    Clinical significance in the number of involved lymph nodes in patients that underwent surgery for pathological stage III-N2 non-small cell lung cancer

    <p>Abstract</p> <p>Purpose</p> <p>This study investigated whether the number of involved lymph nodes is associated with the prognosis in patients that underwent surgery for pathological stage (p-stage) III/N2 NSCLC.</p> <p>Subjects</p> <p>This study evaluated 121 patients with p-stage III/N2 NSCLC.</p> <p>Results</p> <p>The histological types included 65 adenocarcinomas, 39 squamous cell carcinomas and 17 others. The average number of dissected lymph nodes was 23.8 (range: 6-55). The average number of involved lymph nodes was 5.9 (range: 1-23). The 5-year survival rate of the patients was 51.0% for single lymph node positive, 58.9% for 2 lymph nodes positive, 34.2% for 3 lymph nodes positive, and 30.0% for 4 lymph nodes positive, and 20.4% for more than 5 lymph nodes positive. The patients with either single or 2 lymph nodes positive had a significantly more favorable prognosis than the patients with more than 5 lymph nodes positive. A multivariate analysis revealed that the number of involved lymph nodes was a significant independent prognostic factor.</p> <p>Conclusion</p> <p>Surgery appears to be preferable as a one arm of multimodality therapy in p-stage III/N2 patients with single or 2 involved lymph nodes. The optimal incorporation of surgery into the multimodality approach therefore requires further clinical investigation.</p

    Accuracy and prognostic impact of a vessel invasion grading system for stage IA non-small cell lung cancer.

    BACKGROUND: Despite the recognition that lymphatic and blood vessel invasion is an important prognostic factor in lung cancer, there is no common definition for pathological evaluation of vessel invasion. The aim of the present study was to determine whether D2-40 immunostaining can increase the accuracy of detection of lymphatic vessel invasion and whether our new grading system of vessel invasion by "degree" could be used instead of conventional evaluation by "presence" for pathological stage IA non-small cell lung cancer (NSCLC). METHODS: The vessel invasion classification was re-evaluated in 221 recent paraffin-embedded sections of p-stage IA NSCLC stained by Hematoxylin-Eosin (HE), Elastica-Van-Gieson (EVG), and D2-40. RESULTS: After re-assessment using D2-40 immunostaining, 41.2% (31 of 75) of ly1 cases by HE/EVG changed to ly0, and 14.9% (17 of 114) of ly0 cases by HE/EVG changed to ly1. Overall, 4 of 28 ly2 cases on conventional staining were changed to ly1, and 2 were changed to ly0 using D2-40 immunostaining. When the patients were divided into two groups by the presence of vessel invasion (v/ly0 vs. 1, 2, 3), there was no significant difference in cancer-specific survival (p=0.1107, 0.0875, respectively), while when they were divided according to degree of vessel invasion (v/ly0, 1 vs. 2, 3), there was a statistically significant difference (p=0.0038, p=0.0002, respectively). On multivariate analysis, lymphatic vessel invasion had a significant impact on cancer-specific survival (p=0.0061). CONCLUSION: Our results suggest that D2-40 immunostaining provides a precise diagnosis of lymphatic vessel invasion, and our new grading system of vessel invasion by "degree" is accurate and has prognostic value in early lung cancer

    69歳男.直径約30cmの左側腹部腫瘤の診断及び治療の為, 当科へ紹介された.血中ノルエピネフリン値及びその尿中代謝産物の高値, CTスキャンにより巨大な嚢胞性後腹膜腫瘤, 大動静脈リンパ節の腫大が認められ, 悪性褐色細胞腫と診断.血行動態学的に循環血液量の低下と血管抵抗値の上昇が認められた為, 血管拡張剤の使用と共に循環血液量の補充が行われた.これらにより血行動態は正常化し, 腫瘍摘除術が施行された.病理学的に, リンパ節転移を伴う褐色細胞腫と診断され, 腫瘍重量は5, 930gであったA 69-year-old man was referred to our hospital for a left abdominal tumor measuring about 30 cm in diameter. Laboratory examination revealed an elevation of norepinephrine in plasma and of its metabolites in urine. CT scan disclosed a huge cystic tumor in the retroperitoneal space and an enlarged aortocaval lymph node, suggesting a diagnosis of malignant pheochromocytoma. The hemodynamic studies showed low blood volume and high vascular resistance, and therefore, he was treated with vasodilators and volume expansion. His hemodynamic status normalized and a complete excision was performed. Pathological examination revealed that the patient had a pheochromocytoma with metastasis to a lymph node. The total weight of the tumor was 5, 930 g. Since pheochromocytomas can become a large with a risk of malignancy, they should be surgically excised as completely as possible with further treatment after making a definite diagnosis

    CPP-GMR of Co/Cu Multilayered Nanowires Electrodeposited into Anodized Aluminum Oxide Nanochannels with Large Aspect Ratio

    Co/Cu multilayered nanowires with alternating Co and Cu layers of 10 nm in thickness were electrodeposited into extremely long nanochannels of anodized aluminium oxide thick films with 60 μm in thickness. Growth rate of Co/Co multilayered nanowires was around 30 nm/sec and the cylindrical shape was precisely transferred from the nanochannels to the nanowires and the aspect ratio reached up to ca. 1,000 to 1. Co/Cu multilayered nanowires with diameter 60 nm were easily magnetized to the long axis direction of nanowires due to the uni-axial shape anisotropy. 10.5 % of perpendicular giant magnetoresistance effect was observed in Co/Cu multilayered nanowires with 3,000 Co/Cu bilayers.Magnetic Materials, Processes, and Devices 12 - PRiME 2012; Honolulu, HI; United States; 7 October 2012 ~ 12 October 201

    A study of a phenomena deviated from random feature in EAS energy around 100TeV with new Kinki Array

    [Abstract]An EAS observation system was rebuilt in July 1998 at Kinki University which consists of seven 0.25m2 plastic scintillators with fast PMTs. The system works well to detect EASs energy around 100TeV at sea level. In the observation of more than 103 hours, the detector records some phenomenae deviated from average feature in triggering rate, which means each series of the events has extremely small expected frequency in short term data acquisition period. For number of total events this set of consequtive events with upper limit of significant level of 3.6 × 10-2 was found. Although this is not strong evidence deviating from random Poisson distribution, but it has a value of 5σ from average rate. The new Kinki Array is reported briefly and this phenomena is also reported in detail
