    A phase II dose‑escalation trial of perioperative desmopressin (1‑desamino‑8‑D‑arginine vasopressin) in breast cancer patients

    Desmopressin (dDAVP) is a well-known peptide analog of the antidiuretic hormone vasopressin, used to prevent excessive bleeding during surgical procedures. dDAVP increases hemostatic mediators, such as the von Willebrand factor (vWF), recently considered a key element in resistance to metastasis. Studies in mouse models and veterinary trials in dogs with locally-advanced mammary tumors demonstrated that high doses of perioperative dDAVP inhibited lymph node and early blood-borne metastasis and significantly prolonged survival. We conducted a phase II dose-escalation trial in patients with breast cancer, administering a lyophilized formulation of dDAVP by intravenous infusion in saline, 30–60 min before and 24 h after surgical resection. Primary endpoints were safety and tolerability, as well as selection of the best dose for cancer surgery. Secondary endpoints included surgical bleeding, plasma levels of vWF, and circulating tumor cells (CTCs) as measured by quantitative PCR of cytokeratin-19 transcripts. Only 2 of a total of 20 patients experienced reversible adverse events, including hyponatremia (grade 4) and hypersensitivity reaction (grade 2). Reactions were adequately managed by slowing the infusion rate. A reduced intraoperative bleeding was noted with increasing doses of dDAVP. Treatment was associated with higher vWF plasma levels and a postoperative drop in CTC counts. At the highest dose level evaluated (2 μg/kg) dDAVP appeared safe when administered in two slow infusions of 1 μg/kg, before and after surgery.     Como resultado de un viaje etnobotánico al sudoeste de la cuenca amazónica, se colectó información sobre 50 plantas utilizadas como medicinales por los Ese'eja, un grupo de amerindios perteneciente a la familia lingüística takana que habita en la región de Madre de Dios, Perú. En este trabajo se describen las prácticas shamánicas de este pueblo, que comprenden el uso de plantas medicinales y rituales. Se relatan fragmentos del uso del "ayahuasca" en ritos de iniciación shamáhnica y curación. Se estudió la actividad biológica de diferentes extractos de aquellas plantas de mayor uso y disponibilidad. Los bioensayos empleados fueron: el bioensayo de citotoxicidad de Artemia salina, el bioensayo de inhibición del crecimiento de las raíces de los granos de trigo y el bioensayo de metil green-DNA. Se discuten los resultados obtenidos y su posible correlación con el uso etnomédico de las plantas estudiadas.Information on 50 medicinal plants used by the Ese'eja, a group of amerindians belonging to the takana linguistic family that lives in Madre de Dios, Perú, was collected during an ethnobotanical expedition to south-western Amazonia. A description of shamanic practices is given to understand better the position of health practices in this culture, which includes the use of medicinal and ritual plants. Fragments of "ayahuasca" ritual in shamanic initiation and in healing rituals are presented. Different extracts of plants were studied for bioactivity according to their use and accessibility. The bioassays used were: the brine shrimp citotoxicity assay, the wheat rootlet growth inhibition assay and the DNA-methyl green assay.     Structure-activity relationship of 1-desamino-8-D-arginine vasopressin as an antiproliferative agent on human vasopressin V2 receptor-expressing cancer cells

    The synthetic nonapeptide 1-desamino-8-D-arginine vasopressin (dDAVP) can reduce tumor cell growth through agonist action on the vasopressin V2 receptor. A structure-antiproliferative activity relationship analysis of dDAVP was performed using the alanine scanning technique on the aggressive MDA-MB-231 human breast carcinoma cell line. The results from this analysis demonstrated that the amino acids located at the loop of dDAVP are important for the antiproliferative activity of dDAVP, highlighting the key role of the N-terminal region of the peptide in the interaction with the tumor cell surface receptor.
