    Inflammatory bowel disease nurse specialists for patients on biological therapies: a nationwide Italian survey

    Background Management of inflammatory bowel disease (IBD) patients requires a multidisciplinary approach. Among the working team, the role of IBD nurse is expected to be particularly relevant when managing patients receiving biological therapies. We performed a survey to assess the presence of IBD nurse in centers where patients were receiving biologics. Methods For this Italian nationwide survey a specific questionnaire was prepared. IBD nurse was defined as a nurse directly involved in all phases of biological therapy, from pre-therapy screening, administration and monitoring during therapy, to follow up performed by a dedicated helpline, completed a specific training on biological therapy therapy, and observed international guidelines. Results A total of 53 Italian IBD centers participated in the survey, and 91 valid questionnaires were collected. Overall, 34 (37.4%) nurses could be classified as IBD specialists. IBD nurses had a significantly higher educational level than other nurses, they were more frequently operating in Central or Southern than in Northern Italy, they were working in an Academic center rather than in a General hospital, and in IBD centers with >25 patients on biological therapy. On the contrary, mean age, gender distribution, years of nursing, and years working in the IBD unit did not significantly differ between IBD and other nurses. Conclusions Our nationwide survey showed that the presence of an IBD nurse is still lacking in the majority of Italian IBD centers where patients receive biological therapies, suggesting a prompt implementation

    Introduzione: La colite ischemica, rara complicanza del Lupus Eritematoso Sistemico (LES), può essere determinata dalla colonscopia. Case report: Una donna di 43 anni con LES in trattamento con farmaci immunosoppressivi e prednisone è stata sottoposta a colonscopia per modificazioni dell’alvo, A parte un piccolo polipo metaplasico, l’esame colonscopico ha documentato solo alcune aree eritematose nel sigma e nel colon sinistro. Nelle 4 ore successive alla colonscopia, la paziente ha presentato la comparsa di dolore addominale di tipo colico nei quadranti addominali inferiori e diarrea mucosa seguita da rettoragia secondaria ad una colite ischemica, trattata con successo con infusioni e. v., farmaci antispastici, eparina sodica, antibiotici e nutrizione enterale. Conclusioni: La consapevolezza del rischio di insorgenza, dopo colonscopia, di questa temibile complicanza, in pazienti con patologie del tessuto connettivo, può portare ad una diagnosi e ad un efficace trattamento con conseguente evoluzione favorevole

    Bridging the gap in the symptomatic heart failure patient journey: insights from the Italian scenario

    BackgroundThe prognosis for heart failure (HF) patients remains poor, with a high mortality rate, and a marked reduction in quality of life (QOL) and functional status. This study aims to explore the ongoing needs of HF management and the epidemiology of patients followed by Italian HF clinics, with a specific focus on cardiac contractility modulation (CCM).Research design and methodsData from patients admitted to 14 HF outpatients clinics over 4 weeks were collected and compared to the results of a survey open to physicians involved in HF management operating in Italian centers.ResultsOne hundred and five physicians took part in the survey. Despite 94% of patients receive a regular follow-up every 3-6 months, available therapies are considered insufficient in 30% of cases. Physicians reported a lack of treatment options for 23% of symptomatic patients with reduced ejection fraction (EF) and for 66% of those without reduced EF. Approximately 3% of HF population (two patients per month per HF clinic) meets the criteria for immediate CCM treatment, which is considered a useful option by 15% of survey respondents.ConclusionsDespite this relatively small percentage, considering total HF population, CCM could potentially benefit numerous HF patients, particularly the elderly, by reducing hospitalizations, improving functional capacity and QOL

    Background: Severe acute respiratory syndrome coronavirus-2 (SARS-CoV2)-related pneumonia is associated with venous and arterial thrombosis . Aim of the study was to find-out a new score for predicting thrombosis in patients with SARS-CoV-2. Methods: We included a cohort of 674 patients affected by SARS-CoV-2, not requiring intensive care units, and followed-up during the hospitalization until discharge. Routinary analyses performed at in-hospital admission included also serum albumin and D-dimer while arterial and venous thromboses were the end-points of the study. Results: During the follow-up thrombotic events 110 were registered; patients with thrombotic events were older and had lower albumin and higher D-dimer, compared to thrombotic event-free ones. On multivariable logistic regression with step by stepwise procedure age, serum albumin, D-dimer, were independently associated with thrombotic events. The linear combination of age, D-dimer, albumin allowed to build-up the ADA score, whose AUC was 0.752 (95% CI, 0.708-0.795). ADA score was internally validated by bootstrap sampling procedure giving an AUC of 0.752 (95% CI: 0.708 - 0.794). Conclusions: Combination of age, D-dimer, albumin in the ADA score allows identifying SARS-CoV-2 patients at higher risk of thrombotic events

    La mostra presenta al pubblico – per la prima volta in modo organico – la raccolta vasta e sorprendente che i coniugi statunitensi George Washington Wurts ed Henriette Tower misero insieme a cavallo fra XIX e XX secolo e donarono poi allo Stato italiano, per l’esattezza al museo di Palazzo Venezia, dove tuttora è conservata. Alla base della mostra vi è comunque anche l’idea di restituire il contesto della raccolta Wurts, ovvero quella particolare forma di collezionismo che tra Ottocento e Novecento si legò così intimamente all’Italia, fino a concretizzarsi spesso nella donazione allo Stato di singole opere o di intere raccolte. La mostra illustra le dinamiche del collezionismo, soprattutto anglo-americano, e del mercato internazionale, sullo sfondo dei radicali cambiamenti vissuti in quegli anni dalla giovane nazione italiana e dalla sua nuova capitale, Roma. La costruzione del Vittoriano, iniziato nel 1885 e inaugurato nel 1911 nell’occasione dell’Esposizione che celebrava il cinquantenario dell’Unità d’Italia, diviene l’emblema che caratterizza la città all’alba del Novecento
