El problema del concepto de especie en la filogenia del Oso de las Cavernas
[Abstract] This a short review of a series of concepts daily used in palaentology, trying to connect them with their meaning in other fields of study in Biology. Firstly, it should be pointed out if the concept of species comes from an empiric reality or if it has arisen from the scientific need of assembling. The evolutive concept of speciation, as well as its "methods" seem to be based in the term population. Hence, as meaningful discussions on speciation require a common definition of its end result, namely species, a solution to the tricky issue of defining species probably lies in a deeper knowledge of the processes behind speciation itself
Características clínicas, virológicas e inmunológicas de los nuevos diagnósticos de infección por VIH en el área sanitaria de A Coruña en los últimos 10 años: evolución clínica y respuesta al tratamiento antirretroviral
[Resumen] Objetivos: Describir las tendencias temporales y las características de las nuevas infecciones por VIH en A Coruña en los últimos 10 años con el fin de establecer las mejores estrategias para el diagnóstico, prevención y tratamiento de esta infección en nuestra área médica
Métodos: Se identificaron todos los pacientes diagnosticados de novo por VIH desde 2004 hasta Septiembre de 2013 en un área médica de referencia en A Coruña. Se determinaron los datos epidemiológicos, virológicos, inmunológicos y clínicos en el momento del diagnóstico. La evolución clínica y la respuesta al tratamiento antirretroviral también fueron evaluadas.
Resultados: Se identificaron un total de 521 pacientes durante el período de estudio. Principalmente eran hombres (83%) y su media de edad era de 38 años. En los últimos años el patrón de las vías de transmisión ha cambiado, incrementándose en homosexuales y reduciéndose en usuarios de drogas por vía intravenosa. En el momento del diagnóstico la mitad de los pacientes tenían una media de CD4 <350 células/μL y el 33% presentaba una enfermedad definitoria de SIDA.
Los subtipos no-B se determinaron en el 34,2% de los pacientes y el subtipo F fue el más prevalente. El subtipo F se encontró asociado principalmente a hombres que tienen sexo con hombres en comparación con el subtipo B.
En total, la tasa de transmisión de resistencias durante el período de estudio se ha reducido significativamente en los últimos 5 años. Tras un año desde el comienzo del tratamiento antirretroviral, el 74,6% consiguió una viremia indetectable. El análisis multivariante identificó que tener niveles de ARN-VIH <100.000 copias/mL en el diagnóstico era el único predictor positivo de respuesta al tratamiento. Durante el seguimiento, el 11% de los pacientes murieron. La presencia de una enfermedad definitoria de SIDA se asoció significativamente con la mortalidad.
Conclusiones:
El número de nuevos diagnósticos permanece estable en A Coruña desde 2008. Hay un alta prevalencia de diagnósticos tardíos (51%) y el 33% presentaba una enfermedad definitoria de SIDA. El subtipo no-B más prevalente fue el F (25,5%). Se observó un descenso en las tasas de transmisión de resistencias desde 10,2% a un 2,6% en los últimos 5 años. La mayoría de los pacientes comenzaron tratamiento antirretroviral en el primer año del diagnóstico y la mayoría alcanzaron niveles indetectables de viremia. Estos hallazgos apoyan la necesidad de facilitar el acceso a test diagnóstico para el VIH para reducir la tasa de diagnósticos tardíos de VIH, mejorar la evolución clínica y prevenir su transmisión.[Abstract] Objectives: To describe temporal trend and characteristics of newly HIV diagnosed patients in A Coruña over the last 10 years, to establish the best strategies for HIV diagnosis, prevention and treatment in our medical setting.
Methods: All newly diagnosed patients for HIV infection from 2004 to September 2013 at a reference medical care in A Coruña were recorded. Epidemiological, virological, immunological and clinical data at the time of diagnosis were defined. The clinical outcome and response to antiretroviral therapy were also evaluated.
Results: A total of 531 newly HIV diagnosed patients were identified during the study period. The majority were male (83%) and their mean age was 38 years. In the last years the trend of routes transmission has changed, it has been increased in homosexuals and it has been reduced in intravenous drug users. At the time of HIV diagnosis, half of patients had a median CD4 counts < 350 cells/μL and 33% of them of had an AIDS defining disease.
Non-B subtypes were recognized in 34.2% of patients and subtype F was the most prevalent. Subtype F was predominantly recognized in men who have sex with men compared to subtype B.
Overall, the rate of transmitted drug resistance over the study period was a significant decreased in the last 5 years. After one year of start the antiretroviral treatment, 74.6% achieved undetectable viremia. Multivariate analysis identified HIV-RNA < 100.000 copies/mL at diagnosis as the unique predictor of response to treatment. During follow-up, 11% of patients died. The presence of an AIDS-defining disease at the time of diagnosis was significant associated with mortality.
Conclusions: The number of newly HIV diagnosed patients is stable since year 2008. There is a high prevalence of late diagnosis (51%) and 33% of them had an AIDS defining disease. The most prevalent non-B
subtype was F (25.5%). Decreasing rates of transmission of drug resistance was observed from 10.2% to 2,6% in the last 5 years. Most of patients started antiretroviral therapy in the first year of diagnosis and the majority reached undetectable levels of viremia. Caracterización epidemiológica y virológica en pacientes con infección crónica por el Virus de la Hepatitis C: impacto en el tratamiento con antivirales de acción directa
Programa de Doctorado en Ciencias de la Salud. 5007V01[Resumen] La infección por el Virus de la Hepatitis C (VHC) es un problema grave de salud
pública mundial, que actualmente afecta a más de 71 millones de personas en todo el
mundo, y que lidera una significativa morbilidad y mortalidad. Desde la aprobación de
los nuevos antivirales de acción directa (AADs), el paradigma de la infección por VHC
ha cambiado. Los regímenes con AADs son ahora el nuevo estándar de tratamiento
frente al VHC con tasas de respuesta virológica sostenida (RVS) de más del 90%, de
corta duración y sin apenas efectos adversos asociados. Sin embargo, en un primer
momento, sus elevados costes supusieron una limitación para ofrecer un tratamiento
global a los pacientes con infección por VHC. Esta situación fue resuelta en mayo de
2015 cuando el Sistema Nacional de Salud (SNS) español publicó el Plan Estratégico
para el Abordaje de la Hepatitis C (PEAHC) que establecía los criterios para la
priorización del tratamiento con los nuevos AADs.
A pesar de las altas tasas de curación asociadas a los nuevos AADs superiores al
90%, existe todavía una proporción de pacientes entre un 2-10% que no alcanza una
RVS. Entre las diferentes causas asociadas a esta falta de respuesta están numerosos
factores como el grado de fibrosis hepática, la exposición previa a terapias con IFN, la
carga viral, el genotipo viral o la presencia de mutaciones de resistencia (RASs) en
regiones del genoma viral que son las dianas terapéuticas de los AADs (como NS5A).
Además, las RASs generalmente se asocian a fracaso terapéutico cuando su presencia
se combina con algunos de los demás factores relacionados con menores tasas de RVS.
Todos estos factores han de tenerse en cuenta para optimizar la elección de
tratamiento y su duración, con el fin de asegurar la mejor respuesta posible. En este contexto, los objetivos de este trabajo fueron, en primer lugar,
caracterizar el perfil de la infección crónica por VHC en el área sanitaria de A Coruña, y
evaluar el impacto del PEAHC en el abordaje del tratamiento de la infección en esta
población. En segundo lugar, los objetivos fueron evaluar la prevalencia de RASs
basales en NS5A en pacientes con genotipo 1a (G1a) y genotipo 3 (G3), y evaluar los beneficios de la realización de un estudio de resistencias para optimizar las estrategias
terapéuticas disponibles.
Los resultados obtenidos en el primer estudio, señalaron que el perfil de la
infección crónica por VHC en el área sanitaria de A Coruña se caracterizaba por una
prevalencia mayoritaria en varones (76,2%) con una media de edad de 50 ± 9,5 años,
donde la cirrosis hepática (28,7%) y la co-infección por VIH (60,9%) fueron frecuentes.
A nivel virológico, el virus presentó cargas virales elevadas y el genotipo mayoritario
fue el 1 (66,1%), subtipo 1a (41,5%), seguido del G3 (16,8%). Durante el año 2015 el
52,7% de los pacientes iniciaron tratamiento con una tasa de RVS global superior al
96%. No se observaron diferencias significativas entre esta RVS y factores relacionados
con peor respuesta al tratamiento como cirrosis, co-infección por VIH, genotipo viral, o
exposición previa a tratamiento. De los pacientes que iniciaron tratamiento el 72,9%
fueron reconocidos como prioritarios para el tratamiento frente al VHC de acuerdo al
PEAHC (≥F2, trasplantados o con manifestaciones extrahepáticas), indicando que el
PEAHC ha sido crítico para el avance en la cura frente al VHC en el área sanitaria de A
Coruña. Los resultados obtenidos en el segundo estudio, describieron en el área
sanitaria de A Coruña una baja prevalencia de RASs en la región NS5A en pacientes con
G1a y G3 (5,5% versus 0,0%) así como una alta proporción de pacientes con factores
asociados a menores tasas de RVS (78,9% para G1a y 75,8% para G3). En consecuencia,
las tasas de pacientes que combinaban estos factores con la presencia de RASs fueron
bajas, por lo que en la gran mayoría de los pacientes (G1a más del 94% y G3 el 100%)
el tratamiento podría optimizarse. Es decir, todos ellos podrían ser tratados con
regímenes estándar de 12 semanas sin ribavirina (RBV). De esta forma se concluyó que
el estudio de resistencias en determinadas poblaciones podría ser útil para optimizar
las estrategias de tratamiento, lo que conlleva además un ahorro en los costes de las

pacientes con xenotipo 1a (X1a) e xenotipo 3 (X3), e avaliar os beneficios da realización

unha reducción dos efectos adversos asociados ó uso da RBV.[Abstract] Hepatitis C Virus (HCV) infection is an important healthcare problem affecting
more than 71 million people worldwide, and can lead to significant morbidity and
mortality. Since the approval of Direct Acting Antiviral agents (DAAs), the face of HCV
infection has dramatically changed. DAAs combinations are the new standard of
treatment for HCV with high sustained virological response (SVR) rates of more than
90%, with a short duration and almost without adverse events. Nevertheless, in the
first stages, the high price of these medications has limited the spread of a global
treatment to HCV infected patients. This situation was resolved in May 2015 when the
Spanish National Health Service published a Strategic Plan that established the criteria
for the priorization of new DAAs treatments.
Despite the high cure rates associated with new DAAs greater than 90%, there
are still a proportion of patients that range between 2-10% who do not reach SVR.
Among the different causes associated with the lack of response there are several
factors such as the degree of liver fibrosis, prior IFN-based therapy, viral load, viral
genotype or the presence of resistant mutations (RASs) located in viral genome regions
which are therapeutic targets of DAAs (like NS5A). Indeed, RASs are usually associated
with therapeutic failure when they appear together with other factors related to lower
rates of SVR. All of these factors must be taken into account to optimize treatment
choice and treatment duration, in order to ensure the best patient response. In this context, the objectives of this study were, at first, to characterize the
chronic HCV infection profile in the healthcare area of A Coruña, and to evaluate the
impact of the Spanish Strategic Plan in the treatment of this population. Secondly, the
objectives were to assess the prevalence of baseline RASs in NS5A in patients with
genotypes 1a (G1a) and 3 (G3), and to evaluate the benefits of performing a resistance
study in order to optimize the current therapeutic strategies.
The results obtained in the first study, indicated that the profile of chronic HCV
infection in the healthcare area of A Coruña was characterized by a major prevalence
of males (76,2%) with a median age of 50 ± 9.5 years, where liver cirrhosis (28.7%) and Human Immunodeficiency Virus (HIV) co-infection (60.9%) were frequent. At
virological level, the virus presented high viral loads and the major genotype was 1
(66.1%), subtype 1a 41.5%, followed by G3 (16.8%). During 2015 the 52.7% of patients
started treatment with an overall SVR rate higher than 96%. We did not observed
significant differences between the SVR and other factors related to worse treatment
response such as cirrhosis, HIV co-infection, viral genotype, or prior exposure to
treatment. Of patients who initiated treatment, the 72.9% were recognized as priority
according to the Spanish Strategic Plan (≥F2, transplanted or with extrahepatic
manifestations), indicating that this Plan has been critical for the advance in the cure
against HCV in the healthcare area of A Coruña.
The results obtained in the second study, described in the healthcare area of A
Coruña a low prevalence of RASs in NS5A region in patients with G1a and G3 (5.5%
versus 0.0%) as well as a high proportion of patients with other factors related to lower
SVR rates (78.9% for G1a and 75.8% for G3). As a result, the rates of patients harboring
NS5A RASs in combination with the other factors were low, so in the vast majority of
patients (G1a more than 94% and G3 100%) treatment could be optimized. Hence, all
of them could be treated with standard treatments of 12 weeks without ribavirin
(RBV). In conclusion, resistance studies in specific HCV-infected populations might be
useful to optimize current NS5A-based therapies, so it might afford costs to the
healthcare systems and it also might avoid the adverse effects related to the use of
RBV

[Abstract] In this paper we deal with an useful application of isotopic studies on fossil bone remains. Following δ1 3C and δ1 5N signals recorded in bone collagen, it is possible to classify into the proper taxon some doubtful remains. Thus, three fossil ribs from Liñares site (Galicia, NW Iberian Peninsula) have been correctly classified as belonging to Cervus elaphus L., taking into account both some previous works on its isotopic signatures as well as morphological data
La dieta del Oso de las Cavernas: nueva hipótesis basada en isótopos estables
[Abstract] The Ursus spelaeus diet has always been a controversial topic in Paleontology. Both, morphometric and isotopic studies, have raised the hypothesis of herbivorism for this extinct ursid that belongs to the order Carnivora. However, the specific composition of its diet is still in doubt. In this paper, and dealing with stables isotopes of 1 3C and 1 5 N in bone collagen, we point out the possibility of Ursus spelaeus feeding basically on nitrogen fixing plants, because of its low δ1 5 N signature, the lowest one among the published data from other Pleistocene herbivores
About large bovids (Bovidae, Mammalia) in the NW of Iberian Peninsula
[Abstract] Large bovids (Bos primigenius and Bison priscus) bone remains from NW Iberian Peninsula sites are studied in this paper. Anatomic features in both species are highly similar, which makes difficult these bone fossils to identify. Our main interest in being able to distinguish them is based in one fact: both species are indicators of different palaeoenvironmental conditions and they are bound to be represented in Galician sites. Thus, bison keeps a continuous presence in Cantabrian sities, being the aurochs dominant in Portugal. By analysing many morphological features and carring metric comparisons out, we have confirmed the Bison priscus presence in Galicia, whilst the Bos primigenius one is still in doubt.Xunta de Galicia; XUGA10307B93Xunta de Galicia; XUGA10308A9
Implicaciones paleoecológicas inferidas de la caracterización isotópica (δ13C,δ15N) del colágeno óseo de Ursus spelaeus ROS.-HEIN.
[Abstract] Stable isotopic signatures measured in bone collagen provide with data related to the species diet type. In this paper we compare δ13C and δ15N outcomes in Ursus spelaeus ROS.-HEIN. Bone remains from Liñares site and Cova Eirós site (Galicia, NW of the Iberian Peninsula). Some data on fossil Ursus arctos L. and Pleistocene Cervus elaphus L. from Galician caves are also presented, as a first approach to distinguish paleodiets of different species inferred from their isotopic signatures. Once all data have been analyzed with proper statistical tools and since this work was planned in order to reduce variation in stable isotopic signals caused by metabolic causes, we may assume that the observed differences between both studied groups are exclusively due to environmental factors and show a migration of the cave bear population in the Serra do Courel mountains from higher to lower altitudes because of the transition from warm climatic conditions to colder ones
Vida y muerte de dos mujeres de Brigantium (NW de Iberia) mediante isótopos estables y antropología forense
[Resumen] En este trabajo se obtienen datos antropológicos e isotópicos a partir de los restos óseos de dos
individuos de edad tardorromana a efectos de reconstruir sus condiciones de vida y las causas
de su muerte. Se trata de dos esqueletos femeninos conservados en el Museo Arqueolóxico de A
Coruña datados entre los siglos III a VI dC. Se han aplicado técnicas de antropología forense para
determinar patologías y lesiones que puedan haber afectado a los individuos estudiados. Para el
estudio de paleodieta, se ha extraído colágeno óseo que se analiza mediante espectrometría de masas
de relaciones isotópicas. Las relaciones isotópicas de 13C/12C y 15N/14N obtenidas se comparan con
datos procedentes de otras poblaciones contemporáneas. Los perfiles antropológicos confirman que
se trata de mujeres adultas jóvenes, de posible ancestralidad caucásica, con signos de hiponutrición
e infección crónica y cuyas señales isotópicas del colágeno muestran una importante contribución
de los moluscos en su dieta. Estos resultados concuerdan con los datos arqueológicos disponibles,
reflejando el progresivo empobrecimiento de la ciudad durante la crisis del Bajo Imperio[Abstract] In this paper we obtain anthropological and isotopic data from the skeletal remains of two
individuals of the late Roman age in order to rebuild their lives and causes of death. These are
two female skeletons preserved in the Archaeological Museum of A Coruña dating from III to VI
centuries AD. Forensic anthropology techniques have been applied to identify diseases and injuries
that may have affected the individuals studied. For the palaeodiet study, extracted bone collagen was
analysed by isotope ratios mass spectrometry. The 13C/12C and 15N/14N ratios are compared with data
from other contemporary populations. Anthropological profiles confirm that these are two young
adult females of Caucasian ancestry, with signs of chronic malnutrition and infection. Their isotopic
signature shows a significant contribution of molluscs in their diet. These results are consistent with
the available archaeological data, reflecting the progressive impoverishment of the city during the

Indicios de pastoreo extensivo en el noroeste peninsular durante el dominio suevo
The soil conditions in Galicia (NW of Spain), due to the predominant acid substrate, makes difficult the conservation of archaeozoological remains associated with human settlements. In these cases, the search for alternative areas, such as caves and natural traps in pasturelands, can provide information on the animal resources available.We present here a study about domestic cattle remains found in Cova da Tara or Tara da Triega (Serra do Courel, Lugo), with an age of about 1500 years BP. The bone remains were laid at the time of the Swabian domination of peninsular northwest. These animals would come alive to the cave, from which it follows that roamed freely around, indicating an extensive livestock management. The tooth wear and the study of stable isotopes in bone collagen reflect a little abrasive diet, probably shrub, matching the vegetation of mountain areas. The average height calculated for these animals is small, similar to that described for pre-Roman cattle.Las condiciones edáficas de Galicia (NW de España), debido al sustrato ácido predominante, dificultan la conservación de los restos arqueozoológicos asociados a los asentamientos humanos. En estos casos, la búsqueda en zonas alternativas, como cuevas y trampas naturales en las zonas de pastoreo, puede aportar información sobre los recursos animales disponibles.Se presenta aquí un estudio de los restos de ganado vacuno doméstico encontrados en Cova da Tara o Tara da Triega (Serra do Courel, Lugo). Con una edad de alrededor de 1500 años, los huesos corresponderían con la época de la dominación sueva del Noroeste peninsular. Estos animales habrían llegado vivos a la cueva, de donde se deduce que deambulaban libremente en los alrededores, lo que señala un tipo de pastoreo extensivo. El desgaste dentario y el estudio de isótopos estables en el colágeno óseo reflejan una alimentación poco abrasiva, probablemente arbustiva, concordando con la de zonas de montaña. La talla media calculada para estos animales es pequeña, similar a la descrita para ganado vacuno prerromano
Trends on epidemiological, virological, and clinical features among newly diagnosed HIV-1 persons in Northwest Spain over the last 10 years
[Abstract] To describe temporal trend and characteristics of newly HIV-diagnosed patients in a medical care area in Northwest Spain over the last 10 years. All newly diagnosed patients for HIV-infection from 2004 to 2013 at a reference medical care area in Northwest of Spain were identified. Epidemiological, virological, immunological, and clinical data, as well as HIV genotype and drug resistance information were recorded. A total of 565 newly HIV-diagnosed patients were identified. The number of new cases increased in the last 5 years (66 cases/year). Overall, 53.1% had a median CD4 counts < 350 cells/µl and 33.6% had an AIDS defining criteria. Non-B variants were found in 34.4% of patients being subtype F (25.8%) the most common non-B subtype. The rate of transmitted drug resistance (TDR) over the study period was 3.7%, but a decreased to 2.6% was observed in the last 5 years. The most prevalent TDR mutations were: T215 revertants (1.5%), K219QENR (1.2%), for NRTIs; K103N (1.9%), for NNRTIs; L90M (0.3%), for PIs. Overall, 73.2% of patients started antiretroviral treatment and 9.9% of patients died during follow-up. The number of newly HIV diagnosed patients increased since year 2009. There is a high prevalence of late diagnosis (53%) and 33% had an AIDS defining criteria. Interestingly, the most prevalent non-B subtype in our population was F (25.8%).
