Disruption of smooth pursuit eye movements in cirrhosis: relationship to hepatic encephalopathy and its treatment.
Smooth pursuit eye movements (SPEM) are the conjugate movements used to track the smooth trajectory of small dots. Jerky or 'saccadic' ocular pursuit has been reported in patients with cirrhosis, but no formal assessment of SPEM has ever been undertaken. The aim of this study was to evaluate SPEM in patients with cirrhosis and varying degrees of hepatic encephalopathy. The patient population comprised 56 individuals (31 men, 25 women) of mean age 51.1 (range, 25-70) years, with biopsy-proven cirrhosis, classified, using clinical, electroencephalographic, and psychometric variables, as either neuropsychiatrically unimpaired or as having minimal or overt hepatic encephalopathy; patients were further categorized in relation to their treatment status. The reference population comprised 28 healthy volunteers (12 men, 16 women) of mean age 47.3 (range, 26-65) years. SPEM was assessed using an electro-oculographic technique. Visual inspection of the SPEM recordings showed clear disruption of smooth pursuit in the patients with minimal hepatic encephalopathy, and more pronounced disruption, if not complete loss, of smooth pursuit in patients with overt hepatic encephalopathy. The differences observed in quantifiable SPEM indices between the healthy volunteers/unimpaired patients and those with overt hepatic encephalopathy were significant (P < .05). In conclusion, SPEM performance is impaired in patients with hepatic encephalopathy in parallel with the degree of neuropsychiatric disturbance: the pathophysiology of these changes is unknown, but retinal, extrapyramidal, and attentional abnormalities are likely to play a role. Treatment status confounds the classification of neuropsychiatric status and should be taken into account when categorizing these patients
Robustness to misalignment of low-cost, compact quantitative phase imaging architectures
Non-interferometric approaches to quantitative phase imaging could enable its application in low-cost, miniaturised settings such as capsule endoscopy. We present two possible architectures and both analyse and mitigate the effect of sensor misalignment on phase imaging performance. This is a crucial step towards determining the feasibility of implementing phase imaging in a capsule device. First, we investigate a design based on a folded 4f correlator, both in simulation and experimentally. We demonstrate a novel technique for identifying and compensating for axial misalignment and explore the limits of the approach. Next, we explore the implications of axial and transverse misalignment, and of manufacturing variations on the performance of a phase plate-based architecture, identifying a clear trade-off between phase plate resolution and algorithm convergence time. We conclude that while the phase plate architecture is more robust to misalignment, both architectures merit further development with the goal of realising a low-cost, compact system for applying phase imaging in capsule endoscopy
The Effective Fragment Molecular Orbital Method for Fragments Connected by Covalent Bonds
We extend the effective fragment molecular orbital method (EFMO) into
treating fragments connected by covalent bonds. The accuracy of EFMO is
compared to FMO and conventional ab initio electronic structure methods for
polypeptides including proteins. Errors in energy for RHF and MP2 are within 2
kcal/mol for neutral polypeptides and 6 kcal/mol for charged polypeptides
similar to FMO but obtained two to five times faster. For proteins, the errors
are also within a few kcal/mol of the FMO results. We developed both the RHF
and MP2 gradient for EFMO. Compared to ab initio, the EFMO optimized structures
had an RMSD of 0.40 and 0.44 {\AA} for RHF and MP2, respectively.Comment: Revised manuscrip
NDE of Polymer Composites Using Magnetic Resonance Techniques
Polymer based materials have become increasingly important in structural applications primarily due to their high strength to weight ratio. As the use of polymer-based composites has increased, so has the need for reliable non-destructive evaluation techniques. In this paper, a new NDE method for these materials is proposed. The technique relies on the observation of an electron paramagnetic resonance (epr) absorption at the site of damage in a polymer. Using applied magnetic field gradients the physical location of damage can be discerned and an image of the damage site can be obtained. This should allow the detection of cracks and delaminations with high resolution, good sensitivity and good contrast

Structured approach for Reporting In health education of Evidence Synthesis
Background
Evidence synthesis techniques in healthcare education have been enhanced through the activities of experts in the field and the Best Evidence Medical Education (BEME) collaborative. Despite this, significant heterogeneity in techniques and reporting of healthcare education systematic review still exist and limit the usefulness of such reports. The aim of this project was to produce the STORIES (STructured apprOach to the Reporting In healthcare education of Evidence Synthesis) statement to offer a guide for reporting evidence synthesis in health education for use by authors and journal editors.
Methods
A review of existing published evidence synthesis consensus statements was undertaken. A modified Delphi process was used. In stage one, expert participants were asked to state whether common existing items identified were relevant, to suggest relevant texts and specify any items they feel should be included. The results were analysed and a second stage commenced where all synthesised items were presented and participants asked to state whether they should be included or amend as needed. After further analysis, the full statement was sent for final review and comment.
Results
Nineteen experts participated in the panel from 35 invitations. Thirteen text sources were proposed, six existing items amended and twelve new items synthesised. After stage two, 25 amended consensus items were proposed for inclusion. The final statement contains several items unique to this context, including description of relevant conceptual frameworks or theoretical constructs, description of qualitative methodologies with rationale for their choice and presenting the implications for educators in practice of the results obtained.
Conclusions
An international expert panel has agreed upon a consensus statement of 25 items for the reporting of evidence synthesis within healthcare education. This unique set of items is focused on context, rather than a specific methodology. This statement can be used for those writing for publication and reviewing such manuscripts to ensure reporting supports and best informs the wider healthcare education community
Aspergillus mastoiditis, presenting with unexplained progressive otalgia, in an immunocompetent (older) patient
Aspergillus mastoidits and skull-base osteomyelitis are extremely rare, even in immunocompromised patients. We report a case of an 81-year-old immunocompetent man, who underwent a mastoidectomy because of unexplained, progressive otalgia in spite of a noninflamed and air-containing middle-ear space. Histopathology yielded Aspergillus fumigatus. When confronted with otitis with an unexpected clinical course a high index of suspicion is required to facilitate early diagnosis and appropriate therapy of a potential lethal Aspergillus infection, even in immunocompetent patients. This seems to be more so in older patients with an open middle-ear cavity and/or when there is facial nerve involvement
Two-channel Kondo effect and renormalization flow with macroscopic quantum charge states
Many-body correlations and macroscopic quantum behaviors are fascinating
condensed matter problems. A powerful test-bed for the many-body concepts and
methods is the Kondo model which entails the coupling of a quantum impurity to
a continuum of states. It is central in highly correlated systems and can be
explored with tunable nanostructures. Although Kondo physics is usually
associated with the hybridization of itinerant electrons with microscopic
magnetic moments, theory predicts that it can arise whenever degenerate quantum
states are coupled to a continuum. Here we demonstrate the previously elusive
`charge' Kondo effect in a hybrid metal-semiconductor implementation of a
single-electron transistor, with a quantum pseudospin-1/2 constituted by two
degenerate macroscopic charge states of a metallic island. In contrast to other
Kondo nanostructures, each conduction channel connecting the island to an
electrode constitutes a distinct and fully tunable Kondo channel, thereby
providing an unprecedented access to the two-channel Kondo effect and a clear
path to multi-channel Kondo physics. Using a weakly coupled probe, we reveal
the renormalization flow, as temperature is reduced, of two Kondo channels
competing to screen the charge pseudospin. This provides a direct view of how
the predicted quantum phase transition develops across the symmetric quantum
critical point. Detuning the pseudospin away from degeneracy, we demonstrate,
on a fully characterized device, quantitative agreement with the predictions

A new method for determining physician decision thresholds using empiric, uncertain recommendations
<p>Abstract</p> <p>Background</p> <p>The concept of risk thresholds has been studied in medical decision making for over 30 years. During that time, physicians have been shown to be poor at estimating the probabilities required to use this method. To better assess physician risk thresholds and to more closely model medical decision making, we set out to design and test a method that derives thresholds from actual physician treatment recommendations. Such an approach would avoid the need to ask physicians for estimates of patient risk when trying to determine individual thresholds for treatment. Assessments of physician decision making are increasingly relevant as new data are generated from clinical research. For example, recommendations made in the setting of ocular hypertension are of interest as a large clinical trial has identified new risk factors that should be considered by physicians. Precisely how physicians use this new information when making treatment recommendations has not yet been determined.</p> <p>Results</p> <p>We derived a new method for estimating treatment thresholds using ordinal logistic regression and tested it by asking ophthalmologists to review cases of ocular hypertension before expressing how likely they would be to recommend treatment. Fifty-eight physicians were recruited from the American Glaucoma Society. Demographic information was collected from the participating physicians and the treatment threshold for each physician was estimated. The method was validated by showing that while treatment thresholds varied over a wide range, the most common values were consistent with the 10-15% 5-year risk of glaucoma suggested by expert opinion and decision analysis.</p> <p>Conclusions</p> <p>This method has advantages over prior means of assessing treatment thresholds. It does not require physicians to explicitly estimate patient risk and it allows for uncertainty in the recommendations. These advantages will make it possible to use this method when assessing interventions intended to alter clinical decision making.</p
Dealing with missing standard deviation and mean values in meta-analysis of continuous outcomes: a systematic review
Background: Rigorous, informative meta-analyses rely on availability of appropriate summary statistics or individual
participant data. For continuous outcomes, especially those with naturally skewed distributions, summary
information on the mean or variability often goes unreported. While full reporting of original trial data is the ideal,
we sought to identify methods for handling unreported mean or variability summary statistics in meta-analysis.
Methods: We undertook two systematic literature reviews to identify methodological approaches used to deal with
missing mean or variability summary statistics. Five electronic databases were searched, in addition to the Cochrane
Colloquium abstract books and the Cochrane Statistics Methods Group mailing list archive. We also conducted cited
reference searching and emailed topic experts to identify recent methodological developments. Details recorded
included the description of the method, the information required to implement the method, any underlying
assumptions and whether the method could be readily applied in standard statistical software. We provided a
summary description of the methods identified, illustrating selected methods in example meta-analysis scenarios.
Results: For missing standard deviations (SDs), following screening of 503 articles, fifteen methods were identified in
addition to those reported in a previous review. These included Bayesian hierarchical modelling at the meta-analysis
level; summary statistic level imputation based on observed SD values from other trials in the meta-analysis; a practical
approximation based on the range; and algebraic estimation of the SD based on other summary statistics. Following
screening of 1124 articles for methods estimating the mean, one approximate Bayesian computation approach and
three papers based on alternative summary statistics were identified. Illustrative meta-analyses showed that when
replacing a missing SD the approximation using the range minimised loss of precision and generally performed better
than omitting trials. When estimating missing means, a formula using the median, lower quartile and upper quartile
performed best in preserving the precision of the meta-analysis findings, although in some scenarios, omitting trials
gave superior results.
Conclusions: Methods based on summary statistics (minimum, maximum, lower quartile, upper quartile, median)
reported in the literature facilitate more comprehensive inclusion of randomised controlled trials with missing mean or
variability summary statistics within meta-analyses
