimes results in lists of fitted parameters with no quoted uncertainties that have 2--3 times as many significant digits as the data being fitted, or to the results of an analysis being defined by parameters that are (unnecessarily?) quoted to more digits than normal computer double precision, which makes those results difficult or impossible to apply. Alternatively, it may also lead to fitted parameters being `over-rounded' so that the model no longer accurately represents the experimental data. This presentation describes a `best practice' to address these problems, offers a general-purpose least-squares fitting program that applies it, and provides an illustrative application of this approach in a study of the A\,^1\Sigma_u^+ - X\,^1\Sigma_g^+ system of Mg2_2.Ope

    Radiocarbon dating loess deposits in the Mississippi Valley using terrestrial gastropod shells (Polygyridae, Helicinidae, and Discidae)

    Small terrestrial gastropod shells (mainly Succineidae) have been used successfully to date late Quaternary loess deposits in Alaska and the Great Plains. However, Succineidae shells are less common in loess deposits in the Mississippi Valley compared to those of the Polygyridae, Helicinidae, and Discidae families. In this study, we conducted several tests to determine whether shells of these gastropods could provide reliable ages for loess deposits in the Mississippi Valley. Our results show that most of the taxa that we investigated incorporate small amounts (1–5%) of old carbon from limestone in their shells, meaning that they should yield ages that are accurate to within a few hundred years. In contrast, shells of the genus Mesodon (Mesodon elevatus and Mesodon zaletus) contain significant and variable amounts of old carbon, yielding ages that are up to a couple thousand 14C years too old. Although terrestrial gastropod shells have tremendous potential for 14C dating loess deposits throughout North America,weacknowledge that accuracy to within a few hundred years may not be sufficient for those interested in developing high-resolution loess chronologies. Even with this limitation, however, 14C dating of terrestrial gastropod shells present in Mississippi Valley loess deposits may prove useful for researchers interested in processes that took place over multi-millennial timescales or in differentiating stratigraphic units that have significantly different ages but similar physical and geochemical properties.     Among a group of twenty-eight men aged 35-54 a highly significant correlation was found between fasting serum-triglyceride and the ratio of subscapular skinfold thickness to triceps skinfold. This relation was not attributable to a common correlation with obesity or glucose intolerance. In a similarly selected group of twenty-one men aged 55 and over there was no significant correlation between serum-triglycerides and the skinfold ratio.     Genome engineering of isogenic human ES cells to model autism disorders.

    Isogenic pluripotent stem cells are critical tools for studying human neurological diseases by allowing one to study the effects of a mutation in a fixed genetic background. Of particular interest are the spectrum of autism disorders, some of which are monogenic such as Timothy syndrome (TS); others are multigenic such as the microdeletion and microduplication syndromes of the 16p11.2 chromosomal locus. Here, we report engineered human embryonic stem cell (hESC) lines for modeling these two disorders using locus-specific endonucleases to increase the efficiency of homology-directed repair (HDR). We developed a system to: (1) computationally identify unique transcription activator-like effector nuclease (TALEN) binding sites in the genome using a new software program, TALENSeek, (2) assemble the TALEN genes by combining golden gate cloning with modified constructs from the FLASH protocol, and (3) test the TALEN pairs in an amplification-based HDR assay that is more sensitive than the typical non-homologous end joining assay. We applied these methods to identify, construct, and test TALENs that were used with HDR donors in hESCs to generate an isogenic TS cell line in a scarless manner and to model the 16p11.2 copy number disorder without modifying genomic loci with high sequence similarity

    Smoking Cessation Counselling for Pregnant and Postpartum Women among Midwives, Gynaecologists and Paediatricians in Germany

    The incorporation of guidelines for the treatment of tobacco smoking into routine care requires positive attitudes, counselling skills and knowledge about additional help available for smokers. The study assesses performance of smoking cessation intervention, attitudes, training status and knowledge about additional help for smokers in the care for pregnant and parenting women by midwives, gynaecologists and paediatricians. A survey of all midwives, gynaecologists and paediatricians registered for primary medical care in the federal state Saarland, Germany, was conducted. Participation in the postal questionnaires was 85 %. Depending on profession, 90 % to 100 % see smoking cessation counselling as their assignment, 17 % to 80 % screen for, 48 % to 90 % document smoking status, and 55 % to 76 % offer brief or extensive counselling. 61 % to 87 % consider training to enhance their knowledge and/or counselling skills necessary. The compliance of providers with the necessity to give support in smoking cessation is very high. However, the current status of cessation counselling does not sufficiently correspond to the evidence based requirements. Reports in medical press and advanced training courses should support health care providers and establish smoking as an inherent topic of the anamnesis and treatment of current and former pregnant or parenting smokers

    Background: Stroke is a leading cause of death and disability internationally. One of the three effective interventions in the acute phase of stroke care is thrombolytic therapy with tissue plasminogen activator (tPA), if given within 4.5 hours of onset to appropriate cases of ischaemic stroke.Objectives: To test the effectiveness of a multi-component multidisciplinary collaborative approach compared to usual care as a strategy for increasing thrombolysis rates for all stroke patients at intervention hospitals, while maintaining accepted benchmarks for low rates of intracranial haemorrhage and high rates of functional outcomes for both groups at three months.Methods and design: A cluster randomised controlled trial of 20 hospitals across 3 Australian states with 2 groups: multi- component multidisciplinary collaborative intervention as the experimental group and usual care as the control group. The intervention is based on behavioural theory and analysis of the steps, roles and barriers relating to rapid assessment for thrombolysis eligibility; it involves a comprehensive range of strategies addressing individual-level and system-level change at each site. The primary outcome is the difference in tPA rates between the two groups post-intervention.     The impact of Covid-19 pandemic on hospices : a systematic integrated review and synthesis of recommendations for policy and practice

    Background: The Covid-19 pandemic resulted in the development of numerous recommendations for practice and policy for specialist palliative care provided by hospices in United Kingdom (UK), as hospices were significantly affected by the pandemic and protections put in place. The aim of this review is to identify and synthesise recommendations or implications for policy and practice that have been generated for adult hospice specialist palliative care during the first 24 months of the Covid-19 pandemic. Methods: AMED, BNI, CINAHL, EMBASE, EMCARE, HMIC, Medline, PsycINFO, PubMed databases were searched for peer-reviewed papers, as well as hand searchers for grey literature. Literature relating to hospices and Covid-19 in the UK were included and a thematic synthesis of recommendations for hospice policy and practice was undertaken. Results: 858 articles were identified with 12 meeting the inclusion criteria. Fifty-eight recommendations or implications were identified: 31 for policy, 27 for practice, and 10 covering both. Recommendations were organised under ten themes. There were several recommendations seeking to secure hospice resources to mitigate the short-term impact of the pandemic, as well as those focused on longer-term implications such as core funding. The impact of the pandemic on the quality of hospice care was the focus for numerous recommendations around improving integration of hospice care in the community, provision of bereavement support and better use of Advance Care Plans (ACP). However, there were significant gaps related to carer visitation in hospices, inequities of palliative care, or hospice-at-home services. Conclusion: The Covid-19 pandemic and protections exposed several ongoing policy and practice needs, especially around hospice resources, while generating novel issues for hospices to address. Significant policy gaps remain to be addressed to mitigate the impact of the pandemic on the quality of hospice specialist palliative care

    Management of Poststroke Hyperglycemia : Results of the TEXAIS Randomized Clinical Trial

    Hyperglycemia in acute ischemic stroke reduces the efficacy of stroke thrombolysis and thrombectomy, with worse clinical outcomes. Insulin-based therapies are difficult to implement and may cause hypoglycemia. We investigated whether exenatide, a GLP-1 (glucagon-like peptide-1) receptor agonist, would improve stroke outcomes, and control poststroke hyperglycemia with minimal hypoglycemia. Methods:The TEXAIS trial (Treatment With Exenatide in Acute Ischemic Stroke) was an international, multicenter, phase 2 prospective randomized clinical trial (PROBE [Prospective Randomized Open Blinded End-Point] design) enrolling adult patients with acute ischemic stroke ≤9 hours of stroke onset to receive exenatide (5 µg BID subcutaneous injection) or standard care for 5 days, or until hospital discharge (whichever sooner). The primary outcome (intention to treat) was the proportion of patients with ≥8-point improvement in National Institutes of Health Stroke Scale score (or National Institutes of Health Stroke Scale scores 0–1) at 7 days poststroke. Safety outcomes included death, episodes of hyperglycemia, hypoglycemia, and adverse event. Results:From April 2016 to June 2021, 350 patients were randomized (exenatide, n=177, standard care, n=173). Median age, 71 years (interquartile range, 62–79), median National Institutes of Health Stroke Scale score, 4 (interquartile range, 2–8). Planned recruitment (n=528) was stopped early due to COVID-19 disruptions and funding constraints. The primary outcome was achieved in 97 of 171 (56.7%) in the standard care group versus 104 of 170 (61.2%) in the exenatide group (adjusted odds ratio, 1.22 [95% CI, 0.79–1.88]; P=0.38). No differences in secondary outcomes were observed. The per-patient mean daily frequency of hyperglycemia was significantly less in the exenatide group across all quartiles. No episodes of hypoglycemia were recorded over the treatment period. Adverse events of mild nausea and vomiting occurred in 6 (3.5%) exenatide patients versus 0 (0%) standard care with no withdrawal. Conclusions:Treatment with exenatide did not reduce neurological impairment at 7 days in patients with acute ischemic stroke. Exenatide did significantly reduce the frequency of hyperglycemic events, without hypoglycemia, and was safe to use.
